Bien que les réanimations ne comprennent en général qu’une faible proportion des lits hospitaliers; elles constituent la scène d’une forte proportion des infections nosocomiales La gestion du risque infectieux en réanimation constitue aujourd’hui une priorité; l’un des axes stratégiques prioritaires est la mise en place d’un système de surveillance épidémiologique. Nos objectifs étaient de déterminer l’incidence et les Aspects Cliniques afin d’identifier les facteurs de risque. Il s’agit d’une étude descriptive longitudinale d’incidence incluant les patients ayant dépassés 48 heures dans le service de réanimation polyvalente à l’hôpital Ibn El Jazzar de Kairouan sur une période d’une année allant de 01/03/2013 jusqu’au 28/02/2014. L’enquête a porté sur 265 patients dont l’âge moyen était 39±20 ans (18 à 93 ans) et le sexe ratio (H/F) a été de 2,48. Nous avons identifié 125 épisodes d’infections nosocomiales chez 81 malades soit une incidence de 30,6%. La densité d’incidence était de 55 infections pour 1000 jours d’hospitalisation. On a notifié une nette prédominance des pneumopathies avec une incidence de 27,73%, suivies des infections urinaires de 9,73%, des infections liées au cathétérisme veineux central 6,25% et des infections du site opératoire de 2,34%. Le taux de mortalité parmi les enquêtés a été de 28,7% avec une différence significative entre les patients infectés (44,7% des cas) et ceux non infectés (29,07%) (p < 10-3). Les microorganismes étaient des BGN dans 80% des cas. La prévention ne peut se concevoir que sous la forme d’une action globale et multidisciplinaire.
L'antibioprophylaxie (ATBP) est l'une des mesures spécifiques de prévention des infections du site opératoire dont l'impact a été quantifié dans les interventions chirurgicales propres et propres contaminées. L'objectif de notre étude est d'évaluer la conformité des pratiques d'ATBP et le respect des prescripteurs des protocoles adoptés au sein de notre établissement. Notre travail est une étude observationnelle rétrospective, de type « Audit Clinique », évaluant les pratiques d'ATBP dans notre établissement durant le mois de mars 2015. Le critère de jugement principal était la conformité globale des pratiques observées aux 5 critères majeurs définis par la haute autorité française de santé (HAS). Nous avons adopté le référentiel de la société française d'anesthésie réanimation parue en 2010. Nous avons inclus 150 patients opérés des services de chirurgie générale, d'orthopédie et d'urologie. Le taux de conformité globale était de 33,3%. La conformité de chacun des 5 critères majeurs définis par la HAS était de 74% pour l'indication; 84% pour le délai injection-incision; 60% pour le choix de l'ATB; 89,3% pour la dose de 1ère injection et 72% pour la durée de l'ATBP. La conformité était variable selon le service, meilleure en urologie ; en cas de chirurgie programmée et quand le prescripteur est un réanimateur. Une stratégie globale intégrant organisation, éducation et restriction, pourrait permettre une réelle amélioration des taux de conformité des pratiques d'ATBP. La réalisation d'audits successifs devrait s'inscrire dans une démarche régulière, afin d'évaluer l'impact des actions entreprises.
Les intoxications aiguës par les pesticides organophosphorés (OP) au cours de la grossesse sont des événements rares, peu décrites dans la littérature. A travers cette étude rétrospective, nous rapportons les résultats de sept cas d’ingestion suicidaire d’OP chez des femmes enceintes. Cette intoxication était le plus souvent grave. En effet, cinq des sept parturientes avaient un Glasgow initial < 9 et le POP score était ≥ 3 chez toutes les parturientes. Cinq patientes avaient nécessité une ventilation mécanique de durée moyenne de 3,4 jours. L’évolution était favorable pour toutes les patientes mais dans plus de la moitié des cas elle était défavorable pour le fœtus (mort fœtal in utéro). Deux mécanismes peuvent expliquer ces complications fœtales. Le premier est l’hypoxie fœtale associée ou non à un état de choc qui peuvent se traduire au niveau du RCF par une tachycardie ou des décélérations et aboutir ainsi à un décès in utero. Le deuxième mécanisme est le passage de ces pesticides à travers la barrière placentaire, ce qui pose un risque potentiel pour le fœtus par altération des systèmes enzymatiques des microsomes.
Le partogramme est un outil incontournable dans la pratique quotidienne en salle de naissance. C'est le reflet écrit de la qualité de la prise en charge materno-foetale pendant l'accouchement et le post-partum immédiat. Le but de notre travail est d'évaluer la conformité de la tenue du partogramme au sein de notre maternité et de proposer des axes d'amélioration de la qualité de sa tenue. il s'agit d'une étude rétrospective par audit clinique, effectuée sur 400 dossiers des patientes ayant accouché dans la maternité universitaire de Kairouan du 1er Janvier au 31 décembre 2014. Le référentiel utilisé est celui de la Haute Autorité de Santé (2006) comportant 29 critères divisés en 3 domaines (PARTOten, PARTOobs, PARTOeve). le Taux de Conformité Globale (TCG) de la tenue du partogramme dans notre audit était de 55,9%. Pour le domaine I « La tenue du partogramme » (PARTOten), le TCG était de 88,9%. Pour le domaine II « la traçabilité du déroulement du travail » (PARTOobs), le TCG était de 51,4%. Pour le domaine III « la traçabilité des actes, des évènements et des traitements au cours du travail » (PARTOeve), le TCG était de 27,4%. Notre étude a permis de dégager plusieurs points à améliorer. Le but ultime de l'Audit Clinique est l'amélioration des pratiques professionnelles, de ce fait nous allons mettre en place un plan d'action (formation, sensibilisation…) qui sera suivi par une réévaluation afin de vérifier la pérennité des actions correctives.
