RESUMOObjetivo: analisar medidas da dinâmica respiratória em participantes de grupos de terceira idade. Método: participaram 41 pessoas, 32 do sexo feminino e nove do masculino, idade média de 69,90 anos. A avaliação foi realizada por meio das medidas de tempos máximos de fonação (TMF) das vogais /a/, /i/, /u/, das consoantes /s/ e /z/ e contagem de números; relação s/z; medida de capacidade vital (CV) e cocientes fônicos simples (CFS) e composto (CFC). Resultados: os valores médios dos indivíduos do sexo masculino para os TMF das vogais, consoantes e contagem de números foram respectivamente, 18,11s; 19,22s; 19,11s; 15,22s e 15,61s e 18,77s, sendo 0,98 o valor da relação s/z. No sexo feminino, as médias das mesmas medidas de TMF foram respectivamente 13,94s; 14,19s; 13,98s; 10,11s e 11,63s; 13,90s, sendo 0,92 a relação s/z. A média de TMF das vogais foi significativamente maior que a média das consoantes no grupo feminino. O valor médio da CV sem oclusão nasal no sexo feminino foi de 2.247ml e no masculino de 3.211 ml. Foi verificada relação significante entre a medida de CV e a variável idade, no grupo feminino. Os valores médios do CFS nos grupos de homens e mulheres foram respectivamente 182,11 ml/s e 176,58 ml/s e do CFC, 184,35 ml/s e 186,93 ml/s. Conclusão: as medidas da dinâmica respiratória dos participantes estiveram próximas aos valores de referência para adultos na literatura, bem como próximos aos de estudos de idosos, havendo diferenças para mais ou para menos nas diferentes medidas estudadas. DESCRITORES: INTRODUÇÃOAs medidas de dinâmica respiratória compõem diversos protocolos de avaliação vocal de indivíduos em várias faixas etárias, por apresentarem dados do aproveitamento do ar pulmonar na fonação, bem como, do funcionamento glótico 1,2 Dentre as medidas utilizadas destacamos a capacidade vital e os tempos máximos de fonação de vogais e consoantes, sendo que a partir deles podem ser calculados a relação s/z e o cociente fônico simples e composto. Diversos estudos mostraram a validade dessas medidas, a melhor forma de utilizá-las 3,4 , apontaram a necessidade de uso para controle de tratamentos 5 e tentaram encontrar dados normativos em diversas faixas etárias 6,7 . Estudos da função respiratória no idoso apontam para mudanças importantes apresentando um declínio pequeno nos anos iniciais da terceira idade e acelerado nas idades avançadas 8 , diferentemente
OBJETIVO: Analisar a concordância entre distintas escalas para grau de comprometimento em disfagia orofaríngea neurogênica. MÉTODOS: Foi realizado estudo clínico transversal. Participaram 200 indivíduos com disfagia orofaríngea neurogênica, 108 do gênero masculino e 92 do gênero feminino, com idades de 3 meses a 91 anos. Foram aplicadas quatro escalas para classificar o grau de comprometimento da disfagia orofaríngea, sendo duas escalas clínicas e duas videofluoroscópicas. Análises estatísticas foram realizadas para verificar a concordância entre as escalas clínicas e objetivas. RESULTADOS: Os resultados mostraram concordância muito boa entre as escalas clínicas estudadas (Kappa=0,92) e concordância moderada entre as escalas objetivas (Kappa=0,52). CONCLUSÃO: Embora a concordância entre as escalas clínicas tenha sido muito boa e entre as escalas objetivas tenha sido moderada, ainda é necessária ampla discussão e possível revisão dos parâmetros que definem o grau de comprometimento da disfagia orofaríngea em pacientes neurológicos.
Introdução: Penetração e/ou aspiração laringotraqueal (PA) são achados clínicos que constituem importante parâmetro na mensuração do risco para a segurança da deglutição devido às possíveis complicações pulmonares. Objetivo: Identificar a prevalência de penetração e/ou aspiração na Doença de Parkinson (DP) em Centro Especializado de Reabilitação (CER). Método: Estudo clínico observacional retrospectivo. Foram analisados 39 laudos de exames de videoendoscopia da deglutição realizadas em Centro Especializado de Reabilitação “Universidade Estadual Paulista-UNESP/Marília”, no período de 2006 a 2016. Dos 39 (100%) indivíduos, 19 (48,71%) eram do gênero feminino e 20 (51,28%) do gênero masculino, com idade que variou de 43 a 85 anos (média de idade de 71 anos) e somente 29 (74,35%) desses indivíduos apresentavam classificação de Hoehn & Yahr (1967) variando de um a cinco. Realizada análise da PA nas consistências pastosa, líquida espessada e líquida e com volume que variou de 5 a 10 ml segundo a definição proposta por Rosenbek et al, 1996. Para análise estatística foi utilizado o Teste de Fisher. Resultados: Constatou-se que 28 (71,79%) indivíduos não apresentaram penetração e/ou aspiração laringotraqueal e 11 (28,21%) apresentaram. Desses 11 (28,21%), quatro (10,25%) apresentaram penetração/aspiração na consistência pastosa (p=0,05), nove (23,07%) no líquido espessado (p=0,07) e nove (23,07%) no líquido ralo (p=0,02). Conclusão: Na população estudada nesse setting a ausência de PA foi mais frequente e a presença significante apenas no líquido ralo.
Comparing tongue trills in isolation and associated with TENS, there were changes in the comfort and muscle tension perception, as well as in vocal quality. On the other hand, tongue trills associated with TENS performed in 3 or 5 minutes resulted in beneficial effects on the voice identified in the assessments.
Huntington's disease (HD) is a degenerative genetic disorder with autosomal-dominant transmission. The triad of symptoms of this disease consists of psychiatric disorders, jerky movements, and dementia. Oropharyngeal dysphagia, which is more evident with disease progression, is also present. Few studies have addressed the swallowing characteristics using objective analysis in this population. The purpose of this research was to describe the swallowing endoscopic findings of the pharyngeal phase in HD. This is a cross-sectional study addressing a clinical case which included two individuals of the same family, male, 32 and 63 years old, designated as individual A and individual B, with progression of the disease for five and 13 years, respectively. Consistent liquid, nectar, and puree were offered during the evaluation. There was presence of posterior oral spillage in liquid and nectar, small amount of pharyngeal residues, and no laryngeal penetration or aspiration in the individuals with HD in this study.
