A b s t r a c t Background:It is important to identify the clinical indicators of poor prognosis and treatable conditions that might contribute to the progression of heart failure (HF) and pulmonary hypertension (PH) in the group of patients with dilated cardiomyopathy (DCM) and concomitant obstructive sleep apnoea (OSA).
Aim:To evaluate the influence of OSA on echocardiographic and haemodynamic parameters in patients with DCM, and the outcome in long-term follow-up.
Methods:We enrolled patients with DCM and severely impaired ejection fraction (EF < 30%). Each patient underwent polygraphy, echocardiography, and right heart catheterisation. Subjects were divided into groups based on the apnoea-hypopnoea index (AHI): > 0 and < 5 (group I), ≥ 5 and ≤ 15 (group II), > 15 and ≤ 30 (group III), and > 30 (group IV). We compared the OSA-free (AHI < 5) subjects with those with OSA (AHI ≥ 5). The evaluated clinical end-points were death and orthotropic heart transplant.
Results:The study population comprised 51 patients. Mean EF was 22%; 59% of patients were suffering from OSA. The increased severity of OSA correlated with worse pulmonary haemodynamics. Patients with OSA had higher mean pulmonary arterial pressure and pulmonary vascular resistance than individuals without OSA (p = 0.044, p = 0.032, respectively). The highest chamber diameters assessed in echocardiography were found in group IV (p < 0.05). A total of 10 end-points occurred during follow-up (8.9 ± 5.1 months), with significant differences observed between groups I-IV and the highest rate in group IV (p < 0.001).
Conclusions:The increasing severity of OSA worsens the prognosis of DCM patients, independently of severe HF and coexistent PH. Systematic OSA screening in patients with HF might facilitate identification of individuals at high risk of progression of pulmonary haemodynamic impairment and end-point rate.
Kardiomiopatia połogowa jest rzadkim, ale potencjalnie groźnym schorzeniem o niejasnej etiologii. Śmiertelność w różnych doniesieniach sięga 50%, u wielu pacjentek rozwija się przewlekła niewydolność serca (HF). Dlatego tak istotne jest wyselekcjonowanie chorych o niekorzystnym rokowaniu. Rezonans magnetyczny (MR) może okazać się w tym pomocny. Pacjentkę w wieku 23 lat przyjęto na Oddział Kardiologii tydzień po porodzie drogą naturalną z powodu objawów HF oraz upośledzenia frakcji wyrzutowej lewej komory (LVEF). W ostatnim tygodniu ciąży pacjentka była leczona z powodu gestozy. Badanie MR serca wykonano 8 dni po porodzie. Potwierdzono uogólnione zaburzenia kurczliwości (LVEF 45%). Spełnione były wszystkie kryteria rozpoznania aktywnego procesu zapalnego w zakresie mięśniówki serca. Wskaźnik intensywności sygnału T2 (odpowiadający obrzękowi) wynosił 2,0, a wskaźnik wczesnego wzmocnienia T1 pokontrastowego (odpowiadający przekrwieniu) -4,1. Stwierdzono również drobne śródmięśniowe obszary opóźnionego wzmocnienia w segmentach dolnych i dolno-bocznych odpowiadające zwłóknieniu oraz odczyn osierdziowy (płyn za ścianą dolną maksymalnie do 10 mm grubości). U chorej rozpoznano kardiomiopatię połogową. W leczeniu zastosowano bromokryptynę, perindopril, karwedilol i spironolakton. Pacjentka w stanie ogólnym dobrym, w I klasie wg NYHA, została wypisana do domu. Po 3-miesięcznym okresie leczenia wykonano kontrolne badanie MR. Stwierdzono istotną poprawę funkcji skurczowej lewej komory, zmniejszenie jej objętości oraz redukcję ilości płynu w osierdziu (ryc. 1). Zaobserwowano również ustąpienie cech aktywnego procesu zapalnego, tj. zmniejszenie obrzęku (wskaźnik intensywności sygnału T2 zmalał do 1,7) oraz zmniejszenie przekrwienia mięśniówki (wskaźnik wczesnego wzmocnienia T1 zmniejszył się do 2,8) (ryc. 2). W obrazach opóźnionego wzmocnienia była zauważalna redukcja obszarów zwłóknień (ryc. 3). Istnieje jedynie kilka doniesień zastosowania MR w diagnostyce kardiomiopatii połogowej. Opublikowane wyniki badań, wykonywanych w ostrej fazie choroby, u większości pacjentek wykazują obniżenie funkcji skurczowej lewej komory i cechy aktywnego procesu zapalnego -analogicznie jak w opisanym przypadku. W dostępnych doniesieniach niekorzystne rokowanie u chorych (ryzyko ponownych hospitalizacji z powodu zaostrzeń HF) wiązało się z wykryciem zwłóknień w obrazach opóźnionego wzmocnienia. U opisanej pacjentki już w pierwszym badaniu stwierdzono drobne obszary zwłóknień śródmięśniowych. Mimo to, po 3 miesiącach stan chorej był dobry -nie zanotowano objawów HF. Pozostaje ona pod regularną opieką kardiologiczną. Mimo poprawy funkcji skurczowej lewej komory, wykazane w badaniu MR zwłóknienia podają w wątpliwość dobre odległe rokowanie pacjentki. Opisany przypadek potwierdza istotne znaczenie procesu zapalnego w patologii choroby oraz ukazuje przydatność badań MR w diagnostyce choroby i monitorowaniu leczenia. Badanie MR stanowi istotne narzędzie w ocenie rokowania, wnosi bowiem nowe niedostępne dotychczas informacje, choć ich wartość kliniczna...
