Introdução: Devido à frequente ausência de sintomas em sua fase aguda, a maior parte dos diagnósticos dos casos de hepatites virais é realizada inadvertidamente em doações de sangue ou triagem pré-cirúrgica. Objetivo: Inédito na região Norte Fluminense, o presente estudo definiu a soroprevalência das hepatites B e C entre doadores de sangue do Hemocentro do Hospital Ferreira Machado. Metodologia: Trata-se de um estudo transversal analítico retrospectivo, realizado por meio de análise de dados dos doadores de sangue do Hemocentro do HFM dos anos de 2004 a 2015. Os marcadores analisados foram anti-HBc e anti-HCV (IgG e total). Resultados: Um total de 86.815 amostras foram analisadas. Destes, 54.585 (62.9%) eram do sexo masculino e 32.230 (37.1%) eram do sexo feminino. A prevalência de anti-HBc positivo foi de 2.86% (2488), sendo a faixa etária de maior prevalência de anti-HBc positivo de 40 a 49 anos (25.76%, p<0.0001). Em relação ao anti-HCV, a prevalência foi de 0.67% (581), sendo a faixa etária mais acometida de 30 a 39 anos (38%, p<0.0001). A prevalência de soropositividade para os marcadores anti-HBc e anti-HCV foi maior entre os homens (74.1% e 70.6%, respectivamente). Conclusão: Quando comparadas as prevalências das hepatites virais entre triênios selecionados (2004-6, 2007-9, 2010-12 e 2013-15), notou-se relevante decréscimo (p<0.0001). Observou-se que, quando comparada à média nacional, a região Norte Fluminense apresenta uma baixa positividade para o anti-HCV e valores compatíveis com outras regiões do país em relação ao anti-HBc.
Background: The main origin of functional dependence in Brazil and the world is the ischemic stroke. The early thrombolytic treatment reduces this morbidity. However, the extended in-hospital delay is the main risk factor for the suboptimal number of ischemic stroke patients who receive this treatment. Objective: To describe the management of patients admitted with ischemic stroke in two Brazilian public hospitals. Methods: This cross-sectional study was carried out in two emergency units in 2016 in Campos dos Goytacazes: Hospital Ferreira Machado (HFM) and Hospital Geral de Guarus (HGG). We analyzed the prevalence of sex, age, risk factors, time of first symptoms, prescription of thrombolytic therapy and contraindications to its use. Results: None of the 130 patients with ischemic stroke received a prescription of rt-PA, even though HFM had rt-PA available throughout the period of the study. HGG does not keep this data documented. Of all, 41 patients died (31,54%). Advancing age (p < 0.0005) and atrial fibrillation 3.458 (CI = 95%, 1.239 to 10.16, p = 0.0238) were risks factors for the mortality in this population. Conclusions: The lack of information in medical records reveals that low staff training may be the main reason why thrombolytic therapy was not prescribed. The elevated mortality rate shows that medical care for patients admitted with ischemic stroke in our region is far from ideal.
Os pacientes reumatológicos podem desenvolver diversos sintomas gastrointestinais, sendo a diarreia um dos mais prevalentes e de mais difícil manejo. Em primeiro lugar, pela possibilidade de ser uma manifestação gastrointestinal de doenças autoimunes sistêmicas; além disso, por poder corresponder a um efeito colateral de medicações utilizadas no dia a dia do reumatologista; e, finalmente, pela possibilidade de ser um sintoma de doenças intestinais mais prevalentes em pacientes com desordens autoimunes. Dessa forma, o primeiro passo para manejo do paciente com diarreia é identificar os sinais de alarme que geram alerta para a possibilidade de casos graves, como doenças inflamatórias intestinais e neoplasias. Na presença desses sinais, é necessária a solicitação de exames invasivos como colonoscopia e o encaminhamento para o gastroenterologista. Nos pacientes que não apresentam sinais de alarme, a condução clínica deve ser dirigida para as queixas apresentadas, já que as possibilidades diagnósticas são amplas, como síndrome do intestino irritável, intolerâncias alimentares, supercrescimento bacteriano do intestino delgado e doença celíaca. Dessa forma, a pesquisa para cada uma delas pode ser realizada por meio de testes não invasivos. Além disso, o reumatologista deve ter em mente que os sintomas diarreicos podem corresponder a infecções oportunistas desenvolvidas devido à imunossupressão medicamentosa ou, até mesmo, à manifestação intestinal de desordens autoimunes sistêmicas. Desse modo, a condução desses pacientes é um desafio à prática clínica e em muitos casos há benefício do acompanhamento multidisciplinar.
