OBJECTIVE: To describe the characteristics of pain, kinesiophobia and quality of life in patients with chronic low back pain and depression. METHODS: Cross-sectional study in which 193 individuals with chronic low back pain were included. The presence of depression was measured by the Beck Depression Inventory, using a cutoff validated by the Mini International Neuropsychiatric Interview. The intensity and quality of pain in the groups with and without depression were assessed by the McGill Questionnaire. The Tampa Scale for Kinesiophobia was applied to assess fear of movement. With respect to quality of life, the Medical Outcomes Study 36 was used. The statistical significance level was set at p <0.05. RESULTS: The prevalence of depression was 32.1%. The group with depression had worse scores in relation to pain, kinesiophobia and quality of life (physical functioning, rolephysical, bodily pain, general health, vitality, social functioning, role-emotional, and mental health. Sr. Editor, O uso de quetiapina no tratamento de transtornos do humor vem se tornando frequente, inclusive com a prescrição do fármaco em dose única diária para tratamento de transtornos depressivos "unipolares" e bipolares 1 . Entretanto, há poucos dados sobre a possibilidade de a quetiapina induzir um episódio maníaco ou hipomaníaco 2 .Relatamos o caso de uma mulher com o diagnóstico de transtorno esquizoafetivo, tipo depressivo, baseado nos critérios do Manual Diagnóstico e Estatístico de Transtornos Mentais-4a Edição-Texto Revisado (DSM-IV-TR), que apresentou um episódio maníaco possivelmente induzido pelo uso de quetiapina.Trata-se de paciente de 49 anos, com início do quadro aos 27 anos de idade, sem resposta completa a diversas modalidades de tratamento, com a persistência de muitos sintomas depressivos (que frequentemente provocavam afastamento laboral) e sem história de episódios de mania, hipomania ou quadro misto. Já havia feito uso da combinação de tranilcipromina, risperidona, carbonato de lítio e eletroconvulsoterapia nos últimos três anos, sem recuperação funcional, apresentando apenas melhora na apatia e na remissão parcial dos sintomas esquizofreniformes (delírios de influência).A quetiapina foi prescrita em dose de 100mg, três vezes ao dia, durante quatro meses. Devido à ausência de resposta dos sintomas depressivos, a dose de 300mg de quetiapina foi prescrita em dose única diária, durante dois meses, com melhora quase completa dos sintomas depressivos e psicóticos, com recuperação funcional. Após 20 dias do aumento da dose para 500mg, uma vez ao dia, esta apresentou um episódio maníaco.Mania induzida por quetiapina em paciente com transtorno esquizoafetivo do tipo depressivo: um relato de caso Mania induced by quetiapine in patients with schizoaffective disorder, depressive type: a case report Este episódio caracterizou-se por humor irritável, auto-estima inflada, taquipsiquismo, jocosidade, recrudescimento dos delírios de influência e perseguição. Durante o episódio, submeteu-se à cirurgia plástica para fins estéticos e emprestou grande quantidade de dinheiro para familiares. A paciente apresentou melhora dos sintomas 10 dias após a diminuição da dose de quetiapina para 300mg uma vez ao dia, mantendo-se eutímica e com sintomas psicóticos arrefecidos.Há fatores que sugerem uma forte relação causal entre o uso da quetiapina e o episódio maníaco) 2 :-os sintomas nunca foram relatados antes do uso do fármaco, surgindo com aumento da dose e desaparecendo com a diminuição;-houve intervalo apropriado entre uso da medicação e início dos sintomas; -não houve relatos de uso ou interrupção do uso de outros medicamentos.Presume-se que o efeito antidepressivo da quetiapina e de seu metabólito, N-esaquilquetiapina, deva-se à inibição da recaptação de norepinefrina e ao aumento da neurotransmissão serotoninérgica pelo bloqueio de receptores 5-HT2A e da propriedade de agonista parcial de receptores 5-HT1A 3,4 .
