The study was carried out to observe some aspects of hematological changes like Hb concentration, PCV and TC of RBC in different stages of Chronic Renal Failure(CRF) patients suffering from anemia. For this purpose, 65 male CRF patients with anemia and 25 apparently health male (control) subjects of age ranged from 30-50 years were selected randomly from BSMMU out patient department. Hematological parameters and renal creatinine clearance were estimated by usual laboratory technique. Data were analyzed statistically by ANOVA and Pearson's correlation coefficient test. Among the CRF patients 15 were mild, 25 were moderate and 25 were severe CRF patients with anemia. In this study, the mean Hb concentration, PCV and total count of red blood cells were significantly lower in three stages of CRF patients with anemia compared to those of healthy subjects. Again, all of these hematological values were significantly (P< 0.001) lower in moderate and severe CRF patients compared to mild cases and also in severe cases than those of moderate CRF cases with anemia. All the Hematological parameters showed positive correlation with creatinine clearance in all three stages of CRF and it was statistically significant in moderate and severe group. From the present study it may be concluded that CRF patients with anemia had lower hematological indices and the degree of changes depend on severity of renal failure. Background: Anemia is the universal complication of Chronic Kidney Disease (CKD) and reflects problem either in abnormally low production of RBC by the bone marrow or excessive haemolysis or blood loss. This study has been designed to observe the status of anaemia in different stages of chronic kidney disease patients. Materials and methods: It is a hospital based cross sectional comparative study. 100 patients of diagnosed case of chronic kidney disease admitted in the Department of Nephrology, Chittagong Medical College Hospital were selected as cases and 100 apparently healthy persons, age and sex matched were selected as controls. According to stages of CKD, cases were subdivided into three groups stage III, IV and V. Haemoglobin estimation was done manually by traditional method (Sahli’s acidhaematin method) in the Department of Physiology, Chittagong Medical College, Chittagong. Peripheral Blood Film (PBF) study also was performed in the department of Physiology, Chittagong Medical College, Chittagong. Data were analyzed by different statistical methods. Results: In our study, among the case group (100 patiens), we found that 5.5% are on CKD stage III, 21% on stage IV, 23.5% on stage V. And 50% (100 apparently healthy subjects) were taken as control. Here the mean (±SD) hemoglobin concentrations were 8.75(± 1.45) and 12.07(±1.77) gm/ dl of blood in case and control group respectively. Peripheral blood film study among the study groups showed that RBCs were found microcytic hypocromic in 69% of case and 20% control group. On the other hand RBCs were normocytic normocromic in 31% of case and 80% of control group. Again among the case group it was found that 54.5%, 71.4% and 70.2% of RBCs were found microcytic hypocromic in CKD patients with stage III, IV and V respectively. On the other hand RBCs were normocytic normocromic in 05%, 12% and 14% of CKD patients with stage III, IV and V respectively. Conclusion: The results of this study revealed that all the patients (100%) of the study were found anaemic and 69% CKD patients had microcytic hypochromic anaemia. JCMCTA 2017 ; 28 (2) : 59-63
Background: Chronic renal failure (CRF) is a major public health hazard. Anemia is an accompaniment and independent risk factor for the development of cardiac dysfunction and responsible for 40% to 50% deaths in CRF patients. For this reason, effective erythropoiesis to correct anemia by erythropoietin therapy and maintenance of target iron level is very much important. Subjects and Methods: The present study has been designed to observe iron status in different stages of CRF patients suffering from anemia. For this purpose, 65 male CRF patients with anemia and 25 healthy male (control) subjects, in the age group of 30-50 years were selected randomly from BSMMU outpatient department. Iron level, hemoglobin and creatinine clearance were estimated by usual laboratory technique. Data were analyzed statistically by ANOVA and Pearson's correlation coefficient test. Among 65 CRF patients, 15 were mild, 25 were moderate and 25 were reported severe renal failure with anemia. Results: The mean serum iron, transferrin saturation and ferritin were significantly (P < 0.001) lower in moderate but severe in CRF patients with anemia compared to those of healthy subjects and these values were also significantly (P < 0.001) lower in severe CRF than those of mild and moderate groups of patients. On the other hand, total iron binding capacity was significantly (P < 0.001) higher only in severe group of CRF patients compared to healthy, mild and moderate group of CRF patients. In addition, 72% severe group of CRF patients had serum ferritin and 28% had transferrin saturation below the target level (≥100 ng/ml serum ferritin & ≥20% transferrin saturation). Decreasing iron status which is more marked in severe CRF patients are suggestive of iron deficiency and iron status below the target level in severe CRF patients indicate inadequate iron store. Also, a significantly (P < 0.001) decreasing hemoglobin concentration is observed in severe CRF case compared to those of mild and moderate CRF patients with anemia. Conclusion: From the present study it may be concluded that changes in iron status occur in CRF patients with anemia and these changes vary along with severity of CRF.
