Comparison of the malaria parasite and mammalian protein prenyltransferases and their cellular substrates is important for establishing this enzyme as a target for developing antimalarial agents. Nineteen heptapeptides differing only in their carboxyl-terminal amino acid were tested as alternative substrates of partially purified Plasmodium falciparum protein farnesyltransferase. Only NRSCAIM and NRSCAIQ serve as substrates, with NRSCAIM being the best. Peptidomimetics, FTI-276 and GGTI-287, inhibit the transferase with IC 50 values of 1 and 32 nM, respectively. Incubation of P. falciparum-infected erythrocytes with [ 3 H]farnesol labels 50-and 22-28-kDa proteins, whereas [ 3 H]geranylgeraniol labels only 22-28-kDa proteins. The 50-kDa protein is shown to be farnesylated, whereas the 22-28-kDa proteins are geranylgeranylated, irrespective of the labeling prenol. Protein labeling is inhibited more than 50% by either 5 M FTI-277 or GGTI-298. The same concentration of inhibitors also inhibits parasite growth from the ring stage by 50%, decreases expression of prenylated proteins as measured with prenyl-specific antibody, and inhibits parasite differentiation beyond the trophozoite stage. Furthermore, differentiation specific prenylation of P. falciparum proteins is demonstrated. Protein labeling is detected predominantly during the trophozoite to schizont and schizont to ring transitions. These results demonstrate unique properties of protein prenylation in P. falciparum: a limited specificity of the farnesyltransferase for peptide substrates compared with mammalian enzymes, the ability to use farnesol to label both farnesyl and geranylgeranyl moieties on proteins, differentiation specific protein prenylation, and the ability of peptidomimetic prenyltransferase inhibitors to block parasite differentiation.Malaria continues to be a major disease in tropical areas of the world. Parasite resistance to current drugs used in the treatment of malaria has lead investigators to seek out new drug targets. Among those targets are proteins and enzymes, which are necessary for cellular division and differentiation. Such targets include the protein prenyltransferases, which are necessary for the post-translational modification of proteins involved in the signal transduction pathways and in regulation of DNA replication and cell cycling (1-3). These enzymes are currently a major focus of efforts to design drugs that inhibit unregulated cell growth in cancer (4, 5). Several candidate compounds show great promise as antitumor drugs and are currently being tested in clinical trials. Purpose More effective, less toxic treatments for recurrent ovarian cancer (OVCA) are needed. Although >60% of OVCAs express the estrogen receptor (ER), ER-targeted drugs have been disappointing due to drug resistance. In other estrogen-sensitive cancers, estrogen activates Src to phosphorylate p27 promoting its degradation and increasing cell cycle progression. Since Src is activated in most OVCAs, we investigated whether combined Src and ER blockade by saracatinib and fulvestrant would circumvent antiestrogen resistance. Experimental Design ER and Src were assayed in 338 primary OVCAs. Dual ER and Src blockade effects on cell cycle, ER target gene expression, and survival were assayed in ERα+ OVCA lines, a primary human OVCA culture in vitro, and on xenograft growth. Results Most primary OVCAs express ER. Src activity was greater in OVCA lines than normal epithelial lines. Estrogen activated Src, ER-Src binding and ER translocation from cytoplasm to nucleus. Estrogen mediated mitogenesis was via ERα, not ERβ. While each alone had little effect, combined saracatinib and fulvestrant increased p27, inhibited cyclin E-Cdk2 and cell cycle progression. Saracatinib also impaired induction of ER-target genes Myc and FOSL1; this was greatest with dual therapy. Combined therapy induced autophagy and more effectively inhibited OVCA xenograft growth than monotherapy. Conclusions Saracatinib augments effects of fulvestrant by opposing estrogen-mediated Src activation and target gene expression, increasing cell cycle arrest and impairing survival, all of which would oppose antiestrogen resistance in these ER+ OVCA models. These data support further pre-clinical and clinical evaluation of combined fulvestrant and saracatinib in OVCA.

O objetivo desta pesquisa foi descrever os eventos adversos identificados pelo familiar/cuidador em uma Unidade de Tratamento Intensivo Pediátrico (UTIP). Trata-se de pesquisa qualitativa exploratório-descritiva realizada com treze familiares/cuidadores em um hospital pediátrico de Porto Alegre-RS, no período de março a abril de 2009. Utilizou-se entrevista semiestruturada como técnica de coleta dados que foram analisados mediante análise do tipo temática. Das informações emergiram as seguintes categorias: Falhas no processo de comunicação; Práticas não seguras relacionadas à atuação profissional; Requisitos para segurança do paciente na UTIP. A criança neste ambiente encontra-se exposta frente à assistência ofertada. Acredita-se que a segurança da criança hospitalizada em UTIP necessita da incorporação de práticas de cuidado seguro como cultura institucional direcionada a prevenir eventos adversos.
Trata-se de uma pesquisa qualitativa, descritiva exploratória do tipo revisão integrativa da literatura. Teve por objetivo analisar as publicações sobre prática clínica simulada, ou simulação realística, aplicada nos cursos de graduação em Enfermagem como estratégia de ensino e aprendizagem para o desenvolvimento das competências recomendados pelas Diretrizes Curriculares Nacionais (DCNs). As bases de dados exploradas foram LILACS, PUBMED, COCHRANE e SCIELO, abrangendo o período de 2010 - 2020. A coleta de dados realizou-se por meio de um instrumento estruturado utilizando o modelo Preferred Reporting Items for Systematic Reviews and Meta-Analyses – PRISMA. Após a leitura exploratória dos resumos e a aplicação dos critérios de inclusão e exclusão pré-estabelecidos, foram selecionadas 18 publicações, cujo conteúdo foi lido integralmente e destas 14 publicações compuseram estudo final apresentados por meio de um quadro sinóptico. Os resultados evidenciaram que o uso da simulação realista como estratégia de ensino tem sido de interesse nacional e internacional , aproximando os estudantes e profissionais aos contextos reais simulados, oferecendo completa segurança para os envolvidos, buscando desenvolver não somente habilidades técnicas, como também o gerenciamento de crises, liderança, trabalho em equipe, raciocínio clínico em situações críticas ou que possam provocar prejuízos ao paciente real, sendo uma ferramenta poderosa de aprendizagem. A discussão dos principais achados dessas publicações e seus desfechos finais estão sendo compilados em um artigo científico, visando contribuir com a produção de conhecimento para os cursos de graduação em enfermagem.
OBJETIVO: Relatar a construção de uma casa simulada para implementação de cenários de alta fidelidade na realização de visita domiciliar pelos acadêmicos de Enfermagem com vistas à atenção primária à saúde. MÉTODO: Relato de experiência de professores realizado no período de março de 2019 a novembro de 2020. RESULTADO: A casa simulada exigiu esforço na montagem de sua infraestrutura física, pois conta com acústica controlada, equipamentos para gravação, espelho semitransparente e mobiliário funcional. O cenário implementado trata de um atendimento de um paciente com deficiência auditiva e que apresenta hipertensão arterial. Foi utilizado, também, um aplicativo para telefone móvel que auxilia na linguagem de sinais pode ser considerada como algo funcional e proveitoso. CONCLUSÃO: O cenário estruturado apresentado mostra uma maneira elegante de trabalhar as visitas domiciliares com os estudantes, que poderão exaurir seus esforços em um ambiente controlado para dilatar suas habilidades e tornarem-se regentes de qualidade quando chegado o momento da atuação profissional.
Objective: to analyze the profile of pediatric patient safety incidents in a university hospital, with a particular focus on comparing notifications before and during the COVID-19 pandemic and on Nursing professionals' perception. Method: a mixed-methods and convergent strategy study conducted at a university hospital from southern Brazil. The data were collected from a database with 1,663 notifications between March and September 2021. After applying the inclusion criteria, a total of 418 records of patient safety incidents occurred between January 2019 and December 2020 were found, which were analyzed by descriptive statistics with SPSS 18.0; semi-structured interviews were carried out with 18 Nursing professionals, using thematic content analysis. Results: the safety incidents recorded in the pediatric units were more related to intravenous medications and fluids, male gender and infant age group. There were no relevant differences when comparing the notifications from 2019 (219) with those from 2020 (199), with a reduction of only 20 (-9.13%). In the professionals' perception, there was also no expressive change in this situation. Conclusions: the characteristics of the patient safety incidents that occurred in pediatric units showed few statistically significant differences in the periods investigated, corroborating the Nursing professionals' perception that there was no significant change in the institution's scenario regarding incidents. Strategies to encourage notifications and improvements in this system are still required to provide a safe environment to pediatric patients.
RESUMO Objetivo: analisar o perfil de incidentes de segurança do paciente pediátrico em hospital universitário, com foco particular na comparação das notificações antes e durante a pandemia de COVID-19 e na percepção de profissionais de enfermagem. Método: estudo de método misto e estratégia convergente, realizado em hospital universitário, no sul do Brasil. Os dados foram coletados de um banco de dados de 1.663 notificações entre março e setembro de 2021. Após critérios de inclusão, totalizaram 418 registros de incidentes de segurança do paciente, ocorridos entre janeiro de 2019 a dezembro de 2020, que foram analisados por estatística descritiva com o SPSS 18.0; foram realizadas entrevistas semiestruturadas com 18 profissionais de enfermagem, sendo empregada a análise de conteúdo temática. Resultados: os incidentes de segurança ocorridos nas unidades pediátricas foram mais relacionados a medicações e fluídos intravenosos, sexo masculino e faixa etária dos lactentes. Não houve diferenças relevantes comparando as notificações de 2019 (219) com as de 2020 (199), havendo redução de apenas 20 (-9,13%) notificações. Na percepção dos profissionais, também não houve mudança expressiva nesse panorama. Conclusões: as características dos incidentes de segurança do paciente ocorridos em unidades pediátricas apresentaram poucas diferenças estatisticamente significativas nos períodos investigados, corroborando a percepção dos profissionais de enfermagem de que não houve mudança expressiva no panorama de incidentes da instituição. Estratégias de incentivo às notificações e aprimoramento desse sistema ainda são necessários para proporcionar um ambiente seguro ao paciente pediátrico.
