The aim of this study was to validate the German Version of the Yale Pharyngeal Residue Severity Rating Scale and to investigate the impact of rater experience and training. The English original scale was translated into German language using a forward-backward method. For the validation, 30 images of different pharyngeal residue amounts have been selected and assigned to the scales' severity levels by two experts. 28 raters rated the images in randomized order in two passes. To investigate the impact of experience and training, raters were grouped into subgroups. Statistical analysis was carried out using kappa statistics. The results demonstrate excellent residue ratings for construct validity, inter-rater reliability and intra-rater reliability regarding overall group and all subgroups (Kappa > 0.90). No significant differences were found between groups by experience and inconstant differences were found between groups by training. The German version of the Yale Pharyngeal Residue Severity Rating Scale is a valid, reliable instrument for scoring location and severity of pharyngeal residue in the context of flexible endoscopic evaluations of swallowing.

Die in verschiedenen Publikationen vertretenen Auffassungen hinsichtlich der Behandlung reichen vom Abwarten einer Selbstheilung ("wait and see") über Phonochirurgie (z. B. von Stimmlippenknötchen) bis hin zu Stimmübungs-therapie und Psychotherapie. Ziel: Ziel der vorliegenden Arbeit war es deshalb, einen interdisziplinären Konsens über die diagnostischen und therapeutischen Möglichkei-ten bei kindlichen Dysphonien herbeizuführen. Methodik: Nominaler Gruppenprozess, interdisziplinäre Diskussion über diagnostische Mög-lichkeiten und Behandlungskonzepte sowie Literaturrecherche und Evidenzlevelbewertung von entsprechenden Publikationen zu Dysphonien bei Kindern. Ergebnisse: Die ausgewertete Literatur zu Diagnostik und Therapie von Dysphonien bei Kindern erreichte überwiegend nur niedrige Evidenzlevel. Daher galt es, Erfahrungen verschiedener Disziplinen und Fachleute auszuwerten. Nach einer differenzierten Diagnosestellung, die kindgerecht durchgeführt werden muss und die die Besonderheiten der kindlichen Stimmphysiologie zu berücksichtigen hat, sollte entschieden werden, ob die Störung nach Beratung der Familie weiter beobachtet wird, ob mit einer Stimmübungstherapie oder einer Psychotherapie behandelt werden muss oder ein phonochirurgischer Eingriff angezeigt ist. Diskussion und Fazit: Es liegen eine Reihe von Behandlungsmöglichkeiten für Kinder mit Dysphonien vor. Konsens ist, dass sie nach einer ausführli-chen Diagnostik ursachenspezifisch angewandt werden müssen. Die verschiedenen diagnostischen und therapeutischen Verfahren müssen zukünftig jedoch hinsichtlich ihrer Evidenz bei kindlichen Dysphonien umfangreicher evaluiert werden.
Abstract ▼Background: There are nationally and internationally different opinions about and no guidelines available for diagnostics, treatment and management of dysphonia in childhood. For vocal nodules, for example, and non-organic dysphonia, different opinions range from "wait and see" to training interventions, psychotherapy and phonosurgery. Aims: The purpose of this study was to reach an interdisciplinary consensus about diagnostic tools and treatment procedures for voice disorders in children Methods: By means of an interdisciplinary exchange of experience, discussion about treatment concepts as well as a review of the current publications on concepts, prevalence, diagnostics, therapies and their evidence, this study was conducted in order to develop expert opinions. Results: Predominantly low evidence levels were found concerning diagnostic and therapy methods in the evaluated literature.
