Recurrent respiratory papillomatosis (RRP) is a rare disease caused by human papillomavirus. Aggressive forms of RRP require repeated cytoreductive surgery to restore airway patency. Tracheal disease is even less common and lung parenchyma is involved in less than 1% of patients. We present reports of three cases of RRP with progressive lung disease in adult patients.
Recurrent respiratory papillomatosis (RRP) is a rare disease with an unpredictable course. The etiology is associated with the human papillomavirus (HPV). Malignant transformation of papillomas is extremely rare.
Two clinical cases of RRP malignization are presented. In the first case, 22-year-old women with a long history of RRP and the lower respiratory tract lesions, squamous cell lung cancer developed. Second case - development of squamous cell carcinoma from the tracheal papilloma in a 35-year-old patient. Aggressive local endobronchial treatment was carried out; however, the prognosis remains pessimistic due to the absence of etiopathogenetic therapy for RRP. Careful monitoring including HPV typing is mandatory, since infection with aggressive HPV strains leads to rapid proliferation of papillomas and malignant transformation.
Проведено клинико-морфологическое исследование 15 пациентов с папилломатозом трахеи. Для определения пролиферативной ак-
тивности и апоптоза проведено иммуногистохимическое (ИГХ) исследование с антителами Ki 67, p 53, Bcl-2, а также полимеразная цеп-
ная реакция (ПЦР). Микроскопически на фоне плоскоклеточной метаплазии обнаружены клетки с митотической активностью, гипер-
хроматозом, полиморфизмом ядер, дискератозом. Выявлены атипические формы митоза, базальная атипия и начальные стадии инва-
зии с разрушением базальной мембраны в 2 случаях. Экспрессия вируса папилломы человека (ВПЧ) обнаружена у 13 пациентов, ВПЧ
16-го и 18-го типов - у 2 пациенток с рецидивирующим течением папилломатоза. Выраженная экспрессия (+++) Ki67, p53, Bcl-2 вы-
явлена в 2 случаях с клеточным атипизмом и озлокачествлением папилломы. Полученные данные подтверждают значение ВПЧ как ос-
новного этиологического фактора в возникновении папилломы трахеи. Рецидивирующее течение папилломатоза имеет неблагоприят-
ное прогностическое значение. Активность экспрессии Ki67, p53, Bcl-2 коррелирует с гистологической картиной папилломатоза: чем
выше активность экспрессии, тем более выражены процессы дисплазии и озлокачествления, что имеет прогностическое значение.
