Background: Despite numerous studies of women having children later in life, evidence of the relationship between maternal factors and newborn outcomes in Central and Eastern European countries is limited. This study aimed to examine the association between maternal age, biological determinants, including parity and sex of the newborn, demographic and social background, and birth weight in 3.8 million singleton live births in Poland. Methods: The effect of maternal age on birth weight (in grams and Z-scores) adjusted for confounders was assessed using Generalized Linear Models. Results: The mean (±SD) birth weights of neonates born to primiparous women and multiparous women were 3356.3 ± 524.9 g and 3422.7 ± 538.6 g, respectively, which corresponded to a Z-score of −0.07 ± 0.96 and 0.14 ± 1.00, respectively (p ≤ 0.001). After controlling for biological, demographic, and social factors, a significant decrease in birth weight was found for primiparous women of the age group ≥30 years and multiparous women aged ≥35 years compared to the age group of 25–29 years. The lowest neonatal birth weight was observed in the case of women aged ≥45 years. Confounders did not affect birth weight Z-scores among primiparous women, whereas among multiparous women, together with educational factors, they reversed Z-scores from positive to negative values. The lower birth weight of neonates was overall associated with lower maternal education. Conclusions: Regardless of parity, advanced maternal age is strongly associated with a decreased neonatal birth weight, implying complications in early pregnancy and the antenatal period as well as obstetric complications. Counseling to support women’s family planning decisions and improving women’s education during their reproductive age may help to alleviate unfavorable newborn outcomes.
Raktažodžiai: pagyvenę žmonės, traumos, nukritimai, sužalojimų prevencija. SantraukaTirti pagyvenę (65+) Lietuvos žmonės, kurie gydė-si stacionare dėl patirtos traumos nukritus. Taikyta apklausa raštu, analizuota 312 užpildytų anketų. Apklausoje pateikti sociodemografiniai klausimai ir klausimai apie nukritimo vietą, laiką, priežastis, aplinkybes, sveikatos ypatumus, nukritimų prevenciją. Duomenys buvo statistiškai apdoroti SPSS 20.0 statistinių programų paketu. Taikytas chi kvadrato kriterijus. Statistiniam reikšmingumui patvirtinti pasirinktas reikšmingumo lygmuo p<0,05. Stacionare dėl traumų po nukritimo gydėsi reikš-mingai daugiau moterų, nei vyrų. Dauguma (95 proc.) stacionare dėl nukritimo gydytų pacientų gydėsi kaulų lūžius. 89,1 proc. pagyvenusių žmonių nukritimų įvyko patalpose. Namuose nukrito didžioji dauguma pacientų. Namuose 28 proc. nukritimų įvyko kambaryje, 21 proc. -vonios patalpoje, 19 proc. -koridoriuje, 14 proc. -ant laiptų ar laiptinė-je, 13 proc. -virtuvėje. Dažniausiai pagyvenę žmo-nės nukrenta ryte ir vakare. 24 proc. respondentų nukrito užkliuvę už kliūties, 23 proc. -paslydę ant šlapių, slidžių grindų, 20 proc. -prarado pusiausvyrą eidami lygiu pagrindu. 83 proc. pagyvenusio amžiaus žmonių nuolat vartoja kokius nors vaistus, 62 proc. turi regėjimo sutrikimų, 72 proc. turi kaulų-raumenų sistemos problemų, 41 proc. -klausos sutrikimų. Dauguma (71 proc.) nukentėjusiųjų nėra girdėję, nesidomi nukritimų prevencija. Nukritimų prevencija reikšmingai daugiau domisi moterys, nei vyrai. Daugiausia žinių apie sužalojimų, nukritimų prevenciją pacientai gauna iš žiniasklaidos ir medicinos ar sveikatos darbuotojų. 42 proc. pagyvenusių žmonių skiria laiko pasivaikščiojimui. Reguliariai daro mankštą tik 19,1 proc. pagyvenusių žmonių. ĮvadasPagyvenę žmonės yra tarp pažeidžiamiausių visuomenės grupių, tad jų sveikatos ir saugos klausimams turi būti skiriamas padidintas dėmesys. Mokslininkų nuomone, prognozuojama, kad iki 2050-ųjų metų gimstamumas Europoje didės nežymiai, vidutinė tikėtina gyvenimo trukmė ilgės, tad senyvo amžiaus žmonių neišvengiamai daugės. Taigi svarbu užtikrinti pagyvenusių žmonių sveikatą ir sveikesnę senatvę [1].Europos regione 65 metų ir vyresnių žmonių mirčių nuo išorinių priežasčių struktūroje pirmauja savižudybės, kritimai ir eismo įvykiai, be to, pagyvenusių žmonių mirtingumas nuo sužalojimų yra didesnis nei kitose amžiaus grupėse [1,5]. Dėl sveikatos problemų ir regėjimo, eisenos bei pusiausvyros sutrikimų vyresni nei 65 metų žmonės dažniau patiria traumas negu jaunesni. Be to, dėl osteoporozės ir prastos sveikatos jų traumos paprastai sunkesnės, dažniau sukelia mirtinas komplikacijas, po jų ilgiau sveikstama [6]. Pablogėjęs regėjimas, klausa, eisenos bei pusiausvyros sutrikimai, osteoporozė sąlygoja pagyvenusių žmonių sužalojimų gausą [7]. Tyrimai, nagrinėjantys pagyvenusių žmonių sveikatą, mirčių, susirgimų ir sužalojimų priežastis ir jų prevencijos būdus, įgalina stiprinti pagyvenusių žmonių sveikatą, saugą, ge-
<p class="Body">For many years in Lithuania the rate of suicide mortality has been remarkable in magnitude, especially in the young population. Social and public health determinants, risk behaviour issues, prevention and control of suicide in teenagers and young people needs urgent attention.</p><p class="Body">The aim of the study was to investigate the self-reported perception of young people - the university students concerning suicide, the myths and reality on this phenomenon, and the links between suicidal behaviour and endorsement of myths about suicide. The self-reported survey was conducted in the university settings. The results of the survey showed that young people lack knowledge about the phenomenon of suicide and suicidal behaviour, and they still believe myths about suicide. The study revealed that one-fifth of young people had suicidal thoughts, and boys along with urban residents and suicidal family were at higher risk. Half of the students surveyed self-reported suicidal peers in their environment. Young people lack knowledge on suicide prevention, and preventive events. The study revealed that raising awareness, obtaining knowledge, and providing information to young people on the phenomenon of suicide, suicidal behaviour, and warning signs of possible attempt and suicide risk are the priority issues. Suicide prevention activities on individual, community, local and national levels need to be addressed.</p>
