Анатомофизиологические изменения, происходящие в организме таких пациентов-снижение функциональных возможностей органов и систем, компенсаторных и приспособительных реакций-существенно повышают риск развития патологии ЦНС, ведущей к развитию острых послеоперационных психических нарушений, в частности послеоперационного делирия (ПОД). В МКБ-10 делирий (F05) определяется как экологически неспецифический органический церебральный синдром, характеризующийся одновременным нарушением сознания и внимания, восприятия, мышления, памяти, психомоторного поведения, эмоций, цикличности сна и бодрствования.
During the postoperative period, cardiological complications occupy the first position regarding morbidity and mortality rates. They depend on various factors such as compromised cardiovascular history and type of surgical intervention, features and type of anesthesia, water balance and postoperative care of the patient. To prevent complications, one should reply two questions: is there a risk of cardiovascular complications in the perioperative period and how to avoid them. The article presents a review of the literature on current views on the perioperative assessment of cardiac risks in patients undergoing noncardiac surgery.
This report presents a clinical case of the treatment of pain syndrome in isolated chest trauma following an accident of a patient with liver cirrhosis and hypocoagulation syndrome who sustained bilateral fracture of the ribs (II, III, IV, V, and VI ribs on the right and II, III, IV, V, and VI ribs on the left), fracture of the sternum body in the middle third, bilateral hemopneumothorax, and contusion of both lungs. From the initial hours, fentanyl, which was administered intravenously in a complex of multimodal analgesia, was used for anesthesia. However, adequate pain control was not achieved. Against the background of pain syndrome, the patient developed impaired consciousness and respiratory failure. Epidural puncture and catheterization with an epidural injection of ropivacaine were performed, which relieved the pain syndrome and helped prevent pulmonary complications. The present case highlights the need for an individual assessment of the riskbenefit ratio of the use of epidural catheterization in patients with coagulopathy.
Conclusion: The use of epidural analgesia for chest trauma in patients with moderate coagulopathy made it possible to initiate effective analgesia and reduce the risk of respiratory complications, which ultimately ensured a favorable outcome of severe trauma.
Цель работы: клиническая оценка эффективности эпидуральной блокады в сочетании с надгортанным воздуховодом и вспомогательной вентиляцией легких при видеоассистированных торакоскопических тимэктомиях� Материалы и методы. Обследованы 9 пациентов с генерализованной миастенией, которым проводили видеоассистированную торакоскопическую тимэктомию� Результаты. Достаточный уровень анальгезии во время операции подтверждался клинически: больные были сухие, розовые, теплые, темп диуреза составлял 60 мл/ч� По окончании операции пробуждение больного осуществляли в среднем через 1-2 мин� В это же время удаляли надгортанный воздуховод� Пациенты могли самостоятельно передвигаться по палате через 1 ч после операции� Интенсивность боли в послеоперационном периоде составляла в среднем 1,50 ± 0,87 балла по ВАШ� Ни одному пациенту не потребовалось проведение санационной фибробронхоскопии� Каких-либо интра-и послеоперационных осложнений не зафиксировано� Все 9 пациентов переведены в профильное отделение в день операции; 6 (66,7%) пациентов выписали на 5-е сут после операции, 3 (33,3%)-на 6-е сут� Вывод. Использование эпидуральной блокады в сочетании с надгортанным воздуховодом и вспомогательной вентиляцией легких без интубации трахеи и использования миорелаксантов при видеоассистированных торакоскопических тимэктомиях является безопасным и эффективным способом�
