Invasive fungal infections, an important cause of mortality, are primarily treated using amphotericin B, which is available in different formulations, both conventional and lipid-based (liposomal, lipid complex, colloidal dispersion and Intralipid infusion). The aim of our study was to determine the efficacy and safety of conventional amphotericin B vs its lipid-based formulations. A systematic review followed by pairwise meta-analysis was performed, including randomised controlled trials (RCTs) that evaluated the use of lipid-based amphotericin B in patients with any degree of immunosuppression and susceptibility to invasive fungal infection. An electronic search was conducted using PubMed, Scopus, Web of Science and Scielo databases. Extracted outcomes were related to efficacy (cure) and safety (incidence of adverse events). Results were evaluated and meta-analyses were performed. Twenty-three RCTs were identified (n=2677 participants) for meta-analysis. No significant differences between conventional amphotericin B and any of the five formulations evaluated were observed, with regard to the efficacy analysis. With respect to the adverse events of nephrotoxicity, fever, chills and vomiting, all lipid formulations presented better profiles than the conventional formulation. The present systematic review and meta-analysis showed that conventional amphotericin B presents the same efficacy profile as lipid-based formulations, although the latter were associated with a safer profile.
The aim of this study is to gather evidence of head-to-head double-blind randomized-controlled trials on the efficacy and safety of available treatments for attention deficit hyperactivity disorder (ADHD) in children and adolescents. A systematic review was conducted by two independent reviewers in ten electronic databases (PROSPERO register CRD42016043239). Methodological quality of included studies was evaluated according to the Jadad scale. Network meta-analyses were performed including double-blinded head-to-head trials comparing active allopathic drugs in patients (0-18 years old) diagnosed with ADHD. The results of efficacy and safety of atomoxetine (ATX), bupropion, buspirone (BSP), dexamphetamine, edivoxetine (EDX), guanfacine (GXR), lisdexamfetamine (LDX), methylphenidate (MPH), mixed amphetamine salts, modafinil, pindolol (PDL), reboxetine (RBX), selegiline, and venlafaxine were analyzed using ADDIS software v.1.16.5. Forty-eight trials were identified (n = 4169 participants), of which 12 were used for efficacy analysis and 33 for safety analysis. On the CGI-I scale, the analysis revealed that MPH was more effective than ATX and GXR. For the safety outcomes, according to drug ranks, LDX was more likely to cause sleep disorders (39%) as well as loss of appetite (65%) and behavior problems such as irritability (60%). BSP (71%) and EDX (44%) caused less appetite decrease. For behavioral effects, PDL was considered safest (50%). For any adverse events, RBX (89%) was the safest alternative. The lack of head-to-head trials properly reporting outcomes of interest limited some comparisons. Network meta-analysis offered a broader overview on the available treatments for ADHD, especially for safety issues, and contributes towards evidence gathering and clinical practice decisions. A core outcome set for ADHD should be designed to guide the conduction and report of clinical trials.
RESUMO:A Erythrina velutina é popularmente conhecida como suinã, mulungu, corticeira, mulungu-da-catinga dentre outros. Das várias espécies distribuídas pelo mundo cerca de doze estão no Brasil. A casca é utilizada, principalmente no nordeste brasileiro, pelas propriedades sudorífi ca, calmante, emoliente, peitoral, anestésica local e outras. Considerando-se que não existem estudos sobre o potencial microbiológico das cascas de Erythrina velutina este trabalho teve por objetivo avaliá-lo preliminarmente contra oito bactérias patogênicas. Foram utilizados os métodos de difusão em disco e concentração inibitória mínima para o extrato etanólico bruto e difusão em disco para a fração hexano. A atividade contra o Staphylococcus aureus e Streptococcus pyogenes foi evidenciada para ambas as amostras. A viabilidade de futuras pesquisas com outras frações e substâncias isoladas na busca de novas alternativas, visando uma terapêutica racional a partir de fontes naturais foi apontada pelos resultados.Unitermos: Erythrina velutina, Fabaceae, atividade antibacteriana.ABSTRACT: "Preliminary study of the antibacterial activity of Erythrina velutina Willd. Fabaceae (Leguminosae) bark". Erythrina velutina is popularly known as suinã, mulungu, corticeira, mulungu-da-catinga among others. Of the several species spread around the world about twelve are found in Brazil. Mainly in the Brazilian northeast the bark is used due to its sudorifi c, sedative, emollient, pectoral and topical anesthetic properties. Considering that there are no studies about the antibacterial potential of Erythrina velutina, this research aimed at evaluating, at least preliminarily, bark activity against eight pathogenic bacteria. The crude ethanol extract was tested by the disk diffusion method and minimal inhibitory concentration (MIC) and the hexane fraction by the disk diffusion method. Both samples demonstrated activity against Staphylococcus aureus and Streptococcus pyogenes. The results pointed to the viability of future research on other fractions and substances isolated from Erythrina velutina in search for new rational therapeutic alternatives based on natural sources. RESUMO:A procura por novos agentes antimicrobianos a partir de plantas vem sendo intensificada e não existem estudos direcionados à avaliação da atividade antimicrobiana de C. jamacaru frente a microrganismos prejudiciais ao homem. Em vista disso, avaliou-se o efeito antimicrobiano do extrato bruto etanólico do lenho (EBLE) e do córtex (EBCO) desta espécie frente a colônias puras de oito microrganismos patogênicos. Neste estudo foram utilizados os métodos difusão em ágar (meio sólido) e CIM (concentração inibitória mínima). Os resultados obtidos neste ensaio preliminar mostraram que EBCO apresentou atividade antimicrobiana mais pronunciada que EBCE e que os microrganismos cujo crescimento foi mais influenciado por EBCO foram Streptococcus epidermidis, Staphylococcus aureus, Pseudomonas aeruginosa e Escherichia coli. The search for new antibiotic agents from plants is growing, and no study regarding the activity of C. jamacaru pathological microorganisms has been carried out. This research evaluates the antibiotic effect of crude ethanolic extracts of the cortex (EBCO) and wood (EBLE) of the species over eight microorganisms. Two methods were used: agar diffusion and MIC (minimal inhibitory concentration). EBCO showed better inhibition results than EBLE. Streptococcus epidermidis, Staphylococcus aureus, Pseudomonas aeruginosa and Escherichia coli are the microorganisms that were better inhibited by EBCO. INTRODUÇÃOAs cactáceas apresentam grande potencial como fonte de substâncias de uso medicinal, cosmético e alimentício (Biavatti et al., 2007;Mariath et al., 2009) Cereus jamacaru DC, Cactaceae, popularmente conhecido como mandacaru, é um cacto colunar característico da região semi-árida do Brasil. Para o gado, o uso justifica-se pela capacidade de armazenar grande quantidade de água (cerca de 15%). A quantidade de proteína bruta chega a mais de 10% e o resíduo mineral a 10,66%, dos quais 0,22% são em P 2 O 5 e 5,61% em CaO (Braga, s.d.). O uso medicinal popular é pouco difundido; diz-se que as raízes e o caule são diuréticos e melhoram males do coração. Toda a planta é usada no combate ao escorbuto e nas afecções do aparelho respiratório -bronquites, tosse, catarro (Scheinvar, 1985). As raízes são utilizadas nas doenças respiratórias e renais, e como diuréticas (Agra et al., 2007 e Agra et al., 2008. Dentre as substâncias químicas identificadas no mandacaru está a tiramina, conhecida por sua atividade simpatomimética e provável responsável pela atividade cardiotônica (Brhun & Lindgren, 1976).Considerando o desafio que a descoberta de novos fármacos a partir de plantas, este trabalho teve como objetivo realizar uma avaliação preliminar do potencial antimicrobiano in vitro do extrato bruto etanólico de Cereus jamacaru e da substância isolada (tiramina) frente a culturas puras de algumas bactérias relacionadas à saúde humana. "Contribution to the allelopatic study of Erythrina velutina Willd., Fabaceae". This work has used ethanol extract and respective fractions from Erythrina velutina in allelophatic rehearsal with seeds from Lactuca sativa. It was observed that, except for the ethyl acetate fraction, the ethanol extract and fractions has been capable to influence the germination and growth of the seeds from L. sativa.
A Erythrina velutina Willd. é conhecida popularmente como mulungu, suas cascas são utilizadas principalmente pela população do nordeste brasileiro devido a suas propriedades calmante, sudorífica, emoliente e anestésica local. A pesquisa teve como propósitos o isolamento de substâncias químicas, a partir de técnicas cromatográficas, presentes no extrato bruto e frações deste (hexano, diclorometano, acetato de etila e hidroalcoólica remanescente). A identificação dos compostos isolados foi realizada por meio da análise dos dados das espectroscopias de ultravioleta, infravermelho e ressonância magnética nuclear de carbono e hidrogênio; a verificação da atividade antibacteriana do extrato bruto foi realizada através dos testes de difusão em disco e concentração inibitória mínima e, da fração hexano por teste de difusão em disco, além disso também foi procedida a verificação da atividade alelopática na germinação e alteração do crescimento da radícula e hipocótilo das sementes de Lactuca sativa para diferentes concentrações do extrato bruto e frações, os dados foram analisados estatisticamente pela análise de variância e teste de comparação múltipla de médias de Scott & Knott. Na fração hexano foram identificados o ácido fênico, ácido cinâmico, a-amirina, estigmasterol, b-amirina, b-sitosterol e lupeol por cromatografia gasosa e isolados o estigmasterol em mistura com o b-sitosterol. Da fração diclorometano foram isolados dois alcalóides e as estruturas propostas são eritralina, que foi identificada por comparação de seus dados espectrais com os dados publicados e já foi isolada desta espécie, 8-oxo-11b-metoxieritralina e eritrinina (não relatadas para a espécie); da fração hidroalcoólica remanescente isolou-se um composto que exibe algumas características de açúcar monocíclico, ainda não completamente identificado. O extrato bruto e a fração hexano demonstraram atividade antibacteriana moderada, em relação aos antibióticos controle, contra o Staphylococcus aureus e Streptococcus pyogenes. A germinação das sementes de Lactuca sativa não foi inibida e o crescimento da radícula e hipocótilo destas plântulas para o extrato bruto e frações foi estatisticamente significativo e diferente dos controles com água e etanol, a fração hidroalcoólica remanescente teve a maior atividade e alterou a morfologia das plântulas causando enovelamento, ausência de pêlos radiculares e ausência de folíolos. Cabe salientar que a atividade antibacteriana, a atividade alelopática e a presença de estigmasterol e o composto isolado da fração hidroalcoólica remanescente ainda não haviam sido citadas na literatura conforme levantamento bibliográfico para Erythrina velutina. PHITOCHEMICAL AND BIOLOGICAL STUDY FROM ERYTHRINA VELUTINA WILLD., FABACEAE (LEGUMINASAE- PAPILIONOIDEAE) BARK. Abstract Erythrina velutina Willd. is popularly known as mulungu, and its bark is used especially by the population of the Brazilian northeast owning to its sedative, sudorific, emollient and topic anaesthetic properties. This research is aimed at isolating the chemical substances in the crude extract and its partitions (hexane, dichloromethane, ethyl acetate and hydro alcoholic remaining fractions), through chromatographic techniques. The analysis of the spectrum copies of ultraviolet, infrared and nuclear magnetic resonance of carbon and hydrogen were used in the identification of the isolated compounds. The antibacterial activity of the crude extract was verified by using disk diffusion method and minimal inhibition concentration (MIC), and hexane fraction activity was verified through the disk diffusion method, and besides that the allelopathic activity in the germination and growth of the radicle and plantule of the Lactuca sativa seeds was checked at different concentrations of crude extract and fractions. Data were statistically examined by analysis of variance (ANOVA) and Scott Knott multiple comparison test. The hexane fraction GC showed fenic acid, cinnamic acid, a-amirin, stigmasterol, b-amirin, b-sitosterol and lupeol by gas chromatography; stigmasterol was isolated and identified as the major compound in the mixture with b-sitosterol. Two alkaloids were isolated from the dichloromethane fraction and the structures found were: erythraline that has been identified by comparing its spectral characteristics to its published ones and has been isolated from this gender, and 8oxo11(-methoxyerythraline and erythrinin which had not been related to this gender before; from the remaining hidroalcoholic fraction it was possible to segregate a compound that seems to have monocyclic sugar characteristics, but has not been thoroughly identified so far. The crude extract and the hexane fraction gave evidence of moderate antibacterial activity in relation to the antibiotics control, against Staphylococcus aureus and Streptococcus pyogenes. The germination of Lactuca sativa seeds was not inhibited and the growth of the radicle and hypocotyls to the amount of crude extract and fractions were statistically significant and different from the water and ethanol control. The remaining hidroalcoholic fraction was the most active and that caused the plantules morphology to show alterations such as curling, radicle hair absence and follicle absence. It seems important to emphasize that antibacterial activity, allelopathic activity, stigmasterol presence and the isolated compound from the remaining hidroalcoholic fraction had never been mentioned before in literature according to a bibliographic survey of Erythrina velutina.
RESUMO:Conhecida popularmente como áster-arbustiva, margarida-de-são-miguel e montecassino, Aster lanceolatus é uma planta ornamental de corte. Sabendo-se que não existem estudos que atestem a atividade biológica desta espécie, procurou-se neste trabalho atividades antibacteriana e antifúngica. Para tanto se utilizaram extratos brutos etanólicos de duas porções distintas, das fl ores e dos caules com folhas. Para a atividade antibacteriana, oito bactérias patogênicas foram submetidas a ensaio de difusão em gel e concentração inibitória mínima. Demonstrou-se atividade do extrato bruto etanólico de fl ores contra Streptococcus pyogenes, em difusão em gel e atividade de extrato bruto etanólico de caules e folhas contra Salmonella typhimurium e Streptococcus pyogenes em concentração inibitória mínima. Para a atividade antifúngica, utilizaram-se três fungos patogênicos em ensaios de crescimento micelial em placas e bioautografi a direta. No ensaio de crescimento micelial em placas verifi cou-se a inibição de Fusarium oxysporum e na bioautografi a direta, inibição do Cylindrocladium spathulatum. Os resultados delinearam uma nova fonte de pesquisa, as plantas ornamentais. Estas podem ser fonte de constituintes químicos capazes de servirem como protótipos para novos agentes terapêuticos e para tratamento sanitário de plantas medicinais. Popularly known as aster-arbustiva, margarida-de-são-miguel and monte-cassino, Aster lanceolatus is an ornamental plant. Having the knowledge that there is no studies on the biological activity of this species, this work aimed to check the antibacterial and antifungal activities. The ethanol extracts of the fl owers and of the stems with leaves were used. For the antibacterial activity eight pathogenic bacteria were submitted to the diffusion test in gel and minimal inhibitory concentration. The activity of the ethanolic extract of the fl owers has been demonstrated against Streptococcus pyogenes in diffusion in gel and the activity of the ethanol extract of the stems and leaves against Salmonella typhimurium and Streptococcus pyogenes in minimal inhibitory concentration. For the antifungal activity, three pathogenic yeasts have been used in the micelial growth tests in plates and direct bioautography. In the micelial growth tests in plates the inhibition of Fusarium oxysporum has been verifi ed and in the direct bioautography, inhibition of the Cylindrocladium spathulatum. The results indicated a new research source, the ornamental plants. These can be the source of chemicals capable of serving as prototypes for new therapeutic agents and for sanitary treatment of medicinal plants.
