In 1983, the countries of the Americas, with support from the Pan American Health Organization (PAHO), pledged to eliminate human rabies transmitted by dogs 1 . The goal established was initially limited to the main cities and later extended to the entire Region, setting 2005 as the target date.Since then, through the Regional Program for the Elimination of Human Rabies Transmitted by Dogs, the countries have made great efforts to reach the goal, with remarkable success 2,3,4,5 . In the last 20 years, the number of cases of human and canine rabies in the Region has dropped by nearly 90% 6 .Eliminating human and canine rabies in the member states is a technical cooperation priority and key mandate for PAHO. To help analyze the extent to which the goal was reached, PAHO conducted a study entitled Elimination of Human Rabies Transmitted by Dogs in Latin America 7 to evaluate the rabies situation in the Region, resulting in this publication and a summary published in the PAHO Epidemiological Bulletin 8 .
MethodologyPAHO developed standard methods 9 and sent each participating country an interactive CD-ROM with the study methodology and a bibliography pertaining to the subject area. Countries also received a diskette with an Excel spreadsheet
Various authors have identified the potential relevance of innovation system approaches for inclusive innovation, that is, the means by which new goods and services are developed for and by the poor. However, it is still a question how best to operationalize this. Innovation platforms (IPs) represent an example of putting an inclusive innovation system approach into practice by bringing different types of stakeholders together to address issues of mutual concern and interest with a specific focus on the marginalized poor. This paper explores the formation and functioning of IPs with the aim of providing lessons on the conditions and factors that play a role in making them effective. The study shows the importance of social organization, representation, and incentives to ensure a 'true' participatory innovation process, which is based on demand and embedded in the context. Critical to this is a flexible planning process stimulating incremental change through so-called innovation bundles (i.e. combinations of technological, organizational, and institutional innovations) and reflexive learning (systematically challenging constraining factors). Furthermore, local institutions embedded in norms and values are crucial to understand people's decisions. Due to weak linkages between value chain actors, innovation brokers have a vital role in facilitating the innovation process. Overall, IPs are a promising model for inclusive innovation, but they require a careful assessment of and adjustment to the institutional context.
The Global Burden of Disease (GBD) concept has been used by the World Health Organization (WHO) for its reporting on health information for nearly 10 years. The GBD approach results in a single summary measure of morbidity, disability, and mortality, the so-called disability-adjusted life year (DALY). To ensure transparency and objectivity in the derivation of health information, WHO has been urged to use reference groups of external experts to estimate burden of disease.Under the leadership and coordination of WHO, expert groups have been appraising and abstracting burden of disease information. Examples include the Child Health Epidemiology Reference Group (CHERG), the Malaria Monitoring and Evaluation Reference Group (MERG), and the recently established Foodborne Disease Burden Epidemiology Reference Group (FERG). The structure and functioning of and lessons learnt by these groups are described in this paper.External WHO expert groups have provided independent scientific health information while operating under considerable differences in structure and functioning. Although it is not appropriate to devise a single “best practice” model, the common thread described by all groups is the necessity of WHO's leadership and coordination to ensure the provision and dissemination of health information that is to be globally accepted and valued.
The Infectious Diseases Data Observatory (IDDO, https://www.iddo.org) has launched a clinical data platform for the collation, curation, standardisation and reuse of individual participant data (IPD) on treatments for two of the most globally important neglected tropical diseases (NTDs), schistosomiasis (SCH) and soiltransmitted helminthiases (STHs). This initiative aims to harness the power of data-sharing by facilitating collaborative joint analyses of pooled datasets to generate robust evidence on the efficacy and safety of anthelminthic treatment regimens. Participatory surveillance (PS) is the application of participatory rural appraisal methods to the collection of epidemiological information to inform decision-making and action. It was applied in Africa and Asia as part of emergency programs to address the H5N1 highly pathogenic avian influenza (HPAI) pandemic. The approach resulted in markedly increased case detection in countries experiencing HPAI, and a better understanding of the epidemiological situation. Where HPAI was absent and PS was implemented, the method did not result in false positives and contributed to the overall epidemiological assessment that the country was free of disease. It was noted that clarity of surveillance objectives and resulting data needs at the outset was essential to optimize the balance of surveillance methods, size of the program and costs. The quality of training programs and adherence to international guidelines on good PS training practice were important for assuring the competence of PS practitioners. Orientation of senior decision-makers was an important step in assuring effective program management and appropriate use of results. As a problem-solving methodology, PS is best used to rapidly assess situations and inform strategy. Several countries continued PS after the end of projects and went on to apply PS to other health challenges.
