A B S T R A C T ObjectiveTo describe the modeling stages of food and nutrition surveillance in the Primary Health Care of the Unified Health Care System, considering its activities, objectives, and goals. MethodsDocument analysis and semi-structured interviews were used for identifying the components, describe the intervention, and identify potential assessment users. ResultsThe results include identification of the objectives and goals of the intervention, the required inputs, activities, and expected effects. The intervention was then modeled based on these data. The use of the theoretical logic model optimizes times, resources, definition of the indicators that require monitoring, and the aspects that require assessment, identifying more clearly the contribution of the intervention to the results. ConclusionModeling enabled the description of food and nutrition surveillance based on its components and may guide the development of viable plans to monitor food and nutrition surveillance actions so that modeling can be established as a local intersectoral planning instrument.

Objective: to evaluate the implementation of the Results Control, Monitoring and Evaluation System (e-Car) at the Health Surveillance Secretariat (SVS), Brazilian Ministry of Health, in the period 2012-2015. Methods: this was an evaluation study using mixed methods for collection of primary and secondary data, followed by definition of the system's degree of implementation and analysis of the influence of the political-organizational context. Results: the e-Car System was considered to have been implemented (75.4%) at SVS; the worst scores for its structure dimension related to the computer and operating system maintenance service, as well as little knowledge of the system manual; as for the system's process dimension, the existence of collegiate bodies was considered beneficial for monitoring. Conclusion: the e-Car System had been implemented at SVS; however, its sustainability was fragile, and SVS was recommended to institutionalize and strengthen its monitoring and evaluation practices.

Resumo Objetivos: avaliar o fornecimento de leite humano de forma exclusiva aos prematuros em uma Unidade de Terapia Intensiva Neonatal e a influência dos contextos externo e organizacional no grau de implantação dessa intervenção. Métodos: trata-se de uma avaliação de implantação com análise dos contextos externo (situação sociodemográfico das mães, rede de apoio e marketing da indústria) e organizacional (pertencente à unidade hospitalar). Para definir o grau de implantação, foi utilizada a Matriz de Análise e Julgamento, considerando a dimensão conformidade, e as subdimensões disponibilidade e qualidade técnico-científica. Os dados utilizados foram obtidos por meio de entrevistas, questionários semiestruturados e análise de documentos da instituição. Resultados: o grau de implantação da intervenção foi de 80,74%, mostrando-se satisfatório, com destaque para a subdimensão qualidade técnico-científica. Conclusões: o sucesso no fornecimento de leite humano está atrelado às políticas públicas, ao apoio e orientações oferecidos às mães na unidade hospitalar, disponibilidade de rede de apoio, conhecimento das mães acerca dos benefícios do aleitamento materno, infraestrutura adequada e disponibilidade de insumos. Os prematuros estarem internados em hospital amigo da criança contribuiu para a implantação da intervenção.
