OBJECTIVE: to evaluate the use of identification wristbands among patients hospitalized in inpatient units. METHOD: quantitative, descriptive and transversal research, with a sample of 385 patients. Data collection occurred through the observational method through the filling out of a structured questionnaire which aimed to check the presence of the identification wristband and the identifiers used. Descriptive statistics with absolute and relative frequencies was used for analysis. RESULTS: it was obtained that 83.9% of the patients were found to have the correctly identified wristband, 11.9% had a wristband with errors, and 4.2% of the patients were without a wristband. The main nonconformities found on the identification wristbands were incomplete name, different registration numbers, illegibility of the data and problems with the physical integrity of the wristbands. CONCLUSION: the study demonstrated the professionals' engagement in the process of patient identification, evidencing a high rate of conformity of the wristbands. Furthermore, it contributed to identify elements in the use of wristbands which may be improved for a safe identification process.
Objective: to analyze the experience of the patient during hospitalization, focusing on the co-production of care related to patient safety protocols. Method: qualitative study, whose data were collected through the triangulation of multiple sources: document analysis, observation of 10 professionals in the provision of care and 24 interviews with patient-families from 12 clinical and surgical inpatient units of a hospital. Thematic analysis was carried out, based on the concept of co-production. Results: safety protocols according to the experience of the patient portrayed the role of patient-families as co-producers of safe care. It was found an alignment between perceptions of the patients, institutional definitions and basic national and international patient safety protocols. However, these protocols are not always followed by professionals. Conclusion: co-production was perceived in the protocols for safe surgery and prevention of injuries resulting from falls. In patient identification, hand hygiene and medication process, it was found that co-production depends on the proactive behavior of patient-families, as it is not encouraged by professionals. The research contributes with subsidies to leverage the participation of the patient as an agent of their safety, highlighting the co-production of health care as a valuable resource for advancing patient safety.

Method: Experience report on the implementation of the nucleus and strategies for patient safety in a hospital in the south of Brazil, from 2009 to 2017. Results: The concern with patient safety was made official in 2009 with the creation of a specific service for risk management and in 2015 it was named the patient safety nucleus. Eight strategies were implemented in order to disseminate the patient safety policy. Conclusion: An improvement was observed in the processes related to patient safety in the institution. Top management support and leadership engagement were key to this journey. RESUMOObjetivo: Descrever a experiência da implantação do núcleo de segurança do paciente e as estratégias desenvolvidas para garantir uma assistência mais segura. Método: Relato de experiência da implantação do núcleo e das estratégias para segurança do paciente em um hospital no sul do Brasil, de 2009 a 2017. Resultados: A preocupação com a segurança do paciente foi oficializada em 2009 com a criação um serviço específico para gerenciamento dos riscos assistenciais e em 2015 foi nomeado o núcleo de segurança do paciente. Oito estratégias foram implantadas visando disseminar a política de segurança do paciente. Conclusão: Foi observado um avanço na melhoria dos processos relacionados a segurança do paciente na instituição. Objective: Describe the characteristics of falls in patients with damage, their risk factors and injuries. Method: Retrospective and longitudinal study of 260 records of falls with damage of adult patients in clinical and surgical units of a general hospital, from September 2012 to June 2017. Data were collected in May 2018 from the electronic fall and electronic record research instrument, and the statistical analysis was described.Results: There were 260 falls with damage, mostly in the elderly (78%), female (55%), in clinical treatment (68%) and unaccompanied (59.4%). The falls occurred at the same height (63.4%) and in the patient's room (67.3%). The severity of the damage was mild in 80.8% of cases, severe in 11.9% and moderate in 7.3%.Conclusion: Improving understanding of falls and their consequences can assist professionals in identifying and assessing risks and in establishing preventive measures.Objetivo: Descrever as características das quedas com dano de pacientes, seus fatores de risco e lesões decorrentes. Métodos: Estudo longitudinal e retrospectivo de 260 registros de quedas com dano de pacientes adultos em unidades clínicas e cirúrgicas de um hospital geral, de setembro de 2012 a junho de 2017. Os dados foram coletados em maio de 2018 a partir do instrumento de investigação de quedas e prontuário eletrônico, sendo a análise estatística descritiva. Resultados:A maioria dos pacientes que sofreram queda com dano eram idosos (78%), do sexo feminino (55%), em tratamento clínico (68%) e desacompanhados (59,4%). As quedas ocorreram da própria altura (63,4%) e no quarto do paciente (67,3%). A gravidade dos danos foi leve em 80,8% dos casos, grave em 11,9% e moderado, 7,3%. Conclusões:Melhorar a compreensão sobre as quedas e suas consequências pode subsidiar os profissionais na avaliação dos riscos e no estabelecimento de medidas preventivas. Nursing audit is a management tool capable of assisting in the improvement of care quality and efficiency of hospital charges and may be performed concurrently, in other words, while the patient receives care. This study aims to describe nurses' experience about the implementation of concurrent nursing audit in a hospital environment. This is a report on an experience in the period from 2009 to 2014 at a hospital in the state of Rio Grande do Sul, Brazil. The following steps of this process are discussed: Instrument elaboration, Sensitization of the nursing team and Operation of concurrent audit. The results show the feasibility and benefits of adopting this model, especially regarding the reduction of time to send the bill to the health insurances, greater interaction between auditors and care teams, creation of indicators, and contributions to the improvement of healthcare quality and nursing records.
RESUMO Objetivo: analisar os fatores que interferem na segurança no processo de medicação em uma unidade de terapia intensiva (UTI) neonatal. Método: estudo exploratório com abordagem qualitativa. A coleta ocorreu no período de dezembro de 2014 a março de 2015, através de grupos focais e caminhada fotográfica, na perspectiva do pensamento ecológico restaurativo, com 12 profissionais de enfermagem da UTI neonatal de um hospital público da Região Sul do Brasil. As informações foram organizadas através do software Nvivo 10 e submetidas à análise de conteúdo temática. Resultados: a partir da análise, emergiram as seguintes categorias temáticas: Abordagem individualizada e cultura de punição dos erros de medicação; Fatores de (in)segurança relacionados à estrutura física dos medicamentos na UTI neonatal; Fatores de (in)segurança relacionados a rotinas e protocolos; e A enfermagem como barreira para a ocorrência de falhas de prescrição médica. Conclusão: o estudo demonstra a complexidade do processo de medicação em neonatologia e destaca pontos críticos no mesmo que podem ocasionar falhas e eventos adversos, assim como propostas de melhoria para prevenir os erros. Destaca-se o papel da equipe de enfermagem na detecção de erros da prescrição medicamentosa, funcionando como última barreira para prevenção e redução de erros associados à medicação.
Objective:to analyze incidents reported in a neonatal care unit. Method:a quantitative, cross-sectional and retrospective study with a sample of 34 newborns. Data were collected through a structured form, composed of two parts: sociodemographic/clinical characteristics of the newborns, and characteristics of the reported incidents. Data were collected from the institution’s computer system, in a period corresponding to 13 months, and analyzed by means of descriptive statistics. Results:the majority of the newborns were preterm (70.6%), male (52.9%) and born through caesarean section (76.5%). During the study period, 54 incidents were reported, totaling a frequency of 1.6 incident per newborn. It was found that 61.1% of incidents were related to medicines, 14.8% to accidental loss of tracheal tube and 9.3% to catheter obstruction. Conclusion:analysis of the reported incidents has shown that most incidents refer to the drug process. Information about the incidents can increase the perception of health professionals regarding the impact of their actions.
O presente trabalho investigou os conhecimentos e atitudes dos profissionais da equipe de enfermagem do Hospital de Clínicas de Porto Alegre (HCPA), Rio Grande do Sul, em relação ao descarte dos resíduos sólidos dos serviços de saúde (RSSS). Realizou-se uma pesquisa de natureza exploratória descritiva, através de uma abordagem qualitativa. Os dados foram obtidos a partir de entrevistas semi-estruturadas aplicadas a 24 profissionais da equipe de enfermagem. Os dados foram categorizados através da análise de conteúdo, tendo sido identificadas quatro categorias: Significado da expressão resíduos sólidos de serviços de saúde; Separação dos resíduos sólidos dos serviços de saúde; Realização do curso/conhecimento e Profissionais que não realizam a separação. Os resultados ratificaram a importância de tratar com maior seriedade a questão apresentada, reforçando a necessidade do acesso às orientações adequadas.
