Wild polioviruses still remain endemic in three countries (Afghanistan, Pakistan, and Nigeria) and re-emergency of wild polio has been reported in previously polio-free countries. Environmental surveillance has been used as a supplementary tool in monitoring the circulation of wild poliovirus (PVs) and/or vaccine-derived PVs even in the absence of acute flaccid paralysis cases. This study aimed to monitor the presence of polioviruses in wastewater samples collected at one wastewater treatment plant located in the municipality of Rio de Janeiro, Brazil. From December 2011 to June 2012 and from September to December 2012, 31 samples were collected and processed. RD and L20B cell cultures were able to isolate PVs and non-polio enteroviruses in 27/31 samples. Polioviruses were isolated in eight samples (type 1 Sabin = 1, type 2 Sabin = 5, and type 3 Sabin = 2). Vaccine-derived polioviruses were not detected nor evidence of recombination with other PVs or non-polio enterovirus serotypes were observed among the isolates. The Sabin-related serotypes 2 and 3 presented nucleotide substitutions in positions associated with the neurovirulent phenotype at the 5′-UTR. Changes in important Amino acid residues at VP1 were also observed in the serotypes 2 and 3. Environmental surveillance has been used successfully in monitoring the circulation of PVs and non-polio enteroviruses and it is of crucial importance in the final stages of the WHO global polio eradication initiative. Our results show the continuous circulation of Sabin-like PVs and non-polio enteroviruses in the analyzed area during the study period.
Rickettsia rickettsii infection is being increasingly recognized as an important cause of fatal acute illness in Brazil, where this tick-borne disease is designated Brazilian spotted fever (BSF). In this study we report five fatal cases of BSF in employees of an animal shelter in an urban area in the municipality of Rio de Janeiro in southeast Brazil after a natural disaster on 11 January 2011. Four of the cases occurred from 27 January to 11 April 2011, while the fifth fatal case was identified in April 2012. Three cases were confirmed by molecular analysis and two by epidemiological linkage. An investigation of BSF was performed in the animal shelter, and blood samples were collected from 115 employees and 117 randomly selected dogs. The presence of high levels (1024-4096) of antibodies against spotted fever group rickettsiae was found in three (2·6%) employees and 114 (97·5%) dogs. These findings emphasize the need to consider BSF as a possible cause of undifferentiated febrile illness, especially dengue and leptospirosis, in patients occupationally exposed to dogs heavily infested by ticks, mainly working at kennels and animal shelters that have inadequate space for the animals housed and frequently providing an environment conducive to exposure to pathogens such as R. rickettsii.
Objetivos: investigar as internações por doenças respiratórias no Triângulo Mineiro (Minas Gerais, Brasil), discutindo-as no contexto da atenção em saúde, e a pandemia de COVID-19. Métodos: estudo epidemiológico, descritivo e quantitativo sobre as doenças do aparelho respiratório nas macrorregiões de saúde Triângulo do Norte e Triângulo do Sul, de janeiro de 2014 a dezembro de 2019. Foram utilizados dados provenientes do Sistema de Informação Hospitalar do Sistema Único de Saúde (SIH/SUS), disponíveis no endereço eletrônico do Departamento de Informática do SUS (DATASUS). Os dados foram apresentados por números absolutos, frequência relativa e medidas de tendência central. Resultados: nesse período, houve 76.745 internações por doenças do aparelho respiratório no Sistema Único de Saúde (SUS), com aspecto crescente desde 2018. As cidades de Uberlândia e Uberaba somam juntas quase metade desse total. Com os serviços hospitalares e profissionais foram gastos, aproximadamente, 90 milhões de reais. As faixas etárias cuja internação foi mais frequente foram a de 1 a 4 anos e a de 80 anos ou mais. As maiores frequências de óbitos concentraram-se no segundo e terceiro trimestre do ano e entre os indivíduos acima de 60 anos. Conclusões: O contexto caótico e oneroso ao SUS, decorrente das internações por doenças respiratórias, agravar-se-á ao se somar ao cenário de pandemia por COVID-19. Por isso, é imperiosa a prevenção dos fatores de risco e a promoção de saúde por meio da melhora na atenção primária em saúde, bem como a ampliação e a reorganização da rede hospitalar.
Post-transcriptional regulatory elements associated with transcript degradation or transcript instability have been described at the 3’ untranslated region (3’UTR) of the HLA-G gene. Considering that HPV infection and aneuploidy, which causes gene instability, are associated with cervical cell malignancy, as well as the fact that HIV infection and HLA-G may modulate the immune response, the present study aimed to compare the frequencies of HLA-G 3′UTR polymorphic sites (14-base pair insertion/deletion, +3142C/G, and +3187A/G) between 226 HIV+ women co-infected (n = 82) or not with HPV (n = 144) and 138 healthy women. We also evaluated the relationship between those HLA-G 3’UTR variants and aneuploidy in cervical cells. HPV types and HLA-G polymorphisms were determined by PCR and sequencing of cervical samples DNA. Aneuploidy in cervical cell was measured by flow cytometry. The HLA-G 3’UTR 14-bp ins/del was not associated with either HIV nor HIV/HPV co-infection. The +3142G allele (p = 0.049) and +3142GG genotype (p = 0.047) were overrepresented in all HIV-infected women. On the other hand, the +3187G allele (p = 0.028) and the +3187GG genotype (p = 0.026) predominated among healthy women. The +3142G (p = 0.023) and +3187A (p = 0.003) alleles were associated with predisposition to HIV infection, irrespective of the presence or not of HIV/HPV co-infection. The diplotype formed by the combination of the +3142CX (CC or CG) and +3187AA genotype conferred the highest risk for aneuploidy in cervical cell induced by HPV. The HLA-G 3’UTR +3142 and +3187 variants conferred distinct susceptibility to HIV infection and aneuploidy.
Objetivo: Analisar os aspectos epidemiológicos da COVID-19 nos profissionais de enfermagem brasileiros. Metodologia: Estudo transversal e quantitativo, com base em dados secundários de domínio público, do Observatório de Enfermagem do Conselho Federal de Enfermagem. Os dados foram coletados em novembro de 2020 e importados para a versão STATA 12.0. Foi realizada uma análise estatística descritiva, com números absolutos e medidas de freqüência. Resultados e discussão: 38.628 profissionais de enfermagem com suspeita de COVID-19 foram relatados, 52,4% com diagnóstico confirmado e predominantemente técnicos (62,9%). A faixa etária predominante entre os mortos era mais alta (41 a 60 anos) do que entre os infectados (31 a 40 anos), enquanto o sexo feminino era quantitativamente dominante em ambos, apesar da maior taxa de casos fatais entre os homens (4,5%). A mortalidade / 1.000 profissionais era alta no Amapá, Acre, Mato Grosso e Rondônia. O pico no número diário de casos novos (525) ocorreu em julho de 2020, enquanto que o número de mortes (18) ocorreu em setembro, mês em que houve uma tendência de queda na variação da taxa de crescimento da média móvel entre os casos novos, o que não é evidente na variável da média móvel entre as mortes. Este cenário tem uma forte relação com as condições precárias de trabalho, falta de EPI, sobrecarga física e emocional e os resultados da rápida contratação e qualificação para a gestão de pacientes com COVID-19. Conclusão: A compreensão da situação de vulnerabilidade experimentada por esses trabalhadores no contexto da pandemia revela a necessidade de concentrar ações de saúde eficazes voltadas para esse grupo.
The COVID-19 pandemic has the potential to affect all individuals, however in a heterogeneous way. In this sense, identifying specificities of each location is essential to minimize the damage caused by the disease. Therefore, the aim of this research was to assess the vulnerability of the 853 municipalities in the second most populous state in Brazil, Minas Gerais (MG), in order to direct public policies. Then, an epidemiological study was carried out based on Multi-Criteria Decision Analysis (MCDA) using indicators with some relation to the process of illness and death caused by COVID-19. The indicators were selected by a literature review and categorized into: demographic, social, economic, health infrastructure, population at risk and epidemiological. The variables were collected in Brazilian government databases at the municipal level and evaluated according to MCDA, through the Program to Support Decision Making based on indicators (PRADIN). Based on this approach, the study performed simulations by category of indicators and a general simulation that allowed to divide the municipalities into groups of 1 to 5, with 1 being the least vulnerable and 5 being the most vulnerable. The groupings of municipalities were exposed in their respective mesoregions of MG in a thematic map, using the software Tabwin 32. The results revealed that the mesoregion of Norte de Minas stands out with more than 40% of its municipalities belonging to group 5, according to economic, social and health infrastructure indicators. Similarly, the Jequitinhonha mesoregion exhibited almost 60% of the municipalities in this group for economic and health infrastructure indicators. For demographic and epidemiological criteria, the Metropolitana de Belo Horizonte mesoregion is the most vulnerable, with 42.9% and 26.7% of the municipalities in group 5, respectively. Considering the presence of a population at risk, Zona da Mata reported 42.3% of the municipalities in the most vulnerable group. In the joint analysis of data, the Jequitinhonha, Vale do Mucuri and Vale do Rio Doce mesoregions were the most vulnerable in the state of MG. Thus, through the outlined profile, the present study proved how socioeconomic diversity affects the vulnerability of the municipalities to face COVID-19 outbreak, highlighting the need for interventions directed to each reality.
Introdução: Em dezembro de 2019, foi identificado em Wuhan, China, em pessoas que frequentavam um mercado úmido, a presença de um novo coronavírus, denominado SARS-CoV-2, responsável pela COVID-19, que rapidamente se espalhou pelo mundo de forma pandêmica. A história natural da doença se adapta de forma emblemática na abordagem One Health (Saúde Única), que são estratégias de controle de doenças que buscam de forma mais ampla discutir os aspectos indissociáveis da saúde humana, animal e ambiental. Objetivo: Realizar uma revisão sistemática de literatura sobre a aplicação dos conceitos de Saúde Única no atual contexto pandêmico da COVID-19. Método: O estudo seguiu a recomendação PRISMA para elaboração de revisões sistemáticas. As bases de dados pesquisadas foram PubMed, Scopus, Web of Science, Lilacs e Google Scholar. Os termos de pesquisa utilizados foram (COVID-19 OR SARS-CoV-2 OR 2019-nCoV) AND "One Health". Resultados: Na síntese qualitativa foram incluídas 62 publicações, 28 (45,2%) apresentavam propostas para a vigilância em saúde de COVID-19 por meio de abordagem One Health e 34 (54,8%) apenas relataram a importância dessa abordagem no combate ao vírus. Foi observado um incremento das publicações sobre os dois temas ao longo dos meses analisados. Conclusões: Questões biológicas (investigações relacionadas ao vírus, aos hospedeiros e à biotecnologia) foram os temas mais discutidos como estratégias de One Health, em detrimento das questões sociais, econômicas e comportamentais. Políticas governamentais devem ser adotadas para garantir as ações multiprofissionais de saúde. Cabe também aos profissionais a promoção e a aplicação deste conceito, de forma articulada e interdisciplinar. A aplicabilidade da One Health no contexto pandêmico vivenciado pela COVID-19 é imperiosa.
