Зна чи тель ная часть па-ци ен тов об ра тит ся к хи рур гу по по во ду уз ло во го зо ба с име ю щи ми ся при зна ка ми ком прес сии или ти ре о-ток си ко за. Боль ные все-та ки бу дут про опе ри ро ва ны, но не в 40-50 лет, а в бо лее стар шем воз ра с те - также более половины уменьшились более чем на 50% (39 из 67). Среди узловых образований более 4 см 3 не отмечено ни одного узла, который бы уменьшился в объеме более чем на 75%. Однако большая их часть уменьшилась более чем на 25%. Представлен алгоритм дифференцированного лечения кистозных узлов. ЭД выполнена 54 пациентам с кистоз-ной деструкцией в узле более 70%. Отмечено уменьшение среднего объема с 4,68 ± 2,25 до 1,15 ± 0,9 см 3 . КомбиЛИТТ выполнена 63 пациентам с кистозной полостью от 20 до 70%. Объем узлов уменьшился с 7,98 ± 4,56 до 1,87 ± 0,9 см 3 . ЛИТТ выполнена 30 пациентам с крупными узлами и кистозными полостями менее 20% от объема узла. Отмечено уменьшение объема узлов с 2,89 ± 1,25 до 1,42 ± 0,8 см 3 . ЛИТТ проведена 31 пациенту с небольшими кистозными узла-ми до 1 см 3 . Отмечено уменьшение размера с 0,69 ± 0,23 до 0,29 ± 0,12 см 3 . Показано, что изменения тиреоидного ста-туса после малоинвазивных операций не происходит. Laser-induced thermotherapy (LITT) -371 patient, Ethanol destruction of ED -54 patients, 63 -combined technique (KombiLITT). Dynamics of node size was assessed in 356 patients (72.8%). LITT -252 patients, ED -50 patients KombiLITT -52 patients. LITT used in the treatment of solid structure nodes. Objective. To improve the results of treatment in patients with benign nodular pathology of the thyroid gland (TG) by means of perfecting the technique of minimally invasive treatment with a high-intensive laser under the sonographic control. Materials and methods. An original technique of laser-induces thermotherapy (LITT) of the thyroid nodule with the ablation of the vessels, feeding the nodule was developed and introduced. The procedure was used in the treatment of 91 patients with benign nodular pathology of the thyroid gland, 116 patients underwent laser-induced thermotherapy by the standard technique (comparison group). Three months later, US was used to assess and compare the results of treatment in both groups. The results of thermometry were analyzed depending on the initial volume of the TG nodular neoplasm. Results. A decrease in the nodules with the initial volume up to 2 cm3 in both groups was statistically significant and took place practically similarly 78.7 % in the main group and 71.7 % in the comparison group. When exposing the nodules of larger sizes, the efficiency of standard LITT method fell. In the comparison group, patients with the nodular sizes of 68 cm2 had a decrease in the volume only by 25.8 % % (from 6.74 cm3 to 5.0 cm3). When using an original method of thermoablation (TA), a decrease in the nodules of the same size was 69.3 % (from 6.98 cm3 to 2.14 cm3). The difference was statistically significant (p = 0,003) that proves the efficiency of this technique. In the subgroup with the nodules 8 cm3, a nodular volume decrease was 33.3 % (from 11.05 to 7.36 cm3) in the main group and 28.8 % (from 14.07 to 10.02 cm3) in the comparison group. The difference was not statistically significant (p = 279). Conclusions. Laser-induces thermotherapy of the thyroid gland with the ablation of the vessel, feeding the nodule, leads to a statistically significant reduction of the benign colloid nodular neoplasms of any size. The original technique indicated higher efficiency compared with standard LITT in relation to the nodules sized from 6 to 8 cm.
Изучены результаты лечения методом лазериндуцированной термотерапии (ЛИТТ) 107 пациентов с добро качественными узловыми образованиями щитовидной железы. Средний объем узлов до и после лечения сократился с 5,0 до 2,8 см 3 (44%). Среди узловых образований объемом до 1 см 3 средний объем уменьшился более чем на 60%. Среди крупных узловых образований объемом более 8 см 3 средний размер уменьшился всего чуть более чем на 40%. Среди узлов объемом до 1 см 3 после проведения ЛИТТ большая часть уменьшилась более чем на 50% (19 из 33), из них 8 (24,2%) уменьшились более чем на 75%, 6 (18,3%) узлов исчезли полностью. Среди узлов объемом 1-2 см 3 также более половины уменьшились более чем на 50% (17 из 29), из них более чем на 75%-6 узлов (20,7%). Среди узловых образований более 4 см 3 не отмечено ни одного узла, который бы уменьшился в объеме более чем на 75%. Однако большая их часть уменьшилась более чем на 25%. Отмечена статистически значимая разница зависимости уменьшения объема узла от числа точек лазерного воздействия. У трех пациентов (2,8%) был небольшой ожог кожи в месте вкола иглы. Методика ЛИТТ доброкачественных узловых образований щитовидной железы является высокоэффективной, поскольку в подавляющем большинстве наблюдений приводит либо к исчезновению, либо к значительному уменьшению узлового образования. The average volume of nodes before and after treatment was reduced from 5.0 cm 3 to 2.8 cm 3 (44%). Of nod ules up to 1 cm 3 average volume decreased by more than 60%. Among large nodules larger than 8 cm 3 aver age decreased only slightly more than 40%. Among the nodes up to 1 cm 3 after LITT most decreased by more than 50% (19 of 33), 8 of them (24.2%) decreased by more than 75%, 6 (18.3%) nodes have disap peared completely. Among the nodes of 2.1 cm 3 , more than half also fell by more than 50% (17 of 29), of which more than 75%-6 knots (20.7%). Of nodules more than 4 cm 3 was not a single node, which would decrease in volume by more than 75%. However, most of them fell by more than 25%. There was a statisti cally significant difference according to volume reduction of the number of node points of the laser action. Three patients (2.8%) had a small burn the skin at the puncture needle. LITT method benign thyroid nodules is high, because overwhelmingly majority of cases leads to either the disappearance or significant reduction of thyroid nodules.
