Objective: To evaluate the presence of clinical lipodystrophy in children with the acquired immunodeficiency syndrome and to relate it to the antiretroviral regimen employed, to changes in lipid profile and to insulin resistance.Methods: This was a cross-sectional study that evaluated 30 children and adolescents (median age = 9.1 years) with the acquired immunodeficiency syndrome during 2004 and 2005. The following clinical and laboratory evaluations were performed: classification of HIV infection, anthropometric measurements (weight and height), serum glycemia, serum insulin and lipid profile (LDL-c, HDL-c, triglycerides). Lipodystrophy was diagnosed using clinical parameters. The chi-square test was used for statistical analysis. Results:All of the patients were taking antiretroviral therapy regularly (median duration of 28.4 months); 80% were on three drugs in combination (highly active therapy) and 30% were on protease inhibitors. Lipodystrophy and dyslipidemia were observed in 53.3 and 60% of the patients, respectively. Children on a highly active therapy regimen with protease inhibitors exhibited a higher percentage of mixed lipodystrophy; the difference between these children and the group on highly active therapy without protease inhibitors and the group not on a highly active therapy was statistically significant (44.4 vs. 16.7%; p = 0.004). There was no statistically significant association between the presence of lipodystrophy and sex, age (> 10 years), changes to the lipid profile or insulin resistance. Conclusions:The elevated prevalence of dyslipidemia and lipodystrophy observed among children with acquired immunodeficiency syndrome, which exhibited a relationship with the antiretroviral regimen employed, may represent an increased risk for future complications, in particular cardiovascular problems. A deficiência de ferro é o distúrbio nutricional mais comum no mundo e constitui a maior causa de anemia associada às condições onde há erro alimentar, perda crônica de sangue ou quando ocorre o crescimento rápido, como na infância, na gravidez e na adolescência. Esta deficiência acarreta prejuízos no desenvolvimento neuropsicomotor, na capacidade de aprendizagem, no apetite, no crescimento e na resposta do sistema imunológico. Na adolescência, além de com frequência observarmos hábitos alimentares inadequados, estão presentes intensas mudanças fisiológicas e psicossociais que, em associação, podem comprometer o crescimento e aumentar o risco do desenvolvimento de deficiência de ferro e outras carências nutricionais, sobretudo na fase púbere. Desta forma, o diagnóstico de deficiência de ferro entre os adolescentes deve ser lembrado a fim de que medidas possam ser tomadas para diminuir a incidência de anemia, do comprometimento do rendimento escolar e do sistema imunológico, neste período da vida.
Despite the high frequency of dyslipidemia, lipodystrophy, and retinol and beta-carotene deficiencies, it was not possible to demonstrate a correlation of these findings with lipid peroxidation and insulin resistance. More studies are needed to investigate the causes of retinol and beta-carotene deficiencies in this population and the clinical consequences of these findings.
OBJETIVO: Avaliar a prevalência de alterações hepáticas ao ultra-som e de elevação da alanina aminotransferase (ALT) em crianças com sobrepeso e obesidade, além de relacionar a presença de alterações com peroxidação lipídica, perfil lipídico e resistência insulínica. MÉTODOS: Estudo transversal, prospectivo e controlado de 67 crianças com sobrepeso/obesidade (38 do sexo feminino e média de idade de 8,6 anos), pareadas por sexo e idade com 65 eutróficas. A avaliação bioquímica consistiu de perfil lipídico (LDL-c, HDL-c e triglicerídeos); ALT; substâncias reativas ao ácido tiobarbitúrico (TBARS); glicemia e insulina, utilizadas para o cálculo do Homeostasis Model Assessment Insulin Resistance (HOMA-IR). A esteatose hepática, avaliada por ultra-sonografia por um único examinador, foi classificada em difusa leve, moderada e grave. RESULTADOS: Elevação de ALT (>40U/L) foi encontrada em 3% e alterações ultra-sonográficas do fígado em 57,4% das crianças com sobrepeso e obesidade. O Grupo Sobrepeso/Obesidade apresentou maior percentual de ALT >18U/L (OR 4,2; p=0,0006) e de hipertrigliceridemia (OR 4,2; p<0,001), em relação ao controle. A ALT associou-se com aumento de triglicerídeos séricos (OR 3,2; p=0,010). Não houve associação entre os níveis de TBARS (estresse oxidativo) e do HOMA-IR com a presença de sobrepeso/obesidade, nem com ALT >18U/L. CONCLUSÕES: A freqüência de ALT >40U/L foi baixa e a de esteatose foi alta. As alterações ultra-sonográficas hepáticas não mostraram associação com o estado nutricional nem com os níveis de ALT. Por sua vez, triglicerídeos aumentados associaram-se com níveis mais elevados de ALT.
