O atual trabalho discute os avanços e limites da regulação pública da saúde suplementar e propõe mapear a integralidade da assistência pelo acompanhamento da linha do cuidado. Discute um modelo no qual o usuário deveria ser acompanhado segundo determinado projeto terapêutico instituído, comandado por um processo de trabalho cuidador, e não por uma lógica "indutora de consumo". Esse mecanismo visaria assegurar a qualidade da assistência prestada.
Throughout Brazilian Public Health System's (Centralized Health System -SUS) construction history there has been a reasonable investment in the education for the sector. However, it has been frequently noticed by health professionals and managers the fact that this investment in educational programs has not converted into change of healthcare practices. Assuming that education can be used as a tool for changes in health, the text suggests that the pedagogical practices should be directed towards the production of individuals implied with the care production. Hence it proposes to work on a field of subjectivity in addition to cognition. This work reveals the management of the Brazilian public health system and its flows of permanent education, focusing "micromanagement" to think about the context on which they structuralize the diverse scenarios of care production, treating them as Pedagogical Production Units where it would be possible to develop entailed educational methodologies to a general idea of permanent education in health.
A Reforma Sanitária brasileira propiciou maior aplicabilidade das ações locais, favorecendo o surgimento de experiências exitosas nos diversos setores da saúde. Uma destas foi a criação dos Centros de Atenção Psicossocial (Caps), que subvertem a lógica da hierarquização do sistema de saúde e se organizam agregando os diferentes níveis de atenção à saúde. Objetivou-se discutir as interações estabelecidas entre os níveis de complexidade do sistema de saúde na atenção à saúde mental e compreender a conformação da rede de atenção à saúde mental no município de Sobral-CE. Trata-se de estudo do tipo qualitativo realizado na Rede de Atenção Integral à Saúde Mental desse município. Os resultados demonstraram que os serviços de saúde mental em Sobral convivem com diferentes arranjos em rede, operando por uma diversidade de dispositivos de cuidado o que possibilita a negociação de projetos terapêuticos menos medicalizantes, embora se observe que permanece atenção especial dos trabalhadores quanto à centralidade da administração da medicação. Há grande mobilidade dos trabalhadores na rede, circulando os diversos equipamentos de saúde, o que favorece as conecções e fluxos entre equipes na construção das linhas de cuidado. Os fluxos de conexão, considerados como modo de funcionamento das redes rizomáticas, operam na saúde mental por força da ação dos trabalhadores, que tem por base a ideia de que todos são protagonistas no processo de cuidado, e assim, os movimentos são partilhados e articulados entre si.
Palavras-chave Objetivou-se compreender o cuidado psicossocial, tendo como analisador o projeto terapêutico dos usuários, a partir das narrativas de profissionais dos serviços. Trata-se de uma pesquisa qualitativa, embasada na Hermenêutica. As narrativas contextualizam um cenário marcado pelas dimensões epistemológica e técnico-assistencial da atenção psicossocial. O projeto terapêutico descaracteriza-se de sua concepção abrangente, de processo dinâmico, para ser concretizado a partir de um trabalho fragmentado pela especialização. A pesquisa provocou um movimento de reflexão dos profissionais, a partir da problematização de projetos terapêuticos, abrindo caminhos para a reformulação das ações da equipe, numa proposta de trabalho da equipe interdisciplinar. Foi evidente a escassez de discussões que considerassem o protagonismo dos usuários e da família na construção do projeto terapêutico, revelando, assim, a fragilidade das ações de cuidado psicossocial nas dimensões que consideram a participação e construção de cidadania.
ResumoEste artigo teve como objetivo entender os desafios da práxis do fisioterapeuta no Núcleo de Apoio à Saúde da Família (NASF). Apresenta uma abordagem qualitativa numa perspectiva histórico-social. Foram entrevistados 14 indivíduos por meio de entrevista semiestruturada. Os entrevistados foram divididos em três grupos: o grupo I foi composto por Gestores, grupo II por Profissionais de Saúde e grupo III por Usuários. A análise dos dados foi realizada a partir da análise de conteúdo. Assim, foram construídas três categorias, sendo denominadas de tratamento fisioterapêutico e o cuidador, conhecimento e NASF e acesso e trabalho em equipe. O fisioterapeuta, quando inserido no Núcleo de Apoio à Saúde da Família, precisa enfrentar os problemas que aportam ao serviço de saúde, deve promover educação permanente em saúde em seu processo de trabalho juntamente com os usuários e a equipe do programa saúde da família. Há porém, uma escassez de recursos no NASF, assim como a dificuldade de acesso e formação de vínculo entre profissionais e usuários, o que dificulta a produção de um cuidado integral com uma ação compartilhada com a equipe do NASF e da saúde da família. Dessa forma, deve-se fazer uma reflexão sobre a formação desse profissional para atender às necessidades da população. Palavras-chave: AbstractThis article aims to understand the Challenges to the practice of physiotherapist in NASF. It presents a qualitative approach in a historical-social perspective. Were interviewed 14 individuals, using a semi-structured interview. Respondents were divided into three groups: group I was composed by managers, group II by Health Professionals and group III by users. Data analysis was performed based on content analysis. Thus, we constructed three categories, namely physical therapy and caregiver, knowledge and access and NASF and teamwork. Physiotherapist inserted into the Support Center for Family Health must face the problems that contribute to the health service, in their work process, must promote continuing health education along with users and staff of the family health strategy, but there is a shortage of resources in NASF, as there is a difficult access and formation bonds between professionals and users, which hinders the producing an integral care with a shared action with the team NASF and family health. Thus it should be a reflection on the formation of this professional to meet the needs of the population.
BackgroundWe utilized our previous studies analyzing the reforms affecting the health sector developed in the 1990s by financial groups to frame the strategies implemented by the pharmaceutical industry to regain market positions and to understand the challenges that regulatory agencies are confronting.MethodsWe followed an analytical approach for analyzing the process generated by the disputes between the financial groups and the pharmaceutical corporations and the challenges created to governmental regulation. We analyzed primary and secondary sources using situational and discourse analyses. We introduced the concepts of biomedicalization and biopedagogy, which allowed us to analyze how medicalization was radicalized.ResultsIn the 1990s, structural adjustment policies facilitated health reforms that allowed the entrance of multinational financial capital into publicly-financed and employer-based insurance. This model operated in contraposition to the interests of the medical industrial complex, which since the middle of the 1990s had developed silent reforms to regain authority in defining the health-ill-care model. These silent reforms radicalized the medicalization. Some reforms took place through deregulatory processes, such as allowing direct-to-consumer advertisements of prescription drugs in the United States. In other countries different strategies were facilitated by the lack of regulation of other media such as the internet. The pharmaceutical industry also has had a role in changing disease definitions, rebranding others, creating new ones, and pressuring for approval of treatments to be paid by public, employer, and private plans. In recent years in Brazil there has been a substantial increase in the number of judicial claims demanding that public administrations pay for new treatments.ConclusionsWe found that the dispute for the hegemony of the health sector between financial and pharmaceutical companies has deeply transformed the sector. Patients converted into consumers are exposed to the biomedicalization of their lives helped by the biopedagogies, which using subtle mechanisms present discourses as if they are objective and created to empower consumers. The analysis of judicialization of health policies in Brazil could help to understand the complexity of the problem and to develop democratic mechanisms to improve the regulation of the health sector.
