The aim of this study was to understand how physicians from a team in the Brazilian Family Health Program perceive their work in child health, as compared to the program's principles outlined in the Agenda for Commitment to Comprehensive Child Health and Reduction of Infant Mortality, under the Ministry of Health. The backdrop was the strategy for implementation of primary care under the Unified National Health System (SUS). Semi-structured interviews were held, and the material was submitted to content analysis. According to the findings, in general the Agenda is being met. However, there are difficulties with referral and counter-referral; the measures proposed by the Family Health Program require medical and sociological competence and face problems inherent to biomedical training; the infrastructure and inputs are precarious; and the training provided does not prepare physicians sufficiently for a more comprehensive approach. In conclusion, the primary care model in Brazil requires adjustments to the country's reality, and partnerships that transcend the system are necessary.
The perceptions of the physicians that completed their residency in Obstetrics/Gynecology in 2004 at the Fernandes Figueira Institute, of the Oswaldo Cruz Foundation, about their residency process are investigated. The research comprised two steps, participant-observation and the use of oral sources. The methodological procedure includes a qualitative analytical coding of the interviews and a subsequent semiotic analysis. The study aims at understanding the cultural heritage of the residents' discourses regarding their relations with and within the health and illness environment. Some of the elements discussed in this paper were: medicine as a depreciated profession; regret for the loss of the power of medical knowledge; the perception that medicine is seen more as a business than as a profession; the uncertainties of being simultaneously a professional and a student; the difficulties of getting in touch with the suffering and death of another human being.KEY WORDS: internship and residency. profesional practice. medical education.Investiga-se a percepção dos médicos que concluíram a residência em Obstetrícia/Ginecologia no Instituto Fernandes Figueira, Fundação Oswaldo Cruz, em 2004, sobre essa trajetória. Analisou-se como o médico do Programa de Saúde da Família (PSF) de Duque de Caxias percebe sua atuação. Foram feitas entrevistas semiestruturadas com 15 médicos que trabalham no primeiro, terceiro e quarto distritos, sendo o primeiro urbano e os demais rurais. Aplicou-se aos dados a análise de conteúdo, na modalidade temática. Os núcleos de sentidos referiram-se à motivação e ao preparo para ingressar na equipe, e a oportunidade que o PSF oferece para estabelecer vínculos com o paciente, reforçando o acolhimento, a escuta e a responsabilidade. Estabeleceram-se: a) a maioria se ressente da opinião que os colegas fazem dela; b) a preparação ofertada é eminentemente teórica; c) vínculo, escuta e responsabilidade são avanços conquistados na realização profissional, e d) as bases da racionalidade médica formatam a organização das práticas, traduzindo uma concepção de saúde que se define pela ausência das doenças. Consubstancia-se uma lacuna entre a realidade e a proposta do governo de fazer do PSF uma via de reorganização da atenção básica.
Este artigo discute o papel do processo narrativo como elemento constituinte da prática médica. A pesquisa se baseou nos entendimentos que médicos, ao final de um programa de residência médica em Pediatria, fazem da Semiologia Médica, como estrutura fundamental à construção do raciocínio clínico. Os entrevistados se revelaram conscientes da importância de seus conhecimentos médicos, bem como da experiência clínica, posicionando-se como semiotas no ouvir, observar e examinar seus pacientes, aperfeiçoando o "olhar clínico", que, de acordo com as falas, perpassa o palpar e o visualizar, remetendo ao escutar e ao ordenar reflexivamente, no tempo e no espaço, os eventos constituintes do processo da saúde e da doença. Em nenhum deles se manifestou a ilusão absoluta de que a utilização dos recursos tecnológicos por si só revelaria o estado de saúde e/ou de doença de seus pacientes. Constatamos que, embora os entrevistados façam uso da narratologia como base epistemológica do saber médico, não estão conscientes disso.
