A prótese obturadora constitui o principal método para a reabilitação de grandes defeitos maxilares. Tem como objetivo fechar o defeito, separar a cavidade oral da cavidade nasal, prevenir a fala anasalada e a regurgitação nasal de alimentos e líquidos e dar sustentação ao perfil facial. Este artigo visa relatar um caso de reabilitação com prótese obturadora maxilar após maxilectomia parcial. Paciente do gênero feminino, 72 anos, diagnosticada com carcinoma adenoide cístico na região de palato, foi submetida a maxilectomia parcial que resultou em extenso defeito maxilar e comunicação bucosinusal. Após anamnese, exames extra, intraorais e radiográfico, foi planejado para o período cicatricial e de uso da sonda nasogástrica, a confecção de uma placa obturadora para melhorar a deglutição e fala. Em seguida, confecção de uma prótese total obturadora superior e prótese parcial removível inferior. A placa obturadora foi confeccionada e utilizada até a remoção da sonda e instalação das próteses. Após moldagem das duas arcadas, foram obtidos modelos funcionais que foram montados em articulador após os ajustes dos planos de orientação e registro das relações intermaxilares. Foram realizadas provas estéticas e funcionais dos dentes artificiais e ajustes necessários. Posteriormente à acrilização, as próteses foram instaladas e o ajuste oclusal executado. Para obter retenção e estabilidade satisfatórias durante a utilização da prótese obturadora superior foi sugerido o uso de adesivo para dentadura. Três proservações foram realizadas. A prótese obturadora reabilitou a estética, fonética, função mastigatória e deglutição restabelecendo a autoestima e a ressocialização da paciente.
A prótese bucomaxilofacial visa a reabilitação de pacientes que sofreram mutilações na face, restituindo estética e autoestima. Este trabalho visa relatar o caso clínico de um paciente reabilitado com prótese oculopalpebral após sofrer exenteração de órbita, decorrente de um carcinoma espinocelular em pálpebra inferior direita. Paciente, gênero masculino, 56 anos, procurou atendimento odontológico queixando-se de desconforto estético do rosto. Ao exame clínico foi observada ausência do globo ocular, pálpebras e arco superciliar do lado direito, por isto, foi planejada a confecção de uma prótese oculopalpebral. Foi realizada moldagem dos terços superior e médio da face, obteve-se o molde em alginato e, posteriormente, o modelo em gesso. Em seguida, foi confeccionado um globo ocular caracterizado em resina acrílica termopolimerizável. Posteriormente realizou-se, sobre o modelo de gesso, a escultura da área amputada utilizando-se plastilina e cera e após prova e ajustes no paciente, inclusão do conjunto modelo/escultura em mufla e contramufla, com posterior eliminação da peça esculpida. Foi selecionada a cor da pele do paciente e misturou-se uma base ao silicone, que foi incluído na mufla para prensagem. Após a vulcanização do silicone, foram realizados os acabamentos, caracterização e instalação da prótese. Na proservação o paciente relatou grande satisfação com a reconstituição da estética facial. Conclui-se que a prótese bucomaxilofacial é uma alternativa satisfatória para a reabilitação de pacientes que sofreram mutilações faciais, pois restabelece a estética facial, autoestima e convívio social. Dental ceramics have an excellent ability to reproduce natural teeth appearance, but once the substrate has an altered coloration, it is necessary to mask this intercurrence, considering the characteristics of the ceramic and cement selected. This article aims to present a clinical case of rehabilitation with ceramic laminates to mask a darkened central incisor by planning a more opaque porcelain structure under a translucent contact lens. A female patient sought dental care complaining of dissatisfaction with smile aesthetics. It was clinically identified the presence of a Class IV darkened restoration in composite resin in element 11. In treatment planning harmony in the shape and color of elements 11 and 21 was aimed at through lithium disilicate lenses. An opaque intermediate structure was used to cover the darkening in tooth 11. The double molding technique was performed to obtain the working model. Prior to cementing, the surfaces were treated. In ceramics, conditioning with 10% hydrofluoric acid and 37% phosphoric acid, silane application and universal adhesive; in dental substrate, conditioning with 37% phosphoric acid and application of universal adhesive were performed. In laminates definitive cementation Allcem Venner cement was chosen. Patient reported satisfaction with treatment final aesthetics. Thus, cementation technique of a translucent laminate over an opaque facet is a good option for masking substrates with darkened coloration.
The polymerization process of total dentures causes dimensional changes, making it necessary to remounting these prostheses in a semi-adjustable articulator (ASA) to perform the occlusal adjustment. The objective of this study is to report a clinical case of oral rehabilitation with conventional full dentures, which was performed by the clinical remounting of the prostheses in ASA to perform the occlusal adjustment. A 72 years old male patient sought dental care at the Polyclinic of the State University of Amazonas. After anamnesis, extra and intraoral clinical examinations were indicated for conventional full dentures. After anatomical and functional impressions, working models were obtained, on which orientation plans were made. From the individualization of the upper orientation plane, registration of the facial arch and determination of intermaxillary relations, the models were assembled in ASA. After selection, assembly and aesthetic and functional testing of artificial teeth, the prostheses were polymerized, finishing and polishing. During the installation of the prostheses, there were few occlusal contacts and an anterior open bite. With the prostheses positioned on the edges, the facial arch was recorded to remounting the upper prosthesis in ASA and interocclusal registration in the central relation position for remounting of the lower prosthesis. The occlusal adjustment was performed by means of selective wear to obtain the correct centric and eccentric occlusal contacts. Clinical remounting of total prostheses in ASA is an efficient procedure for performing occlusal adjustment and obtaining bilateral balanced occlusion.
Ressecções cirúrgicas de tumores maxilares podem causar comunicação bucosinusal que possuem como método de reabilitação a confecção de prótese obturadora maxilar. Sendo os aspectos biomecânicos o principal desafio para um tratamento protético eficaz. Este artigo visa relatar um caso de reabilitação com prótese obturadora maxilar após maxilectomia parcial. Paciente do gênero feminino, 14 anos, procurou atendimento odontológico desejando substituir a prótese obturadora provisória que havia sido confeccionada três anos atrás, para correção de defeito maxilar gerado pela remoção cirúrgica de um osteoma. Após anamnese, exames intra e extra-oral optou-se pela realização de uma prótese parcial removível superior com obturador palatino. Iniciou-se pela moldagem das duas arcadas para obtenção dos modelos de estudo onde foi realizado o delineamento e planejamento do caso. Após realização do preparo de boca e moldagem funcional, obteve-se o modelo de trabalho, utilizado para confecção da armação metálica que foi provada e ajustada na boca analisando os aspectos biomecânicos. Após registro do arco facial e determinação das relações intermaxilares os modelos foram montados em articulador semi-ajustável, executada a seleção e montagem dos dentes artificiais. Em seguida foi realizada a prova estética e funcional e ajustes necessários. Posteriormente a moldagem de boca fechada, foram realizadas a acrilização, polimerização, instalação e proservações da prótese. A prótese obturadora se mostrou eficaz, em seu aspecto biomecânico, gerando boa retenção e estabilidade. Sendo a prótese parcial removível uma alternativa favorável a reabilitação de pacientes submetidos a cirurgias mutiladoras maxilofaciais.
Modern dentistry seeks to offer increasingly esthetic treatments, aimed at patient’s well-being and satisfaction. In this perspective, this study aimed to conduct a literature review on the use of ceramic laminates as conservative and esthetic solution, addressing their indications, contraindications, limitations and its importance in dentistry. Literature review:The evidence in the literature show that the ceramic laminates are used for restoring anterior teeth, reestablishing dental function and esthetic mainly due to its excellent optical property, and preserving tooth structure, as it is necessary less tooth wear to return shape, texture, color and harmony to the teeth involved. Conclusion: It was conclude that this is a treatment option with high success rate when properly indicated, planned and executed according to each case.
