ObjectivesVeterans Affairs (VA) patients are at risk for rehospitalisation due to their lower socioeconomic status, older age, poor social support or multiple comorbidities. The study explored inpatients’ perceptions about factors contributing to their rehospitalisation and their recommendations to reduce this risk.DesignThematic qualitative data analysis of interviews with 18 VA inpatients.SettingVA Connecticut Healthcare System, West Haven Hospital medical inpatient units.ParticipantsAll were aged 18+ years, rehospitalised within 30 days of most recent discharge, medically stable and competent to provide consent.MeasurementsInterviews assessed inpatients’ health status after last discharge, reason for rehospitalisation, access to and support from primary care providers (PCP), medication management, home support systems and history of substance use or mental health disorders.ResultsThe mean age was 71.6 years (11.1 SD); all were Caucasian, living on limited budgets, and many had serious medical conditions or histories of mental health disorders. Participants considered structural barriers to accessing PCP and limited PCP involvement in medical decision-making as contributing to their rehospitalisation, although most believed that rehospitalisation had been inevitable. Peridischarge themes included beliefs about premature discharge, inadequate understanding of postdischarge plans and insufficiently coordinated postdischarge services. Most highly valued their VA healthcare but recommended increasing PCPs’ involvement and reducing structural barriers to accessing primary and specialty care.ConclusionsIncreased PCP involvement in medical decision-making about rehospitalisation, expanded clinic hours, reduced travel distances, improved communications to patients and their families about predischarge and postdischarge plans and proactive postdischarge outreach to high-risk patients may reduce rehospitalisation risk.

É sabido que ele foi um pensador atento ao contexto de ideias que afetaram a cultura acadêmica estadunidense. Tal atenção dialoga com a comple-xidade de diversas influências sobre sua obra. Este artigo objetiva analisar algumas ideias psicológicas e filosóficas que influen-ciaram Rogers. Entende-se que exerceram influências diretas no pensamento rogeriano: o Funcionalismo de John Dewey, Leta Hollingworth e Kurt Goldstein; e o Pragmatismo de John Dewey e William Kilpatrick. Segundo uma perspectiva metodológica histórica-crítica, elucida-se como cada perspectiva de influência se desenvolveu nos EUA, nas universidades de Chicago e de Columbia. Em seguida, identificam-se os aportes contatados e elaborados por Rogers de cada expoente mencionado. Constata-se que essas apropriações, embora pontuais, foram importantes para a circulação de Rogers no contexto acadêmico estadunidense. Contudo, incorre equívoco afigurar Rogers, exclusivamente, como um psicólogo funcionalista ou pragmatista, dado que ele não deu continuidade a essas perspectivas. O estudo, finalmente, destaca a necessidade de aprofundar investigações sobre outras bases históricas de influência na teoria rogeriana. Abstract: It is possibly to identify in the theory of Carl Rogers many influences that deserve to be revisited. It is known that he was a careful thinker of the context about some ideas that affected the American academic culture. Such attention dialogs with the complexity of many influences on his work. This article aims to analyze some psychological and philosophical ideas that influenced Rogers. It is understood that the followings theories influenced directly Rogers' thought: the Functionalism of John Dewey, Leta Hollingworth and Kurt Goldstein, and the Pragmatism of John Dewey and William Kilpatrick. Through a critical-historical methodological perspective, it is elucidated how each perspective of influence was developed in the USA, in the universities of Chicago and Columbia. After that, we identified the theories contacted and elaborated by Rogers about each mentioned exponent. It was found that those appropriations, although punctual, were important for Rogers' circulation in the American academic context. However, it is a misunderstanding considering Rogers exclusively as a functionalist psychologist, or a pragmatist one, because he did not give continuity to those prospects. The study, finally, highlights the need for further investigations on other historical bases of influence in Rogers' theory.
Este trabalho discute a hiperatividade considerando a metáfora do corpo da criança e o seu funcionamento psicológico, de forma a esclarecer se a sua expressão constitui doença ou um modo próprio de ser. O transtorno de déficit de atenção/hiperatividade (TDAH) tem sido extensivamente investigado em pesquisas que visam aprimorar os critérios diagnósticos e conhecer sua etiologia. Poucos estudos são orientados para elucidar a dimensão psíquica da criança hiperativa. A Gestalt-Terapia é uma abordagem fenomenológico-existencial assentada em teorias holísticas que reconhecem a multidimensionalidade do humano, enfocando a relação (o contato) como ontológica à existência humana.
Abstract. Approximately one million persons infected with human immunodeficiency virus (HIV) live in Ethiopia. Socio-cultural factors influence prevention and treatment adherence. We applied a qualitative descriptive approach to evaluate community perception, knowledge, and the role of spiritual factors in regard to HIV. We conducted 14 focus groups with sampling of HIV+ and HIV-participants (n = 52) by using open-ended questions. We coded and analyzed data for major themes. There are misconceptions, including transmission via casual contact, and pervasive beliefs of holy water as a cure. Many HIV-participants believe treatment is ineffective or incompatible with holy water. Most HIV+ participants believe treatment and holy water can be taken together, but experienced either pressure to stop treatment or stigma when taking medications. Participants emphasized the role of spiritual leaders in directing and shaping community perspectives on HIV. Ongoing community education via local initiatives, nation-wide structural and environmental strategies, and efforts tailored toward Ethiopian society to reconcile treatment with faith are crucial.
A hospitalist-led transition-of-care intervention designed to improve communication and information exchange between PCPs and hospitalists at the time of admission demonstrated that encrypted e-mail could be used as a tool to obtain useful additional medical and psychosocial information and to better understand PCP attitudes and preferences. The increased level of communication did not yield statistically significant decreases in LOS, 30-day readmission rates, or 30-day post-discharge ED visits, however.
RESUMO A Gestalt-Terapia é uma abordagem fenomenológico-existencial com uma visão holística de homem e mundo que dá primazia à relação e valoriza a influência mútua entre criança e ambiente. Os fenômenos psicológicos emergem das trocas emocionais vividas na unidade criança-outro-mundo, no "entre" das relações humanas. Portanto, a doença emocional é fruto de um distúrbio em uma relação. Criança doente emocionalmente é família adoecida em suas relações afetivas (quando não há algum trauma vivido ou outras experiências negativas causadas por fatores externos). Os distúrbios psicológicos da criança são, geralmente, oriundos dos dramas infantis não resolvidos dos pais que são projetados na criança. Na perspectiva gestáltica, doença significa perturbações da autoregulação originadas por mecanismos psicológicos defensivos de contato, que visam inibir a consciência de sentimentos, pensamentos, necessidades, comportamentos que geram angústia e colocam em risco a relação com o outro significativo. A partir desses mecanismos psicológicos de bloqueio do contato da teoria gestáltica que regem a dinâmica interna do indivíduo e das relações humanas, iremos abordar as psicopatologias da depressão, da fobia e do transtorno obsessivo-compulsivo.
O presente estudo visa estabelecer interface entre a abordagem gestáltica e a Psicopedagogia. Focalizam-se as teorias de base, a concepção de campo holístico-relacional a teoria do Ciclo do Contato, bem como as contribuições da Gestaltpedagogia para a compreensão do processo de aprendizagem e prática escolar.Da Psicopedagogia destaca-se sua conceituação, abrangência, processo de avaliação e intervenção juntoà criança e instituição educativa; a convergência de suas bases teóricas e práticas abrangem os fenômenosaprendizagem, avaliação e intervenção na ótica gestáltica. Tal interface visa nova compreensão e açãointerventiva do processo de aprendizagem, de modo a proporcionar o desenvolvimento da criança.
