Aim. The purpose of the work was the development of a machine learning model for diagnosing the stage of liver fibrosis in patients with chronic viral hepatitis C according to the data of routine clinical examination. Materials and methods. A total of 1240 patients with chronic viral hepatitis C was examined. A set of data obtained from 689 patients balancing by the stage of liver fibrosis was used for developing and testing machine learning models. 9 routine clinical parameters were selected as the most important predictors for determining the likelihood of liver fibrosis the 3–4 stages presence: age, height, weight and body mass index of the patient, the number of platelets in the clinical blood test, levels of alanine transaminase, aspartate transaminase, gamma-glutamyltransferase, and total bilirubin in a biochemical blood test. Results. The accuracy of the developed method for determining the 3–4 stages of liver fibrosis in patients with chronic viral hepatitis C in comparison with the «gold standard» of diagnosis (liver biopsy) was 80.56% (95% CI: 69.53–88.94%), sensitivity — 66.67%, specificity — 94.44%. Conclusion. The developed method is an alternative to more expensive and geographically inaccessible studies. The method does not require the purchase of additional equipment or software, as well as additional laboratory tests, when used in real clinical practice. The introduction of the method into clinical practice can help to solve the problem of low material and territorial availability of diagnostic tests and allow determining the stage of liver fibrosis in patients with chronic viral hepatitis C.
Цель. Сравнительное изучение клинической эффективности и безопасности применения противовирусных препаратов Триазавирин ® и Тамифлю ® в лечении больных гриппом. Материалы и методы. Всего в исследование были включены 127 пациентов в возрасте от 18 до 65 лет с лабораторно подтвержденным диагнозом «грипп средней степени тяжести». У абсолютного большинства больных (90,6%, n = 114) был диагностирован грипп типа A. Основную группу составили 82 пациента, получавших препарат Триазавирин ® по 1 капсуле (250 мг) 3 раза в день в течение 5 сут. Группу сравнения составили 45 пациентов, получавших препарат Тамифлю ® по 1 капсуле (75 мг) 2 раза в день в течение 5 сут. Эффективность и безопасность исследуемых противовирусных препаратов оценивались на основании данных клинико-лабораторного обследования пациентов в динамике заболевания. Результаты. Среди пациентов, получавших препарат Триазавирин ® , показатели времени до выздоровления и времени до исчезновения лихорадки, головной боли и миалгии были меньше, чем среди больных, получавших препарат Тамифлю ®. У пациентов, принимавших препарат Триазавирин ® , частота повторного выявления РНК вирусов гриппа на 5-й день лечения была ниже, чем у больных группы сравнения. Пациенты основной группы реже требовали назначения симптоматических лекарственных средств, а длительность их применения была меньше, чем среди больных, получавших препарат Тамифлю ®. Заключение. Эффективность препарата Триазавирин ® в терапии больных гриппом по ряду изучаемых показателей превосходит эффективность препарата Тамифлю ®. Использование препарата Триазавирин ® в лечении больных гриппом является безопасным и хорошо переносимым.

The results of numerous preclinical and clinical trials suggest that the kagocel substance is highly safe and that it is appropriate to use the drug for the treatment and prevention of influenza and acute respiratory viral infections of another etiology.
