Notre travail a été une étude rétrospective, menée du 1 er janvier 1986 au 31 décembre 2006, dans les trois laboratoires d'anatomie et de cytologie pathologiques de la ville de Ouagadougou que compte le Burkina Faso. Il avait pour objectif d'étudier les aspects épidémiologiques et histologiques des cancers à Ouagadougou. Il est ressorti que : 1) le nombre de cas diagnostiqués a connu une augmentation progressive au fil des années ; 2) la fréquence annuelle du cancer était de 200,2 cas ; 3) une prédominance féminine avec 54,9 % des effectifs et 45,1 % chez l'homme. Le sex-ratio était de 0,82 ; 4) l'âge moyen de survenue du cancer a été de 45,88 ans, tout sexe confondu, chez l'homme, l'âge moyen de survenue du cancer a été de 47,35 ans, alors que chez la femme l'âge moyen a été de 44,68 ans ; 5) chez l'homme, les cancers les plus fréquents étaient : le cancer de la peau (11,37 %) qui occupait le premier rang, suivi du lymphome non hodgkinien (9,80 %) et du cancer de la prostate (9,69 %). Chez la femme, le cancer du sein occupait la première place (23,81 %), suivi du cancer du col de l'utérus (22,99 %) ; 6) sur le plan histologique, les carcinomes ont été les plus représentés avec 75,53 % des cas, suivis des sarcomes (10,86 %) ; 7) chez l'enfant, le lymphome de Burkitt (41,95 %) était la tumeur la plus fréquente, suivis du rétinoblastome (12,29 %) et du néphroblastome (11,86 %). The general goal was to determine the histological and epidemiological aspects of cancers in Ouagadougou. It was noted: 1) the number of the cases has increased with the time; 2) the annual average frequency of the cancer was 200.2 cases; 3) women were predominant with 54.9% of cases with a sex ratio of 0.82. The average age of happening of the cancer was 45.88 years for the both sex. It was 47.35 years for men and 44.68 years for women. The most frequent cancers for men were skin cancer (11.37%), followed by non-Hodgkin lymphoma (9.80%) and prostate one (9.69%). For women we had breast cancer (23.81%) followed by cancer of uterine neck (22.99%). At the histological plan, carcinoma represented 75.53% of cases followed by sarcoma (10.86%). Burkitt lymphoma (41.95%) represented childhood cancers followed by retinoblastoma (12.29%) and nephroblastoma (11.86%).
Notre travail a porté sur 992 cas d'affections mammaires colligés, de janvier 2001 à décembre 2006, dans les laboratoires d'anatomie pathologique de la ville de Ouagadougou. L'objectif a été de déterminer les aspects épi-démiologiques et histologiques des affections mammaires dans la ville de Ouagadougou. Il est ressorti de cette étude que : 1) les cas d'affections mammaires diagnostiqués ont augmenté avec le temps ; 2) les trois quarts des lésions diagnostiquées ont été bénignes ; 3) l'âge moyen des patients a été de 34,4 ans pour l'affection mammaire en général ; 4) chez l'homme, l'âge a été de 70 ans pour le seul cas de carcinome, et la moyenne d'âge lors du diagnostic de la lésion bénigne a été de 29 ans ; 5) la lésion a été le plus souvent localisée à droite avec 49,4 % des cas. Sur le plan de l'histologie, la première affection mammaire a été l'adéno-fibrome avec 37,2 % des cas, suivi du carcinome canalaire infiltrant (CCI) avec 16,8 % des cas. Le grade III de ScarffBloom-Richardson a été le plus souvent retrouvé avec 49,3 % des cas. La lésion fibrokystique a occupé le troisième rang avec 12,1 % des cas. La tuberculose mammaire a constitué une affection rare avec 0,4 % des cas d'affections mammaires. Chez l'homme, l'affection mammaire bénigne a été repré-sentée par la gynécomastie avec 0,6 % des cas d'affections mammaires. Un seul cas de tumeur maligne, à savoir le CCI bilatéral, a été diagnostiqué chez celui-ci. The main objective was to determine the histological and epidemiological aspects of breast disease in Ouagadougou. The following results were obtained. 1) The number of the cases has increased with the time.2) The three-quarters of diagnosed lesions were minor.3) The average age of patients was 34.4 years for breast disease in general. With regard to men, the age was 70 years for carcinoma, and the average age during the minor lesion diagnosis was 29 years. 4) The lesion was located mostly on the right side with 49.2% of the cases. 5) At the histological level, adenofibroma with 37.2% of the cases was considered as the highest occurrence followed by infiltrated canal carcinoma with 16.8% of the cases. The grade II by Scarff-BloomRichardson was found most of the time with 49.3% of the cases. 6) The fibrocystic lesion having 12.1% was ranked third. Breast tuberculosis was a rare disease with 0.4% of breast disease. 7) As far as men were concerned, the minor chest disease was shown by gynecomastia, representing 0.6% of breast disease cases. A single case of malignant tumor namely the bilateral infiltrating carcinoma canal was diagnosed.
Introduction. Gestational gigantomastia is a rare benign disease of unknown cause. We report a case of bilateral gigantomastia in pregnancy in order to locate the place of reductive surgery in its care which is quite controversial. Case. A 25-year-old woman gravida 2 para 1 was referred for an exaggerated bilateral breast enlargement at 32-week gestation. The examination showed bilateral giant breasts with collateral venous circulation and trophic changes marked by the necrosis of the distal third of the mammary skin involving the nipple-areolar complex. She underwent a biopsy of the ulcerative breast tissue, and the histology report did not show a malignant cell. After active foetal lung maturation and induced delivery, a breast reductive surgery with nipple plasty was performed 21 days postpartum. The postoperative course was marked by a period of lymphangitis. The cosmetic and psychological result was satisfactory at 6 months and at 18 months. Conclusion. Gestational gigantomastia is a benign disease that can simulate carcinomatous mastitis. The breast reductive plasty keeps its place in our context.
Background: Nodular fasciitis is a benign soft-tissue tumour mimicking a sarcoma. It is generally subcutaneous and small in size.
Introduction: Gastrointestinal stromal tumor (GIST) is the most common non-epithelial, mesenchymal tumor of the digestive tract. Targeted therapy has improved the prognosis but the diagnosis must be accurate before using the existing drugs. Aim: To report the diagnostic difficulty in our context through the clinical polymorphism and define the position of targeted therapy in the management of GIST metastatic stomach. Casuistry: We report 3 cases of gastric GIST with liver metastasis in different circumstances of discovery. Patients were 21, 45, and 73 years old. Discovery circumstances were respectively digestive hemorrhage, severe clinical anemia, abdominal tumor and gastric tumor. There was hepatic metastasis in the three cases. The three patients received Imatinib treatment, adjuvant treatment for the first two cases, and neo-adjuvant treatment for the third case, with a very good clinical response and CT response on metastases. Conclusion: Because of their rarity, GISTs are often difficult to diagnose, and necessarily require immunohistochemistry which is not available in our work context. The effectiveness of targeted therapy even on metastasis needs a rigorous diagnostic approach to improve patient survival.
The authors report a case of tuberculosis of the uterine cervix observed at the maternity unit of the Souro Sanon Teaching Hospital (CHUSS) of Bobo-Dioulasso. This is a rare localization of genital tuberculosis and can take the appearance of a cervical cancer. Biopsy with histological study of the lesion confirms the diagnosis. The frequency of this pathology can reach up to 8%. The extension to the endometrium and/or tubes in a young woman may compromise future fertility. Early and well conducted TB treatment allows the ad integrum restitution cervix.
Contexte : Les études sur les méningiomes rapportent des résultats variables à travers le monde. Différentes études trouvent les méningiomes, les gliomes ou les méta-stases comme la plus fréquente des tumeurs du système nerveux central (SNC). Notre travail a pour but d'étudier la place des méningiomes dans la morbidité liée aux tumeurs du SNC dans notre contexte. Matériel et méthode : Quatre-vingt-six cas de tumeurs du SNC, issus de l'unité de neurochirurgie du CHU-YO et diagnostiqués à l'histologie ont été rétrospectivement collectés de 2005 à 2011 à travers tous les laboratoires d'anatomie pathologique de Ouagadougou. Au cours de cette période, 198 cas de tumeurs du SNC diagnostiqués au scanner ont été répertoriés dans cette unité, seule prenant en charge les cas au Burkina. Résultats : Trente et un cas de méningiome ont été histologiquement diagnostiqués, et constituaient les tumeurs les plus fréquentes (36,04 %), suivis des gliomes (34,49 %), des métastases (7,33 %), puis des tumeurs hypophysaires (5,81 %). L'ensemble des 86 cas de tumeurs du SNC étaient constitués de 55,61 % d'hommes et 44,39 % de femmes ; l'âge moyen était de 37,05 ans. Les 31 cas de méningiome ont concerné 19 femmes (61,3 %) et 12 hommes (38,7 %), soit un sex-ratio hommes/femmes de 0,63. L'âge variait de 13 à 86 ans, avec une moyenne de 45,32 ans. Le sous-type histologique méningothélial était prédominant avec 35,48 % des cas. Les méningiomes de grade I (bénins) représentaient 90,31 %, et les méningiomes atypiques (grade II) et malins (grade III) 9,69 % des cas. Conclusion : Les méningiomes sont les tumeurs les plus fré-quentes du SNC dans notre contexte, avec prédominance du sous-type méningothélial de grade I. Different reports state meningiomas, gliomas, or metastatic tumors as the most common tumors among central nervous system neoplasms. The aim of our study was to determine the relative frequency of meningiomas among central nervous system neoplasms in our environment. Method: Consecutive patients (86) seen at Neurosurgery Office of Yalgado Ouédraogo Teaching Hospital of Ouagadougou, with histologically proven central nervous system tumor over a 7 years period (2005)(2006)(2007)(2008)(2009)(2010)(2011), were retrospectively analyzed. Results: The commonest histological types were meningiomas, gliomas, metastasis, and pituitary tumors. Thirty-one patients (36.04%) had histologically confirmed meningioma during the study period, 25 (29.07%) had glioma, 6 (7.33%) had metastasis, and 5 (5.81%) had pituitary adenoma. The mean age of the 86 patients was 37.05 years, with 55.61% of male. The mean age of the meningiomas cases was 45.32, with 61.3% of female. 90.31% had grade I meningioma.
