Chorioangioma constitutes the commonest benign growth of placenta. It consists of a benign angioma arising from chorionic tissue. Chorioangioma of the placenta most often goes unnoticed. With the advent of gray scale and color flow ultrasonography in prenatal diagnosis these tumours can be easily detected antenatally. It has potentially serious fetal risks and so the pregnancy needs to have regular surveillance. The larger the size of the tumor the more is the chance of developing complications. Objective: To assess the prevalence and severity of pain in patients with endometriosis and to evaluate the relationship between the severity of pelvic pain with different stages of endometriosis.Methods: A prospective observational study conducted among 65 patients diagnosed with endometriosis during surgery in Infertility Management Center Dhaka, a specialized center for treatment of infertility and assisted reproductive technologies from January 2008 to January 2009.Result: Majority of the patients were between 26-30 years. Pelvic pain was the predominant symptom in 78.5% patients and the rest were asymptomatic. Among the symptomatic patients, most common symptom was dysmenorrhoea (n=47, 92.1%).When severity of pain was graded; it was observed that most (37%) presented with moderate pain. Infertility was present in 85% patients, mostly (n- 47 =74%) in primary sub fertility group. The commonest site of endometriosis was uterosacral ligament (n-58=89%) and ovarian endometriosis is noted in (63%) cases. Most of the lesions (35%) were black, haemosiderin deposits. When r-ASRM staging system was applied, majority (58.4%) of the patients was in stage IV disease and most of the patients (31%) with endometrioma were in stage IV disease. The study revealed a strong positive correlation between severe pain and stage IV disease (Correlation co efficient 0.711). Moderate forms of pain and severity of disease did not show any positive correlation in this study (Correlation co efficient 0.390). There was negative correlation between milder forms of pain with severity of disease.Conclusion: There was no relationship between frequency and severity of pain symptoms and disease stage of endometriosis.J Bangladesh Coll Phys Surg 2016; 34(3): 135-139

Background: Tubal evaluation is an integral part of the evaluation of female infertility. Hysterosalpingography (HSG) is the radiographic evaluation of the uterus and fallopian tubes, which has been used as a first-line test for tubal assessment since 1920s. Laparoscopy is considered to be the gold standard for the diagnosis and management of tubo-peritoneal pathology. The objective of this study is to evaluate and compare the diagnostic value of both HSG and laparoscopy for assessment of tubal occlusion. Methods: This is a prospective cross-sectional study. Laparoscopic chromopertubation was conducted on 125 consecutive infertile women who attended the “Infertility Management Center” from October 2018 to September 2019. All patients had HSG performed in the radiology department of different hospitals. Laparoscopic findings were used as a reference standard to analyze the findings of HSG for tubal occlusion. Data were analyzed by SPSS software (version 16). Results: Normal HSG findings were seen in 53 patients (42.4%). Unilateral tubal occlusion was present in 38 (30.7%), and bilateral occlusion in 34 (27.2%) patients. Laparoscopy showed normal findings in 85 (68.0%) patients. Unilateral tubal occlusion was demonstrated in laparoscopy in 27 (21.6%) and bilateral tubal occlusion in 13 (10.4%) patients. HSG findings for diagnosis of tubal occlusion was true positive in 30 (24%) cases, false positive in 42 (33.6%) cases, false negative in 10 (8%) cases, and true negative in 43 (34.4%) cases in comparison to laparoscopy findings. Sensitivity of HSG was found to be 75.0%, specificity 50.6%, and accuracy 58.4%. Positive predictive value and negative predictive value were 41.7% and 81.1%, respectively. Conclusion: HSG is widely used for tubal assessment in Bangladesh as it is a relatively inexpensive, low-risk office procedure. But it has a low specificity and positive predictive value with a relatively high negative predictive value. Laparoscopy, on the other hand, is preferable when there is high suspicion of tubal pathology.
