Objetivo: caracterizar las alteraciones genómicas (cromosómicas y moleculares) identificadas en pacientes pediátricos con diagnóstico de leucemia mieloide aguda y sus patrones de coocurrencia, con miras a identificar factores pronósticos y eventuales terapias personalizadas. Materiales y métodos: muestras remitidas con diagnóstico de leucemia mieloide aguda por clínica y hematopatología entre las edades de 0 y 18 años. Se realizó estudio cromosómico, pruebas moleculares para los genes FLT3, NPM1 y CEBPA, y panel de secuencia de nueva generación (NGS) para trastornos mieloides de 30 genes. Resultados: cohorte de 41 pacientes, 26 hombres y 15 mujeres. Veintiséis pacientes presentan alteraciones citogenéticas (63 %), de los cuales 11 (27 %) tienen alteraciones del Core-Binding Factor (7, t(8;21); 4, inv(16)); 6 pacientes (15 %) con compromiso del gen MLL (KMT2A) y 9 (22 %) con otras alteraciones cromosómicas. En cuanto a las alteraciones moleculares identificadas por pruebas rápidas y NGS, en 29 pacientes (71%) se encontró la presencia de al menos una mutación en genes recurrentemente mutados en esta patología. Se identificaron mutaciones en 16 genes, entre los cuales están: gen FLT3 en 11 pacientes (38 %); (7, ITD;4, TKD); KIT 9 (31 %); NRAS 9 (31 %); KRAS 4 (14 %); CBL 4 (14 %); WT1 4 (14 %); CEBPA 4 (14 %) (2 bialélicos); ASXL1 4 (14 %); PTPN11 3 (10 %); RUNX1 3 (10 %); NPM1 2 (7 %); ETV6 2 (7 %); TET2 2 (7 %); IDH1 1 (3 %); CSF3R 1(3 %); EZH2 1(3 %).
