Un número considerable de estudiantes presenta dificultades de aprendizaje y bajo rendimiento académico, sin embargo su evaluación no siempre deriva en un diagnóstico concreto. Son categorizados como inmaduros, pero no se determinan ni la naturaleza ni las características de sus dificultades. El objetivo de este estudio fue identificar los dominios evolutivos afectados en niños con retraso del desarrollo (RD) con la finalidad de valorar el concepto de dificultades neuroevolutivas como categoría comprensiva y perfil de las dificultades generalizadas de aprendizaje. Para ello, se realizó una revisión sistemática en las bases electrónicas Medline, PsycINFO, WOS, Eric, Dialnet y CSIC y, tras aplicar los criterios de inclusión, se seleccionaron 18 artículos. Los resultados confirman que RD se utiliza como etiqueta diagnóstica para caracterizar a niños con retrasos significativos en uno o varios ámbitos del desarrollo, pero no existe una definición de consenso ni criterios específicos para su diagnóstico, y solo sería de aplicación a niños de corta edad. Los dominios afectados coinciden con funciones neuroevolutivas y, en su etiología, destacan factores de riesgo biológico y ambiental. Las dificultades neuroevolutivas abarcarían un amplio espectro de déficits con distintos niveles de gravedad que, al interactuar entre sí, dan lugar a una variedad de perfiles. A considerable number of students have learning difficulties and low academic performance, yet their evaluation does not always lead to a concrete diagnosis. They are categorized as immature, but neither the nature nor the characteristics of their difficulties are determined. The aim of this study was to identify the developmental domains which are affected in children with developmental delay (DD) in order to assess the concept of neurodevelopmental difficulties as a comprehensive category and profile of generalized learning difficulties. To this end, a systematic review was carried out on the electronic databases Medline, PsycINFO, WOS, Eric, Dialnet and CSIC and, after applying the inclusion criteria, 18 articles were selected. The results confirm that DD is used as a diagnostic label to characterize children with significant delays in one or more developmental domains, but there is no definition of consensus nor specific criteria for its diagnosis, and it would only be applicable to young children. The affected domains coincide with neurodevelopmental functions, and biological and environmental risk factors stand out in their aetiology. Neurodevelopmental difficulties would encompass a wide spectrum of deficits with different levels of severity that, on interacting with each other, give rise to a variety of profiles.
El estudio de las posibles causas de las dificultades lecto-escritoras ha generado muchos conocimientos sobre su génesis, y se configura como un campo que todavía no está cerrado. En la etiología de las dislexias se han barajado factores genéticos, neurológicos, neurofisiológicos, cognitivos, madurativos, de personalidad, pedagógicos, etc. Sin embargo, resulta evidente la dificultad de poder identificar una única causa que las explique, ya que estos factores incluyen numerosas estructuras, procesos y funciones que, de modo interdependiente, contribuyen al dominio lecto-escritor. Este hecho complica más su estudio. Abordamos el estado de la cuestión sobre la etiología de las dislexias. Para ello se revisan diferentes estudios considerando los diversos enfoques que mayor visibilidad tienen en la actualidad. Concretamente, se revisan los estudios neurológicos, los neurofisiológicos, los estudios cognitivos y los genéticos. Las aportaciones más notorias, si bien, lejos de ser concluyentes, tratan de demostrar que las conexiones entre las disfuncionalidades hemisféricas y las dificultades lecto-escritoras son cada vez más precisas y prometedoras.
ResumenLas dificultades lectoescritoras -dislexia-afectan a un 5-10% de los niños en edad escolar. Se trata de un trastorno que conlleva múltiples consecuencias negativas en los diferentes ámbitos del desarrollo. Para conseguir reducir dicha prevalencia y mejorar los resultados de las intervenciones, se hace necesaria la detección precoz. Para ello, se necesita de una evaluación temprana capaz de detectar a aquellos niños en riesgo de desarrollar el trastorno. De acuerdo con ello, el objetivo de este trabajo ha sido confeccionar, desde la perspectiva de la neuropsicología escolar, una propuesta de evaluación para la detección temprana de estas dificultades. It is a disorder that involves multiple negative consequences. Early detection is necessary to achieve reduce prevalence and improve outcomes of interventions. To do this, it is need an early evaluation that can detect those children at risk of developing the disorder. Accordingly, the objective of this work has been make an evaluation proposal for early detection of literacy difficulties, from the perspective of school neuropsychology.
Algunas complicaciones maternas perinatales específicas pueden ser determinantes en el desarrollo infantil. Se constata que muchos niños con una amplia variedad de alteraciones han sufrido diversas complicaciones durante su desarrollo gestacional. Por lo tanto, los riesgos maternos ocurridos a lo largo del período pre-perinatal llevan asociados, en ocasiones, diferentes daños, sean neuropsicológicos, físicos y/o psicosociales. En el presente artículo se identifican los principales factores asociados al riesgo perinatal y sus implicaciones, haciendo un recorrido por los procedimientos estandarizados de evaluación del riesgo, desde la perspectiva médico neuropsicológica y psicosocial del neonato.
La pragmática es un elemento clave en la comunicación al analizar cómo los hablantes producen enunciados en un determinado contexto. La pragmática considera los elementos extralingüísticos que determinan el uso del lenguaje. Regula, por tanto, el uso del lenguaje en función de la relación que se establece entre el enunciado, el contexto y los interlocutores. Las dificultades pragmáticas repercuten en la comunicación social funcional eficaz del sujeto que las padece, en su participación social, en sus relaciones y logros. Dentro de las alteraciones pragmáticas pueden existir dificultades comunicativas diversas, verbales o no, que limitan y deterioran la comunicación comprensiva y expresiva con propósitos sociales y que afectan a la adaptación hacia el contexto comunicativo, al seguimiento de las normas conversacionales y a la comprensión del lenguaje no explícito, que se tipifican, en el DSM-5, como Trastorno de la comunicación social (TCS). Se trata de un trastorno poco conocido y resulta ser, además, el menos preciso de todos los trastornos de la comunicación, debido a que sus manifestaciones no se observan dentro de dimensiones objetivables y medibles como la semántica o la fonología, sino que se evidencian dentro de la heterogeneidad y amplitud que se delimitan en la interacción social, implicando a las habilidades sociales y a la adaptación de la conducta. La imprecisión actual del TCS es evidente. A esta imprecisión cabe añadir el hecho de que el perfil de dificultades pragmáticas podría variar, pues podría involucrar la existencia de diferentes subtipos de dificultad pragmática. Hay propuestas que consideran al TCS como síntoma, frente a otras que le confieren una entidad gnoseológica propia. Para clarificar esta situación se hace un estudio bibliográfico de las publicaciones que versan sobre el TCS (años 2000-2014) en el WOS. Los resultados obtenidos, tras analizar los artículos seleccionados, muestran que las dificultades pragmáticas repercuten en la comunicación social del sujeto que las padece. La problemática con la pragmática es, en un gran número de estudios, encuadrada como sintomatología de otros trastornos, principalmente dentro de TEA e TEL. Los límites entre TCS e TEL y/o TEA son difusos. Este hecho se define por una fuerte comorbilidad entre el trastorno de la comunicación social y otros trastornos, tales como el TEL y el TEA que es necesario concretar a través de diagnósticos fiables y objetivos de las futuras investigaciones.
