INTRODUÇÃO: o texto descreve a contribuição do Sistema Único de Saúde (SUS) para a atenção integral à saúde dos trabalhadores de acordo com a Política Nacional de Segurança e Saúde no Trabalho, considerando a organização das Redes de Atenção e o papel de coordenação atribuído à Atenção Primária em Saúde (APS). MÉTODO: realizaram-se revisão bibliográfica e análise de documentos normativos para identificar ações de atenção aos trabalhadores a serem desenvolvidas pelas equipes da APS. RESULTADOS: as ações foram organizadas em seis grupos: a) diagnóstico situacional, incluindo a identificação das atividades produtivas; b) caracterização do perfil demográfico e epidemiológico dos trabalhadores; c) planejamento das ações com priorização de intervenções sobre problemas de saúde de maior frequência, risco e vulnerabilidade; d) assistência ao trabalhador vítima de acidente ou doença relacionada ao trabalho; e) ações educativas e de promoção da saúde; f) ações caracterizadas como de Vigilância em Saúde do Trabalhador. Discute-se as potencialidades e limitações da APS para desenvolver as ações e recomenda-se o fomento da educação permanente e o matriciamento das ações pelos Centros de Referência em Saúde do Trabalhador (Cerest) e outras instâncias do SUS.
RESUMO A Política Nacional de Atenção Básica (PNAB) 2017 propõe alterações na composição e na atuação das equipes, com implicações no trabalho do Agente Comunitário de Saúde (ACS). O objetivo deste estudo foi investigar os sentidos que os ACS, atuantes em territórios do Norte de Minas Gerais, imprimem às mudanças propostas pela PNAB e os possíveis desdobramentos para o seu trabalho e para o cuidado à população. Foram realizados oito grupos focais, com participação aproximada de 15 ACS por grupo, no período de julho a dezembro de 2018. Os resultados apontaram distintas implicações da PNAB 2017, que, por um lado, demostram a possibilidade de ampliação do escopo de atuação do ACS com a população residente em áreas rurais e, por outro, revelam uma descaracterização da natureza do seu trabalho educativo, com ameaça à existência da categoria e fragilização dos princípios da universalidade e da integralidade na atenção à saúde da população rural. Há que se discutir as condições e a sobrecarga de trabalho do ACS que atua em área rural e a necessidade de reorganizar o processo de trabalho das equipes da Atenção Primária à Saúde para fazer com que um cuidado mais ampliado e equânime chegue aos domicílios rurais.
Resumo Introdução: a garantia da atenção integral à saúde do trabalhador requer a incorporação sistemática de ações na atenção básica, sendo o apoio matricial uma estratégia potente para sua viabilização. Objetivo: descrever e analisar as práticas de apoio matricial desenvolvidas por um Centro de Referência em Saúde do Trabalhador (Cerest) junto à rede de serviços de atenção básica, com vistas à incorporação de ações de saúde do trabalhador. Métodos: estudo de natureza descritiva. Realizou-se entrevista semiestruturada com 41 profissionais que participaram do processo de matriciamento. Os dados foram analisados segundo a técnica de análise de conteúdo. Resultados: o apoio matricial possibilitou a ampliação de práticas que reconhecem o usuário enquanto trabalhador e o trabalho como determinante do processo saúde-doença. Com o aumento da resolutividade dos casos e maior articulação entre os profissionais do Cerest e da rede de atenção básica, houve redução dos encaminhamentos aos Cerest. Os desafios identificados incluem a necessidade de qualificar apoiadores e o enfrentamento da sobrecarga de trabalho e da ênfase nas ações assistenciais que ocorrem em detrimento das de vigilância, dificultando a integralidade do cuidado. Conclusão: o apoio matricial pode operar como estratégia potente de fortalecimento da saúde do trabalhador no Sistema Único de Saúde (SUS).
This study sought to recognize the perceptions of family health teams regarding work-health-illness relationships, and to identify strategies, facilities and difficulties relating to providing workers with healthcare.Focus groups were conducted among primary care professionals in three Brazilian cities: Palmas (TO), Sobral (CE), and Alpinópolis (MG). The Bardin technique for content analysis was used to organize and analyze the data. The results showed that the healthcare actions provided to workers were unsystematic and out of line with the guidelines and objectives of the Brazilian National Occupational Health Policy. The major problems identified were: work overload; unpreparedness among the teams regarding issues involving work-healthillness relationships; and lack of institutional support, among others. Central organizational support for healthcare teams, provided by occupational health reference centers and other parts of the Brazilian National Health System, was identified as a facilitator of actions.
