SummaryHepatitis C virus (HCV) is a major cause of hepatic disease and of liver transplantation worldwide. Mannan-binding lectin (MBL), encoded by the MBL2 gene, can have an important role as an opsonin and complement activating molecule in HCV persistence and liver injury. We assessed the MBL2 polymorphism in 102 Euro-Brazilian patients with moderate and severe chronic hepatitis C, paired for gender and age with 102 HCV seronegative healthy individuals. Six common single nucleotide polymorphisms in the MBL2 gene, three in the promoter (H/L, X/Y and P/Q) and three in exon 1 (A, the wild-type, and B, C or D also known as O) were evaluated using real-time polymerase chain reaction with fluorescent hybridization probes. The concentration of MBL in plasma was measured by enzyme-linked immunosorbent assay. The frequency of the YA/YO genotype was significantly higher in the HCV patients compared with the controls (P = 0·022). On the other hand, the genotypes associated with low levels of MBL (XA/XA, XA/YO and YO/YO) were decreased significantly in the patients with severe fibrosis (stage F4), when compared with the patients with moderate fibrosis (stage F2) (P = 0·04) and to the control group (P = 0·011). Furthermore, MBL2 genotypes containing X or O mutations were found to be associated with non-responsiveness to pginterferon and ribavirin treatment (P = 0·023). MBL2 polymorphisms may therefore be associated not only with the development of chronic hepatitis C, but also with its clinical evolution and response to treatment.
Summary:The hepatitis C virus (HCV) infection may change the outcome of patients undergoing stem cell transplantation. This study aimed at determining the prevalence of the HCV antibody in patients who were alive 10 or more years after BMT, defining the annual progression rate of hepatic fibrosis in those patients, and identifying cases of cirrhosis among those who were positive for HCV antibody. Between 1979 and 1990, 259 patients had a bone marrow transplant, and 91 were alive in March 2000. Of those, 80 were included in the study after having been scanned for serum HCV antibodies. A total of 39 were positive (48.8%), one was indeterminate and 40 were negative (50%). The patients who were HCV positive or undetermined were called for a medical appointment and 22 (55%) attended. A total of 16 patients (72.7%) were male, the mean age was 37.879.2 years and all of them had had an allogeneic transplant. Of the 22 patients studied, 12 (54.5%) agreed to have a liver biopsy. Hepatic fibrosis was diagnosed in 10 patients. The hepatic fibrosis annual progression rate was 0.156 UF/year. Among the anti-HCV-positive patients assessed, three (13.6%) already had cirrhosis.
ABSTRATCPurpose: This study has analyzed the giochemical and morphological effects on the remmant kidney in rats which were submitted to progressive surgical ablation of renal mass. Methods: Sixty Wistar male rats, weighing between 210 and 380g, were used and they were distributed in 3 groups of 20 animals each. The rats from the groups called 1, 2 and 3 were submitted to the surgical removal of renal tissue equivalent to ½, 2/3 and 5/6 of the whole renal mass, respectively. Then the groups were subdivided into 2 subgroups and they were operated again within 24 hours (subgroups 1B, 2B and 3B) and within 8 weeks (subgroups 1C, 2C and 3C) for the removal of the remnant kidney. 24-hour urine and blood were collected to analyze serum creatinine, clearance of creatinine and proteinuria in the first surgical intervention and at the time of the re-operation. The remnant kidney was submitted to a macroscopic evaluation for the degree of hypertrophy and to the analysis of histology. Results: There was a significant increase of the volume of the remnant kidney (164%) and glomerular sclerosis was present in 40% of the animals submitted to the ablation of 5/6 of renal mass. Functional alterations characterized by the increase of urinary excretion of proteins (50% in group 3), rise in the serum creatinine (261% subgroup 2B; 371% subgroup 3B, 118% subgroup 3C) and a significant reduction of the clearance of creatinine (control x subgroup 3C = 2,88 x 1,15 ml/min: p<0,05 were also observed. Conclusion: The compensatory renal hypertrophy, as well as the glomerular injury translated in the form of proteinuria and sclerosis, are closed related to the volume of the remnant kidney, thus they are more evident when a greater fraction of the renal tissue is excised. RESUMOObjetivo: O estudo analisou os efeitos bioquímicos e morfológicos sobre o rim remanescente em ratos submetidos à ablação cirúrgica progressiva da massa renal. Métodos: Foram utilizados 60 ratos machos Wistar, pesando entre 210 e 380g, distribuídos em 3 grupos contendo 20 animais cada. Os ratos dos grupos denominados de 1,2 e 3 foram submetidos à remoção cirúrgica de tecido renal equivalente a ½, 2/3 e 5/6 da massa renal total, respectivamente. Os grupos foram então subdivididos em 2 subgrupos e reoperados em 24 horas (subgrupos 1B, 2B, 3B) e em 8 semanas (subgrupos 1C, 2C, 3C) para remoção do rim remanescente. Foram obtidas coletas de urina de 24 horas e sangue para análise da creatinina sérica, depuração da creatinina e proteinúria na primeira intervenção cirúrgica e por ocasião da reoperação. O rim remanescente foi submetido à avaliação macroscópica do grau de hipertrofia e à análise histológica. Resultados: Houve aumento significativo do volume do rim remanescente (164%) e presença de esclerose glomerular em 40% dos animais submetidos à ablação de 5/6 da massa renal. Purpose:To establish an experimental model of laparoscopic partial nephrectomy (LPN) in rats and to analyze morphological alterations in the renal parenchyma utilizing an electric cautery and harmonic scalpel. Methods: Forty Wistar rats were used, divided in 2 experiments with 20 rats each: experiment I, LPN was performed with an electric cautery and the rats were subdivided into groups A and B; experiment II, LPN was performed with a harmonic scalpel and they were subdivided into groups C and D. The animals in groups A and C were sacrificed shortly after surgery and the remnant kidney was removed to study the following variables: necroses and degeneration. In groups B and D a laparatomy was performed for retrieval of the remnant kidney on the 14th day after surgery to analyze fibrous scarring. Results: For the variables necroses and fibrous scarring, the electric cautery creates, on average, greater width than that produced by the harmonic scalpel (p=0.0002 and p=0.0068 respectively). Regarding the variable of degeneration, we found no significant difference between the two types of scalpels (p=0.1267). Conclusions: LPN in rats is an adequate and feasible experimental model. RESUMO Objetivo:Estabelecer um modelo experimental de nefrectomia parcial laparoscópica (NPL) em ratos e analisar as alterações morfológicas no parênquima renal utilizando-se bisturi elétrico e harmônico. Métodos: Foram utilizados 40 ratos Wistar, distribuídos em dois experimentos com 20 ratos cada: experimento I, NPL utilizando-se de bisturi elétrico e subdividindose os ratos em grupos A e B; experimento II, NPL realizada com bisturi harmônico e subdividindo-se os ratos em grupos C e D. Os animais dos grupos A e C foram sacrificados após a cirurgia para a remoção do rim operado e estudo das seguintes variáveis: necrose e degeneração. Nos grupos B e D a laparotomia para a retirada do rim operado foi após o décimo quarto dia de pós-operatório para a análise da cicatriz fibrosa. Resultados: O bisturi elétrico provocou uma necrose e cicatriz fibrosa mais extensas em relação ao bisturi harmônico (p=0.0002 e p=0.0068 respectivamente). Em relação a variável degeneração, não houve diferença entre os tipos de bisturis (p=0.1267). Conclusões: NPL em ratos é um modelo experimental adequado e factível. O bisturi elétrico causa danos teciduais mais intensos no rim operado quando comparado com o bisturi harmônico. Descritores: Laparoscopia. Nefrectomia. Eletrocoagulação. Bisturi harmônico. Necrose.
