RESUMO Objetivo Estimar a prevalência de tuberculose entre a população privada de liberdade. Métodos Realizou-se uma revisão sistemática com metanálise. Foram selecionados estudos publicados de janeiro de 1997 a dezembro de 2016 nas bases de dados MEDLINE (via PubMed), SciELO e LILACS. O termos de busca foram (“tuberculosis”[MeSH] OR “tuberculosis”[TIAB]) AND (“prisons”[MeSH] OR “prisons”[TIAB]); ou (“tuberculose” [DeCS] OU “tuberculose” [palavras] E “prisões” [DeCS] OU “prisões” [palavras]). O desfecho primário foi a prevalência de tuberculose com intervalo de confiança de 95% (IC95%). Na metanálise, as variáveis associadas com as taxas de prevalência de tuberculose na população privada de liberdade na análise univariada (P ≤ 0,20) foram incluídas no modelo final multivariado. Resultados Com base nos 29 estudos incluídos na metanálise, 2 163 presos foram identificados com tuberculose. A prevalência combinada de tuberculose entre os prisioneiros foi de 2% (IC95%: 0,02 a 0,02). A prevalência de tuberculose entre prisioneiros em países com prevalência de 0 a 24 por 100 mil habitantes na população geral ficou abaixo de 1% (IC95% = 0,00 a 0,00). Nos países com prevalência de tuberculose de 25 a 99/100 mil, a estimativa foi de 3% (IC95% = 0,02 a 0,04); e nos países com prevalência ≥ 300/100 mil, a estimativa foi de 8% (IC95% = 0,05 a 0,11). Conclusão O presente estudo reafirma a alta magnitude da tuberculose entre a população privada de liberdade no contexto mundial. Os resultados mostram ainda uma ligação entre a prevalência de tuberculose na população geral e a prevalência da doença dentro dos presídios.
OBJETIVO: Estimar o risco de infecção tuberculosa em agentes comunitários de saúde envolvidos no controle da doença. MÉTODOS: Foi seguida uma coorte prospectiva, de abril de 2007 a maio de 2008, no município de Cachoeiro de Itapemirim, ES. A coorte foi composta por 61 agentes comunitários, divididos em não-expostos (n=37) e expostos (que acompanharam pacientes com tuberculose, n=24). Durante os 12 meses de seguimento, foi realizado teste tuberculínico, utilizando a tuberculina PPD RT23. Foi calculado o risco relativo e intervalo com 95% de confiança e foi avaliada a correlação entre a viragem tuberculínica e a história ocupacional dos agentes por meio do coeficiente de correlação de Pearson. RESULTADOS: A incidência da viragem foi de 41,7% no grupo dos expostos e 13,5% no grupo dos não expostos. O risco anual de infecção foi de 52,8% no grupo dos expostos e de 14,4% no grupo dos não expostos (p= 0,013). Observou-se associação entre viragem tuberculínica e exposição a paciente com tuberculose (RR= 3,08; IC 95%: 1,201;7,914). CONCLUSÕES: Os agentes que acompanharam pacientes com tuberculose em suas rotinas de serviço apresentaram risco de infecção maior que aqueles que não acompanharam pacientes com essa doença. A implementação de medidas administrativas de biossegurança de rotina, entre as quais a prova tuberculínica, devem ser priorizadas, considerando o alto risco de infecção tuberculosa entre os agentes comunitários de saúde.
Background optimal management of end-stage renal disease (ESRD) in hemodialysis (HD) patients should be more studied because it is a serious risk factor for mortality, being considered an unquestionable global priority. Methods we performed a retrospective cohort study from the Nephrology Service in Brazil evaluating the survival of patients with ESRD in HD during 20 years. Kaplan-Meier method with the Log-Rank and Cox’s proportional hazards model explored the association between survival time and demographic factors, quality of treatment and laboratory values. Results Data from 422 patients were included. The mean survival time was 6.79 ± 0.37. The overall survival rates at first year was 82,3%. The survival time correlated significantly with clinical prognostic factors. Prognostic analyses with the Cox proportional hazards regression model and Kaplan-Meier survival curves further identified that leukocyte count (HR = 2.665, 95% CI: 1.39–5.12), serum iron (HR = 8.396, 95% CI: 2.02–34.96), serum calcium (HR = 4.102, 95% CI: 1.35–12.46) and serum protein (HR = 4.630, 95% CI: 2.07–10.34) as an independent risk factor for the prognosis of survival time, while patients with chronic obstructive pyelonephritis (HR = 0.085, 95% CI: 0.01–0.74), high ferritin values (HR = 0.392, 95% CI: 0.19–0.80), serum phosphorus (HR = 0.290, 95% CI: 0.19–0.61) and serum albumin (HR = 0.230, 95% CI: 0.10–0.54) were less risk to die. Conclusion survival remains low in the early years of ESRD treatment. The present study identified that elevated values of ferritin, serum calcium, phosphorus, albumin, leukocyte, serum protein and serum iron values as a useful prognostic factor for the survival time.
Este artigo tem o objetivo de contribuir para o debate sobre a transmissão nosocomial da TB em profissionais de saúde em um país onde esta é endêmica. Verificamos que até 1900 não se aceitava que os profissionais envolvidos no cuidado de pacientes portadores de TB pudessem ser mais susceptíveis à infecção pelo bacilo que a população geral. Vários estudos entre 1920 e 1930 apresentaram achados significativos nas taxas de conversão do teste tuberculínico dos estudantes da área de saúde, mas a maioria dos clínicos continuava se recusando a reconhecer a suscetibilidade dos profissionais de saúde em relação à TB. Nos diferentes locais onde o cuidado ao paciente com TB foi implantado, os profissionais de saúde são descritos como uma população especialmente exposta ao risco de contrair a infecção e adoecer. É urgente que a comunidade científica e os trabalhadores de saúde se organizem, que se reconheçam como uma população sujeita ao risco de adoecimento, e que ações se efetivem no sentido de minimizar os riscos potenciais nos locais onde acontece o cuidado a pacientes com TB.
Objective: To determine the prevalence of vitamin D deficiency/insufficiency in children 0-18 years of age with recurrent wheezing and/or asthma residing in the microregion of Viçosa, Minas Gerais, Brazil, and treated at a referral center, and to determine its association with major risk factors for wheezing. Methods: A cross-sectional study was performed using a semi-structured questionnaire, which was administered by trained interviewers to the legal guardians of the study participants. Data were obtained regarding general characteristics of recurrent wheezing; general sociodemographic, environmental, and biologic factors; and atopy-related factors. The magnitude of the statistical association was assessed by calculating ORs and their corresponding 95% CIs by using multiple logistic regression. Results: We included 124 children in the study. The prevalence of vitamin D deficiency/insufficiency in the sample was 57.3%. Vitamin D deficiency/insufficiency was found to be associated with wheezing in the first year of life, personal history of atopic dermatitis, environmental pollution, and vitamin D supplementation until 2 years of age. Conclusions: The prevalence of vitamin D deficiency/insufficiency was high in our sample. Vitamin D concentrations were directly associated with vitamin D supplementation until 2 years of age and were inversely associated with wheezing events in the first year of life, personal history of atopic dermatitis, and environmental pollution.
Objective: To evaluate the incidence of Mycobacterium tuberculosis infection, using tuberculin skin test, among community health agents (CHAs) monitoring TB patients in the city of Cachoeiro de Itapemirim, Brazil. Methods: We included 30 CHAs acting in the Family Health Program and 30 of their family members residing in the same household. The tuberculin skin test results of each CHA were compared with those of the corresponding family member. Results: Of the 30 CHAs, 27 (90.0%) were female, compared with 23 (76.7%) of the 30 family members (p = 0.299). The mean age of the CHA group and of the family member group was, respectively, 36.8 and 39.7 years. No statistically significant difference was found between the groups regarding the level of education.Regarding M. tuberculosis exposure, the same number of participants in the two groups reported having known or had contact with a TB patient (17 individuals; 56.7%). There was a statistically significant difference regarding positive tuberculin skin test results (26.7% in the CHA group and 3.3% in the family member group; p = 0.011).Conclusions: M. tuberculosis infection was significantly higher among CHAs than among their family members, fueling the debate on the occupational risk involved in the activities of these professionals. grupos afirmou conhecer ou já ter tido algum contato com paciente com TB (17 indivíduos; 56,7%). Conclusões: A infecção por M. tuberculosis entre os ACS foi significativamente maior que entre seus familiares, e isso contribui para o debate em torno do risco ocupacional envolvido nas atividades destes profissionais.
