RESUMO Objetivo Levantar pontos críticos do processo de medicação, suas repercussões nas demandas de trabalho da equipe de enfermagem e riscos para a segurança dos pacientes. Método Estudo descritivo, com abordagem qualitativa, na perspectiva ecológico-restaurativa. Os dados foram coletados por meio de grupos focais e fotografias. Participaram enfermeiros e técnicos de enfermagem. Resultados Três categorias emergiram da análise temática: Desafios nos processos de prescrição e dispensação de medicamentos; Administração de medicamentos – organização no turno de trabalho; Uso de novas tecnologias para diminuir erros de medicamentos. Os resultados apontam que o processo de medicação assume um caráter de centralidade na organização do trabalho da equipe de enfermagem, sendo que estes profissionais configuram-se como a última barreira para detectar falhas de prescrição e dispensação de medicamentos. Conclusão Para a identificação de vulnerabilidades na etapa de administração de medicamentos, o uso de tecnologias, sem dúvida, agrega valor para o processo de cuidado seguro.
Objective: to analyze the experience of the patient during hospitalization, focusing on the co-production of care related to patient safety protocols. Method: qualitative study, whose data were collected through the triangulation of multiple sources: document analysis, observation of 10 professionals in the provision of care and 24 interviews with patient-families from 12 clinical and surgical inpatient units of a hospital. Thematic analysis was carried out, based on the concept of co-production. Results: safety protocols according to the experience of the patient portrayed the role of patient-families as co-producers of safe care. It was found an alignment between perceptions of the patients, institutional definitions and basic national and international patient safety protocols. However, these protocols are not always followed by professionals. Conclusion: co-production was perceived in the protocols for safe surgery and prevention of injuries resulting from falls. In patient identification, hand hygiene and medication process, it was found that co-production depends on the proactive behavior of patient-families, as it is not encouraged by professionals. The research contributes with subsidies to leverage the participation of the patient as an agent of their safety, highlighting the co-production of health care as a valuable resource for advancing patient safety.
Objective: to analyze the characteristics of the work organization performed by nursing staff regarding medication administration procedures and their implications on the workload of these professionals and on patient safety. Method: the study design is exploratory, with mixed method research and an ecological restorative approach. Data were collected between January 2014 and March 2015, in three inpatient units of a teaching hospital in the south of Brazil, by means of photo walkabout and focus groups, in the qualitative step. In the quantitative phase data were collected from the 162 lists of patients assigned to nursing technicians during their work shifts. Results: the administration of medications has an impact on the professionals' workload and patient safety. Final considerations: there are weaknesses in the process that may contribute to medication administration errors, which are related to the number of doses and the number of patients assigned to each professional.
The adapted version of the PPP in Brazilian Portuguese demonstrated good internal consistency (similar to the original instrument) and stability over time, decisive for the next step of clinical validation for CSCP.
Objectives: To analyze the current way medications are prepared and administered in the hospital setting and to apply a method to establish priorities for problems detected. Method: This is an exploratory-descriptive case study. The data were collected through observation and focus groups with 13 participants who were part of a health team in a surgical inpatient unit of a public university hospital. The analysis was based on a lean production framework. Results: A value stream map was constructed of the current way drugs are prepared and administered, identifying the clients in the process and their requirements. Forty-five current problems were identified, based on requirements that were not met with eight being prioritized to improve planning. Conclusion: Having prioritized the problems, the planning and implementation of continuous improvements in the medication process were started in order to reduce errors and improve the quality of services.
RESUMOObjetivo: analisar os registros da equipe multiprofissional sobre condutas de educação do familiar para promoção da segurança da criança hospitalizada. Método: estudo transversal, descritivo e retrospectivo com uso de dados secundários. A amostra foi constituída por 126 prontuários de pacientes pediátricos internados nas unidades clínico-cirúrgicas de um hospital no Sul do Brasil, no período entre setembro/2013 e agosto/2014. Resultados: a média de idade foi de 44,58 meses, sendo a maioria do sexo masculino (55,4%), residente em Porto Alegre-RS, Brasil (43,7%) e internado por disfunção do sistema ventilatório (36,5%). Todos os registros foram realizados por enfermeiros. A principal orientação relacionou-se à prevenção de quedas no momento da admissão (72,2%) e durante a primeira semana de internação (21,4%). Conclusão: as informações relacionadas às condutas de educação do familiar para promover a segurança do paciente pediátrico ainda são pouco registradas em prontuário, sendo necessário envolver a equipe multiprofissional nesse processo. PURPOSE: Acute pain occurs in over 50% of hospitalized children. The accuracy of this diagnosis has been underexplored in the literature, as has the role of training to implement pain assessment. This study analyzed the accuracy of acute pain diagnoses after the implementation of a systematic evaluation of pain (study intervention). METHOD: The sample was divided into: pre‐ and postintervention. The Nursing Diagnosis Accuracy Scale, which scores accuracy as null, low, moderate, or high, was used. RESULTS: In the postimplementation, acute pain was diagnosed more often. However, accuracy only improved in the moderate category. CONCLUSION: Diagnosis of acute pain increased in the postimplementation period, but accuracy did not. IMPLICATIONS: The development of strategies for improvement of diagnostic accuracy is warranted.
Objetivo: desenvolver e validar o conteúdo de instrumentos para padronizar a passagem de plantão da enfermagem.Método: estudo de desenvolvimento e validação por consenso de especialistas, conduzido de janeiro a julho de 2019 em hospital do sul do Brasil. Participaram do estudo 24 profissionais de diferentes áreas, incluindo enfermeiros, técnicos e professores de enfermagem, com experiência na passagem de plantão hospitalar. Avaliou-se abrangência, clareza e pertinência dos itens dos instrumentos, sendo validados pelos especialistas sobre cada item dos instrumentos desenvolvidos,obtendo-se índice de concordância de 93%.Resultados: foram elaborados o “Formulário de Passagem de Plantão”, contemplando identificação dos pacientes, informações clínicas e intercorrências de cada turno, e o “Procedimento Operacional Padrão para Passagem de Plantão”, descrevendo atividades a serem realizadas para garantir a transmissão de informações precisas.Considerações finais: os instrumentos elaborados podem nortear a passagem de plantão da enfermagem na prática clínica, promovendo a continuidade e segurança do cuidado.
