Introducción: la leucemia linfoblástica aguda de precursores B (LLA-B) es la neoplasia más frecuente en la infancia. Las alteraciones genéticas más comunes en niños son t(12;21), t(4;11) y Philadelphia-like (Ph-like). El estudio de estas alteraciones es muy importante para la estadificación y el tratamiento adecuado de los pacientes. En el año 2009 se describió el perfil de expresión Ph-like, asociado a alto riesgo de recaída. Varios estudios han descrito frecuencias de más del 15 % en población pediátrica. Además, se ha reportado una mayor prevalencia de estas alteraciones en pacientes con ancestros hispanos, lo cual puede ser explicado por la asociación con rearreglos de CRLF2 en este grupo étnico. Los pacientes con alteración en el perfil de expresión Ph-like tienen características de mal pronóstico z como alto recuento de leucocitos al diagnóstico, falla de remisión y enfermedad mínima residual (EMR) positiva al final de la inducción. La guía de práctica clínica para el diagnóstico de leucemia linfoide aguda en niños, niñas y adolescentes del Ministerio de Salud y Protección Social (2013), recomienda la búsqueda de algunas mutaciones recurrentes; no está incluida la evaluación del perfil de expresión génica Ph-like. Objetivo: identificar la prevalencia de Philadelphia-like en <18 años con LLA de precursores B (LLA-B) y su asociación con enfermedad mínima residual (EMR).
