Objective: To assess feeding practices and dietary intake of healthy infants in three Brazilian municipalities.Methods: By means of a prospective study, we analyzed the food record of 7 consecutive days of an intentional sample (quota and weighted sampling) of 179 healthy infants, aged between 4 and 12 months, from the municipalities of Curitiba, São Paulo, and Recife, who were not being exclusively breastfed. Mothers received oral and written information provided by a nutritionist with the purpose of standardizing the feeding data. The computer program NutWin was used to calculate the dietary intake. Results:The median of the infants' age was 6.8 months (4.0-12.6 months). We found that 50.3% of the infants were no longer being exclusively breastfed. Of these, 12.0 and 6.7% among the infants younger and older than 6 months, respectively, were fed with infant formulae instead of breast milk. Therefore, most infants received whole cow's milk. Infant formula dilution was correct in only 23.8 and 34.7% of the infants younger and older than 6 months old, respectively. With regards to complementary feeding, we found that the median age was 4 months for its introduction and 5.5 months for the introduction of family diet. There was high quantitative inappropriateness of micronutrient intake for infants between 6 and 12 months old who were not exclusively breastfed, mainly in terms of zinc (75%) and iron (45%). Conclusion:The present study showed a high frequency of inappropriate feeding practices and dietary intake in very young infants. BackgroundAtaxia telangiectasia (A-T) is a neurodegenerative disease that leads to mitochondrial dysfunction and oxidative stress. Insulin resistance (IR), type 2 diabetes and the risk for development of cardiovascular disease was recently associated as an extended phenotype of the disease. We aimed to assess IR; liver involvement; carotid intima-media thickness (cIMT) and metabolic alterations associated to cardiovascular risk in A-T patients, and relate them with age.ResultsGlucose metabolism alterations were found in 54.6% of the patients. Hepatic steatosis was diagnosed in 11/17 (64.7%) A-T patients. AST/ALT ratio > 1 was observed in 10/17 (58.8%). A strong positive correlation was observed between insulin sum concentrations with ALT (r = 0.782, p < 0.004) and age (r = 0.818, p = 0.002). Dyslipidemia was observed in 55.5% of the patients. The apolipoprotein (Apo-B)/ApoA-I ratio (r = 0.619; p < 0.01), LDL/HDL-c (r = 0.490; p < 0.05) and the Apo-B levels (r = 0.545; p < 0.05) were positively correlated to cIMT.ConclusionsMetabolic disorders implicated in cardiovascular and liver diseases are frequently observed in adolescent A-T patients and those tend to get worse as they become older. Therefore, nutritional intervention and the use of drugs may be necessary.
OBJETIVO: Avaliar a freqüência de síndrome metabólica em crianças e adolescentes com sobrepeso ou obesos, além de relacionar a sua presença com variáveis socioeconômicas, atividade física e risco cardiovascular familiar. MÉTODOS: Por meio de estudo transversal, 59 pacientes com sobrepeso/obesidade, matriculados em ambulatório universitário, foram avaliados entre janeiro de 2004 e junho de 2006. Coletaram-se os seguintes dados: nível socioeconômico, escolaridade materna, risco cardiovascular familiar e atividade física. Utilizou-se o percentil do índice de massa corpórea (IMC) para classificar a condição nutricional. Considerou-se síndrome metabólica quando presentes três ou mais das seguintes alterações: triglicérides (>110mg/dL), HDL-c (<40mg/dL), circunferência abdominal (> percentil 90), resistência insulínica (glicemia >100mg/dL) e pressão arterial (> percentil 90). A análise estatística foi descritiva. RESULTADOS: Dos pacientes, 52,5% eram do gênero masculino e a média de idade foi 10,9±0,5 anos. A síndrome metabólica esteve presente em 42,4%. Inadequações das variáveis que compõem a síndrome metabólica foram observadas em 88,1% para a circunferência abdominal; 47,5% com hipertensão arterial; 23,7% com resistência insulínica; 42,4% com aumento dos triglicérides e 6,8% com elevação do HDL-c. Não se observou associação estatisticamente significante entre síndrome metabólica e gênero, idade, desenvolvimento puberal, renda per capita, escolaridade materna, horas de televisão/dia, horas de atividade física ou risco cardiovascular familiar. CONCLUSÕES: É alta a prevalência de síndrome metabólica em crianças e adolescentes obesos avaliados e, dentre os seus componentes, a circunferência abdominal e a pressão arterial elevadas são as mais freqüentes.
BackgroundVitamin D deficiency is a global public health issue. More than half of pregnant women are affected by vitamin D insufficiency/deficiency. Studies suggest an association between low vitamin D concentrations during pregnancy with intrauterine growth restriction and prematurity. This study aimed to describe the concentrations of 25(OH)D (25-hydroxyvitamin D) of mothers who delivered preterm newborns compared to women with full-term pregnancy deliveries, as well as to relate 25(OH)D blood concentrations of mothers with those of their newborns.MethodThis cross-sectional study was conducted with 66 mothers who had given birth to preterm babies and their preterm newborns (PTNB, < 32 weeks), and 92 women who had given birth at the full-term of their pregnancy and their newborns (FTNB). Data were collected on the characteristics of mothers (gestational age, diseases, and habits) and newborns (anthropometry and adequacy for gestational age). Ten milliliters of blood were drawn from the mothers and the umbilical cord of newborns at birth to identify the 25(OH)D, parathyroid hormone, calcium, phosphorus, and alkaline phosphatase concentrations.ResultsMothers in the PTNB group had significantly lower mean 25(OH)D blood levels (21.7 ± 10.8 ng/mL vs. 26.2 ± 9.8 ng/mL; p = 0.011) and were three times more likely to have insufficiency when compared to mothers in the FTNB group (OR = 2.993; 95%CI 1.02–8.74). Newborns in the PTNB group also had lower 25(OH)D concentrations compared to FTNB group (25.9 ± 13.9 ng/dL vs. 31.9 ± 12.3 ng/dL; p = 0.009). There was a directly proportional correlation between mother and newborn umbilical cord 25(OH)D concentrations in PTNB (r = 0.596; p < 0.001) and FTNB (r = 0.765; p < 0.001).ConclusionMothers who delivered preterm babies and their preterm newborns had lower 25(OH)D concentrations compared to women who had given birth at the full-term of their pregnancy. In both groups, 25(OH)D concentrations of the mothers correlated directly with those of the newborns, and this correlation was higher in the full-term birth group. Nevertheless, the recommended universal vitamin D supplementation in pregnant women to curb the risk of preterm birth is still incipient. More studies are required to clarify the particularities of vitamin D metabolism further and define the adequate 25(OH)D concentrations throughout pregnancy.
