BACKGROUND: COVID-19 is affecting almost the entire world, causing more than four hundred thousand deaths and undermining the health care systems, as much as the economy, of the afflicted countries. The strategies for prevention depend on largely lacking information, as infection prevalence and virus pathogenicity. This study aimed to determine the prevalence, the pathogenicity, and the speed of infection spreading in a large population in Brazil. MATERIALS AND METHODS: This is a serial cross-sectional study designed on a population basis and structured over houses as the sampling units. The sampling consisted of four visits at 15 days intervals in randomly selected census-designated sectors of the State major municipalities (reference municipalities) and two visits at 30 days intervals in smaller municipalities of the same regions of those of reference. At each visit, the investigators sampled houses and sampled one individual in each house for data collection. After the informed consent, the investigators performed a rapid antibody detection test (Celer Technology, Inc) and applied a questionnaire containing clinical and demographic questions. RESULTS: From May 13th to 15th, the investigators performed 6,393 rapid tests in 4,612 individuals of the reference municipalities, 1,163 individuals of the smaller municipalities, and 166 contacts of the positive individuals. Ninety-seven dwellers were positive in the reference municipalities, giving a prevalence of 2.1% (CI 95%: 1.67-2.52%). In the smaller municipalities, the figure was 0.26% (CI 95%: 0.05%-0.75%) (three positives). There was an association of the positive result with female sex (p = 0.013) and houses with five dwellers or more (p = 0.003). Seventy-eight positive individuals reported symptoms in the previous 15 days (80.4%), being anosmia (45.4%), cough (40.2%), and myalgia (38.1%) the more frequent. About one-third of them reported fever (28.9%). CONCLUSIONS: The results reveal a still small prevalence of infection in the study area, despite the significant number of sick people overloading the health system. The figures indicate an important underreporting in the area and a frequency that still can grow, making necessary public health actions for the containment of the transmission.

Resumo Estudo descritivo, de abordagem quantitativa, para avaliar as estratégias realizadas pelo enfermeiro no ambiente escolar de um Centro

Objective: To determine the principal adverse effects of the tuberculosis treatment regimen recommended by the Brazilian Ministry of Health. Methods: A prospective descriptive study involving 79 tuberculosis patients treated at the Clinical Research Center of the Cassiano Antonio Moraes University Hospital, in the city of Vitória, Brazil, between 2003 and2006. The treatment regimen consisted of isoniazid, rifampicin, pyrazinamide and ethambutol for four months, followed by rifampicin and isoniazid for two months. During the treatment period, the patients were clinically evaluated every week and had a monthly medical visit. Results: The overall incidence of adverse effects was 83.54%. Articular/bone/muscle involvement was the most common, followed by skin involvement (24.94% and 22.09%, respectively). Adverse effects were more common in the second month of treatment (41.59%). Modification of the treatment regimen was unnecessary. One patient required concomitant medication to counter the adverse effects. The cure rate was 100%. Conclusions: The overall incidence of adverse effects related to the new treatment regimen recommended by the Brazilian Ministry of Health was high. However, none of those effects demanded a change in the regimen, which was effective in the patients evaluated.Keywords: Treatment outcome; Tuberculosis; Antitubercular agents; Adverse drug reaction reporting systems. The objective of this study was to determine the adverse effects of TB treatment with the RHZE regimen.

Estudo retrospectivo de perfil dos casos de TBMR do Espírito Santo, entre 2000 e 2004. Identificou-se 61 pacientes com TBMR, sendo a amostra composta por 57 casos, que tiveram seus prontuários revisados para obtenção das variáveis estudadas. Estimou-se como prevalência para a TBMR combinada 0,87% (0,66 - 1,13; IC95%). O teste anti-HIV foi feito em 78,9% dos casos, sendo 11,1% positivos. Investigando co-morbidades, destacaram-se etilismo e tabagismo. Encontrou-se 11 casos de resistência primária (RI) e 46 de resistência adquirida (RA), com média de 2,3 ± 1,3 tratamentos anteriores. Em 35,1% dos casos houve relato de contato prévio conhecido com doente de tuberculose, enquanto em 67,9% não houve ou nega-se ter havido. Dez pacientes (17,5%) foram tratados com o esquema de 1ª linha, 18 (31,6%) com o de 2ª linha e 27 (47,4%) com o de 3ª linha. Dezoito (31,6%) tiveram tratamento auto-administrado, e 39 (68,4%) supervisionado. Quanto ao desfecho, houve cura em 33 casos (71,7%), abandono em 7 (15,2%) e óbito em 5 (10,9%), 1 caso de falência e 11 (19,3%) estão em tratamento. Dos 10 casos encerrados de RI, 80% (8/10) foram curados, contra 69,4% (25/36) dos casos de RA. Concluímos que a prevalência de TBMR é baixa no ES. A cura foi alcançada em 70% dos casos. As co-morbidades podem ser importante fator interveniente para um desfecho satisfatório do tratamento. Os resultados enfatizam a necessidade de busca, diagnóstico e realização de TSA para identificação da RI e a supervisão do tratamento de todos os casos de tuberculose.

As ações de educação em saúde para crianças e adolescentes nas unidades básicas da região de Maruípe no município de VitóriaActions of education health for child and adolescents in the city of Vitória

O objetivo deste estudo foi avaliar a aplicabilidade da escala de escala de Coelho (EC) e determinar a sua adequação à realidade observada nas famílias acompanhadas pela Unidade de Saúde Maria Rangel Passos (USFMRP) no município de Vitória (ES). Trata-se de um estudo exploratório-descritivo de corte transversal, desenvolvido na região de saúde da Grande Maruípe. A princípio, foi feita a classificação das famílias segundo a EC e os resultados foram apresentados a um grupo de discussão, que propôs mudanças e possíveis alterações na EC, sendo a nova ferramenta denominada como "critério UFES". Para verificar a aplicabilidade desse instrumento e realizar uma comparação com os resultados obtidos da classificação pela EC, foram selecionadas, de forma aleatória, e visitadas, inclusive pelo ACS, trinta famílias atendidas na USFMRP. Esta nova escala permitiu identificar um maior número de famílias de risco mínimo e orientar as diversas ações que são realizadas no cotidiano de uma Unidade de Saúde, com o objetivo de definir prioridades e reorientar as práticas exercidas pelos profissionais rumo à consolidação de uma assistência mais equânime e integral, centrada no atendimento das reais necessidades sociais em saúde das famílias atendidas no âmbito da Estratégia de Saúde da Família.
