Lower extremity deformities of patients with arthrogryposis multiplex congenita present a wide spectrum of severity and deformity combinations. Treatment goals range from merely ensuring comfortable seating and shoe wear, to fully independent and active ambulation, but the overarching intention is to help realize the patient's greatest potential for independence and function. Treatment of hip and knee contractures and dislocations has become more interventional, whereas treatment of foot deformities has paradoxically become much less surgical. This article synopsizes the treatment strategies presented in September 2014 in Saint Petersburg, Russia at the second international symposium on arthrogryposis.
Patients with arthrogryposis multiplex congenita have a characteristic upper extremity resting posture consisting of internal rotation of the shoulders, elbow extension, flexed wrists, thumb-in palm deformities, and variable degrees of finger contractures. Treatment of these patients is aimed at improving independence and performance of activities of daily living. Although each area needs to be assessed independently for the most appropriate surgical procedure, often multiple areas can be addressed at the same operative setting. This limits the number of anesthetic exposures and cast immobilization time. The following is a synopsis of treatment strategies presented at the second international symposium on Arthrogryposis which took place in St Petersburg in September 2014.

Однако у пациентов с артрогрипозом выбор до-норских мышц значительно ограничен в связи с их гипоплазией или аплазией. Biesalski и L. Mayer (1916) [5]. Нам удалось найти лишь две работы, посвященные транспози-ции длинной головки трехглавой мышцы плеча в позицию двуглавой у пациентов с артрогрипо-зом [6,7]. Однако данные об отдаленных резуль-татах лечения таких пациентов отсутствуют, что обусловливает актуальность исследования.Цель исследования -проанализировать ре-зультаты изолированной транспозиции длинной головки трехглавой мышцы плеча в позицию дву-главой мышцы с целью восстановления активно-го сгибания предплечья у пациентов с артрогри-позом. Материалы и методы исследованияВ период с 2008 по 2014 г. в ФГБУ «НИДОИ им. Г. И. Турнера» было проведено клиническое, неврологическое, ультрасонографическое и физио-логическое обследование 29 пациентов (35 верх-них конечностей) с артрогрипозом в возрасте от 10 мес. до 15 лет. У всех пациентов было резко ограничено или отсутствовало активное сгибание предплечья, у 13 из них также отсутствовало или было ограничено пассивное сгибание в локтевом суставе.
Цель. Оценить результаты восстановления активного сгибания в локтевом суставе путем монополярной транспозиции широчайшей мышцы спины (ШМС) у больных с артрогрипозом и выявить корреляцию с уровнем сегментарного поражения спинного мозга. Материалы и методы. С 2011 по 2018 год в ФБГУ "НИДОИ им. Г.И. Турнера" было выполнено восстановление активного сгибания в локтевом суставе у 30 больных с артрогрипозом (44 верхних конечности) путем монополярной пересадки ШМС. Возраст пациентов на момент операции составил от 1 года до 10 лет (средний возраст 3,98 ± 2,35). Проводились клиническое, неврологическое обследование пациентов. Полученные данные были подвергнуты статистической обработке. Результаты. По уровню сегментарного поражения спинного мозга все пациенты были разделены нами на следующие группы: уровень С6-С7 (8 больных, 13 конечностей)-29,6 %, С5-С7 (17 детей, 24 конечности)-54,5 %, С6 (5 пациентов, 7 конечностей)-15,9 %. Результаты лечения были изучены в сроки от 1 года до 7 лет после операции (3,2 ± 1,9). После операции пассивное сгибание в локтевом суставе составило 100 ± 7,0° (min 80°, max 110°), активное сгибание 90,5 ± 14,7° (min 40°, max 110). Дефицит разгибания в локтевом суставе увеличился в 18 случаях (51 %) на 12,8 ± 4,8° (min 10°, max 20°), что, однако, не вызвало ограничения возможности выполнения пациентом основных бытовых навыков. Амплитуда активных движений в локтевом суставе составила 75,4 ± 18,0° (min 40°, max 110°). Хорошие результаты были отмечены в 20 случаях (55,6 %), удовлетворительные-в 12 (33,3 %), неудовлетворительные-в 4 (11,1 %). При оценке разницы изменений активного сгибания после операции, а также дефицита разгибания в зависимости от уровня сегментарного поражения спинного мозга у пациентов с уровнями поражения С6-C7, С5-С7 корреляции не было выявлено. Заключение. ШМС является надежным аутотрансплантатом, позволяющим восстановить активное сгибание в локтевом суставе у больных артрогрипозом с уровнями сегментарного поражения С6, С6-С7, C5-C7 в тех случаях, когда сила донорской мышцы составляет 4 балла и более, а пассивное сгибание в локтевом суставе не менее 90°. Introduction. Ponseti method is a widespread treatment for clubfoot in children with arthrogryposis. Closed subcutaneous achillotomy in these patients could not completely rectify the equinus deformity due to tissue rigidity which often leads to reconsideration of the tenotomy principles. Aim. This study aimed to formulate the anticipating criteria to assess the effectiveness of achillotomy in order to develop a different achillotomy approach for children with arthrogryposis. Materials and methods. This study retrospectively analyzed closed subcutaneous achillotomy in 28 patients (56 feet) with arthrogryposis. The mean age of the patients was 5.4 months (range 2–8 months). The children were subdivided into two groups according to the residual equinus deformity after the completion of Ponseti serial casting. All patients were physically and radiographically examined. Results and discussion. The first group included 12 patients (24 feet), which achieved foot neutral position or dorsiflexion ≥5° after achillotomy. The second group consisted of 16 patients (32 feet) with residual equinus after achillotomy who required surgery. X-ray images showed that the patients in the second group had significantly wider tibiocalcaneal angle and smaller talocalcaneal angle in lateral view (р < 0.01). The correction values of the equinus deformity after achillotomy in the children with arthrogryposis were greatly limited: 27° (20°–30°) and 19° (10°–30°) in the first and second groups, respectively. Conclusion. Closed subcutaneous achillotomy for effective equinus elimination during clubfoot treatment by Ponseti method should be performed only after complete correction at the level of tarsal joints. X-ray examination of the feet is recommended for the children with arthrogryposis in order to evaluate the talocalcaneal divergence and heel position more comprehensively. Furthermore, the values of tibiocalcaneal and talocalcaneal angles in lateral view prior to achillotomy are essential prognostic factors of its effectiveness. Moreover, the severity of equinus contracture should be considered prior to achillotomy. Achilles tenotomy is inappropriate if equinus deformity exceeds 30°. In such cases, open surgery should be considered.
Операция открытого вправления бедра в настоящее время является методом выбора для детей грудного возраста с амиоплазией, однако сопровождается высокой частотой осложнений в виде релюксации и подвывиха (11-46%), а также аваскулярного некроза головки бедренной кости (АНГБК) (5-63%). Цель исследования -оценить эффективность операции открытого вправления бедра у детей с вывихом бедра и различными вариантами контрактур тазобедренных суставов при амиоплазии и проанализировать причины неблагоприятных исходов. Проанализированы результаты 25 операций открытого вправления бедра, выполненных 21 пациенту с амиоплазией, сроки наблюдения составили от 2,6 до 6 лет. Использованы клинический, лучевой и статистический методы. У детей с отводящими контрактурами тазобедренных суставов после открытого вправления бедра хорошие результаты отмечены в 1 случае (17%), удовлетворительные в 3 наблюдениях (50%), неудовлетворительные в 2 случаях (33%). Причинами неблагоприятных исходов (релюксация, подвывих, АНГБК III группы по Kalamchi-MacEwen) в 4 случаях (83%) являлись неустранимые наружноротационная контрактура и ретроторсия бедренной кости, не позволяющие достигнуть стабильного концентрического вправления, а также недоразвитие наружного и переднего краев вертлужной впадины. После операции открытого вправления бедра, выполненной пациентам с приводящими контрактурами, отмечалось доразвитие наружного и переднего краев вертлужной впадины, уменьшение ее антеверзии, что привело к хорошим результатам лечения в 17 случаях (90%), к удовлетворительным в 2 наблюдениях (10%). После данной операции в 2 случаях (10%) наблюдались осложнения (подвывих, АНГБК III группы по Kalamchi-MacEwen). Операция открытого вправления бедра у детей грудного возраста с приводящими контрактурами тазобедренных суставов при амиоплазии значимо эффективнее по сравнению с пациентами с отводящими контрактурами (р≤0,05). BACKGROUND: The absence of active elbow flexion is the most common problem in children with amyoplasia, leading to daily living difficulties. Many variants of muscle transfer are used for the restoration of active elbow flexion. The pectoralis major and latissimus dorsi muscles are the most used muscles for this purpose; however, the optimal age for these operations is not reported in the literature. AIM: This study aimed to determine the optimal age of children with amyoplasia for the restoration of active elbow flexion. MATERIALS AND METHODS: The retrospective study involved 61 patients (90 upper limbs) with amyoplasia (30 [49%] girls and 31 [51%] boys) who were examined and treated between 2011 and 2020. In 46 (51.1%) cases, we used major muscles, and in 44 (48.9%) cases, the latissimus dorsi muscle was used as a donor muscle. All patients were divided into four groups: group 1 included children aged 13 years (n = 17, 27.9%); group 2, 37 years (n = 30, 49.2%); group 3, 711 years (n = 8, 13.1%), and group 4, 1218 years (n = 6, 9.8%). The clinical examination of the patients was conducted before and after the operation (6 months). Statistical data processing was performed using Statistica 10 and SAS JMP 11. To describe the numerical scales, the average value and standard deviation (M SD) were used. RESULTS: The age of the patients at the time of surgery was 5.16 3.72 years, and the postoperative follow-up period was 41.93 30.13 months. Elbow flexion contractures were observed mainly in groups 13 (p 0.05). The greatest changes in indicators such as the strength of forearm flexor muscles, active elbow flexion, and function of the elbow were noted in group 1 (p 0.05). The same postoperative indicators were worse in group 4 than in younger patients (p 0.05). Groups 3 and 4 had less strength of the donor muscles than groups 1 and 2 (p 0.05). CONCLUSIONS: The retrospective analysis of the results of the restoration of active elbow flexion in children with amyoplasia allowed us to recommend these operations in children aged 13 years. The prevention of elbow flexion contractures and the formation of a new stereotype of movement help improve the self-ability of these patients and the treatment results.
