The aim: To analyse long-term results of treating patients with open fractures of low-leg bones within a specific time period and to describe concomitant problems. Materials and methods: A retrospective study was carried out to evaluate the results of the final treatment of patients after one year and after five years. Patients were divided into two groups. Group I included 47 (61.84%) patients for whom the treatment method was changed from external fixation to internal fixation. Group II included 29 (38.16%) patients for whom the final treatment of an open fracture was performed with an external fixation device without changing the method. The study exclusion criteria were patients under 18 years old and patients with gunshot fractures of the lower leg bones. Results: Patient treatment included initial surgical debridement and fracture stabilization with external fixation devices. In all patients, wound healing occurred within 30 days (on average, 24 ± 4.5 days). When evaluating the treatment of patients in group I, 32 (68.09%) of them underwent a one-stage removal of an external fixation device and osteosynthesis of fragments during the first 7-10 days. In 3 (20.0%) patients, osteosynthesis was performed with an intramedullary locking nail, in 12 (80.0%) patients – with a plate. The decision to change the fixation method was made taking into account clinical and laboratory parameters. It is noted that the treatment of patients with type III open fractures of the lower leg bones according to the Gustilo-Andersen classification, provided that the method of external fixation is replaced with an internal one, gives better results compared to osteosynthesis with external fixation devices. Conclusions: Treatment of patients with high-energy injuries of the lower leg bones is a long-term process. It is possible to improve the therapy efficiency by changing the fixation method from external to internal one. In this regard, the main evaluative characteristics are such indicators as uncomplicated wound healing combined with the absence of laboratory and clinical signs of inflammation.
Objective. To analyze the late complications after osteosynthesis of the acetabular fractures that led to reoperations; to determine the features of surgical intervention and the choice of the acetabular component during endoprosthetics in these patients. Methods. From 2009 to 2015, the results of endoprosthetics in patients (n=35) who underwent primary osteosynthesis of the acetabulum and subsequently hip arthroplasty were evaluated at the Orthopedic and Trauma Center of Kiev Regional Clinical Hospital. To assess damage volume, the Letournel-Judet classification was used. 5 years after the endoprosthetics to evaluate functional outcomes the the Harris Hip Scale (HHS) and radiographic method have been used. Results. The initial preoperative assessment in 35 patients using Harris Hip Scale showed results: 64 (58-71) Ме (LQ; UQ) points. A year after endoprosthetics when examining 33 (94.3%) patients the Harris scale improved the results to 81 (74-88) points (p<sub>0-1</sub><0.001). 5 years after arthroplasty the Harris scale was 85 (77-92) points (p<sub>0-5</sub><0.001). After 5 years in 31 (88.6%) patients a radiographic evaluation showed stable integration of the acetabular component without any signs of attenuation in 1-3 zones according to the De Lee and Charnley classification. Conclusion. The most effective way to treat the recent acetabular fractures with fragment displacement is considered to be the early osteosynthesis with anatomical reposition of fragments, which with the development of degenerative changes in the operated joint, makes it possible to perform endoprosthetics using a full-fledged bone mass for immersion of the acetabular component. Careful planning of the operation, preliminary removal of metal fixators, which can affect the placement of the acetabular component, as well as increase the risk of postoperative complications, allows achieving good results. What this paper adds For the first time the late complications after osteosynthesis of the acetabular fractures, which led to reoperations, have been analyzed; the features of surgical intervention and the choice of the acetabular component during endoprosthetics in such patients have been determined. The most effective method for treating recent acetabular fractures with fragment displacement has been studied in detail - early osteosynthesis with anatomical reposition of fragments, which, with the development of degenerative changes in the operated joint, makes possible to perform endoprosthetics using full bone mass for immersion of the acetabular component.
Цель. Изучить путем сравнения результаты применения при ОП классической тактики и тактики многоэтапного хирургического лечения, обсудить вопросы выбора метода фиксации ОП и возможности раннего закрытия обширных ран и дефектов мягких тканей. Материалы и методы. Проанализированы результаты лечения 286 пациентов с ОП костей верхних и нижних конечностей в ортопедо-травматологическом центре Киевской областной клинической больницы за период с 2008 по 2016 г. Применяли классические методы и тактику многоэтапного хирургического лечения. Результаты. Оценивали результаты хирургического лечения ОП костей верхних и нижних конечностей с использованием балльной шкалы Neer-Grantham-Shelton. Хорошие результаты получены у 54,84% пациентов 1-й группы и у 80,28% пациентов 2-й группы. Выводы. Успех лечения ОП зависит от ранней госпитализации, ранней радикальной хирургической обработки, стабильной фиксации сломанной кости, применения рациональных доз антибиотиков, коррекции иммунологического статуса. Одними из наиболее спорных и нерешенных остаются проблемы выбора метода фиксации ОП и раннего закрытия раны.

Л., 2019 Останніми десятиліттями невплинно зростає кількість переломів головки стегнової кістки (ГСК), які обумовлені підвищенням частоти дорожньо-транспортних пригод і виживаності після високошвидкісних травм. Мета: проаналізувати результати хірургічного та консервативного лікування переломів ГСК і обґрунтувати тактику лікування цього ушкодження. Методи: проведено ретроспективний аналіз результатів лікування пацієнтів із переломами ГСК з 2002 по 2017 р. Перед операцією постраждалим виконано рентгенографію і КТ кісток таза. Переломи розподілили за класифікацією Pipkin: тип I -10 (25,6 %) осіб, 9 %), 7 %), 8 %). Консервативне лікування після усунення вивиху виконано 8 (20,5 %) пацієнтам: тип Pipkin I -6 (15,4 %); Pipkin II -2 (5,1 %). Результати: виявлено ускладнення, серед яких гетеротопічна осифікаціяу 1 (3,0 %) пацієнта, асептичний некроз ГСК -7 (23,3 %); 2 -з переломами типу Pipkin II, 5 -Pipkin IV. У період від 12 до 36 міс. після травми у 32 (76,9 %) пацієнтів отримані такі результати лікування за шкалою 1 %),3 %),6 %),0 %). Висновки: у разі переломів типів Pipkin I і II досягнуто кращі функціональні результати порівняно з важчим типом перелому Pipkin IV. Пізнє відкрите вправлення невправлених переломовивихів призвело до розвитку асептичного некрозу ГСК в 66,7 % випадків. Вибір методу лікування переломів ГСК залежить від типу перелому. У разі переломів типу Pipkin I консервативне лікування привело до високого відсотка хороших результатів. За умов переломів типу Pipkin II кращі показники спостерігали після відкритого вправлення й остеосинтезу фрагментів, Pipkin III -ендопротезування, Pipkin IV -остеосинтезу ГСК і перелому кульшової западини. Among all fractures, pelvic injuries are 5-8 %, and combined trauma-30-58 %. The level of mortality due to unstable pelvic fractures ranges from 10 to 18 %, and in patients with polytrauma-36.5-50 %. Objective: to analyze the organizational problems of treating patients with pelvic injuries in Ukrainian clinics and to propose ways of their solutions. Methods: a retrospective study of surgical treatment results of patients with pelvic fractures was made. Objective. To specify the role and indications for application of аpparatus of external fixation (АEF) in treatment of unstable pelvic osseous injuries (UPOI); to study up the advantages and faults of the external fixation method; to share the experience of treatment, using this procedure. Маterials and methods. There were 229 patients, suffering pelvic injuries, examined on the base of orthopedic-traumatological centre of Kyiv’s Regional Clinical Hospital in 2005 - 2017 yrs and distributed into three Groups. In Group I 143 (62.4%) injured persons were included, in whom external osteosynthesis played the function of temporary measures of fixation, in Group II - 27 (11.8%) patients, in whom while hospitalization a fracture was fixed, using AEF, аnd after stabilization of the state the posterior pelvis osteosynthesis and reposition was conducted, in Group III - 59 (25.8%) patients, who, using several indications, were definitely treated, applying the external fixation method only. Results. Excellent effect of treatment in Group I was registered in 23 (19.0%), a good one - in 58 (47.9%), satisfactory - in 32 (26.4%), poor - in 8 (6.6%) of 121 patients; in Group II - accordingly, - in 5 (20.8%), 11 (45.8%), 7 (29.2%) and 1 (4.2%) of 24 patients; in Group III - accordingly, in 4 (8.3%), 12 (25%), 23 (47.9%) and 9 (18.6%) of 48 patients. Conclusion. Best results of treatment of patients with unstable pelvic osseous injuries were obtained while applying the tactics of change from external fixation into internal one. On the first stage the pelvic ring fixation, using AEF, was performed, аnd after stabilization of a patient general status - internal osteosynthesis. Application of the internal osteosynthesis method permits to control and stabilize the reposition achieved, to conduct the sacro-ileal joint revision.
Цель. Определение оптимальной тактики микробиологического обследования пациента с перипротезной инфекци- ей (ППИ) путем оценки достоверности результатов бактериологических исследований в пред– и послеоперацион- ном периоде. Материалы и методы. На базе Ортопедо–травматологического центра Киевской обласной клинической больни- цы на протяжении 2016 – 2018 гг. обследованы 32 пациента с подозрением на наличие ППИ. Во время предопераци- онной подготовки всем пациентам провели бактериологическое исследование, в ходе которого их разделили на две группы: 1–я – 23 пациента, которым проводили бактериологическое исследование посевов из свищевого хода; 2–я – 9 пациентов, которым осуществляли пункционный забор материала, так как свищевое отверстие у них отсутствовало. Результаты. Из 147 предоперационных посевов у 83 (56,5%) определены грамположительные кокки; у 49 (33,3%) – грамотрицательные палочки; рост микроорганизмов не определялся в 15 (10,2%) посевах. У 22 (68,7%) пациентов вы- делена монокультура, у 10 (31,3%) –два и больше микроорганизмов. Выводы. Диагностика ППИ является сложной и требует мультидисциплинарного подхода, предполагающего тесное сотрудничество ортопедов и микробиологов.
