Sosiaali- ja terveydenhuollon digitalisaatiolle on asetettu paljon odotuksia niin palveluiden laadun, saavutettavuuden kuin tehokkuuden osalta. Asiakas- ja potilastietojärjestelmiltä odotetaan paljon tavoitteiden saavuttamiseksi. Asiakastietojärjestelmät ovat laajasti käytössä sosiaalialalla, mutta niiden käyttöä työvälineenä ja niiden avulla saavutettuja hyötyjä on tutkittu ammattilaisten näkökulmasta vain vähän. Keväällä 2019 kartoitettiin ensimmäistä kertaa valtakunnallisesti sosiaalialan ammattilaisten käyttäjäkokemuksia asiakastietojärjestelmistä. Tässä artikkelissa kuvataan kyselytutkimuksen toteutus ja tulokset eniten käytettyjen tuotemerkkien osalta. Tutkimus toimii pilottina vuonna 2020 toteutettavalle sosiaalialan ammattilaisten käyttäjäkokemuskyselylle, joka on osa valtakunnallista Sosiaali- ja terveydenhuollon tietojärjestelmien seurannan ja arvioinnin hanketta (STePS 3.0). Tutkimusaineisto kerättiin keväällä 2019 sähköisellä kyselyllä, joka pohjautui lääkäreille ja hoitajille aiemmin toteutettuihin käyttäjäkokemuskyselyihin. Sosiaalialan kyselyn teemoina olivat tekninen toimivuus, käytettävyys, tuki asiakastapauksen hahmottamiselle ja oman työn hallinnalle, yhteistyö ja tiedonkulku sekä järjestelmien hyödyt ja haitat. Aineisto koostui 1145 eri tehtävässä ja toimintaympäristössä toimivan sosiaalialan ammattilaisen vastauksista. Tätä artikkelia varten aineistosta on analysoitu vastaajien eniten käyttämien asiakastietojärjestelmien tuotemerkkien osalta joukko edustavia väittämiä kyselyn eri teema-alueista. Kaikkien asiakastietojärjestelmien kouluarvosanojen keskiarvo oli 6,7. Asiakastietojärjestelmiä pidettiin teknisesti vakaina ja toimivina, mutta arviot tiedonkulussa ja monialaisen yhteistyön tuessa, asiakkuuksien hallinnassa ja asiakkaasta saatavan kokonaiskuvan hahmottamisessa tietojärjestelmissä olivat varsin kriittisiä. Käyttäjien kokemuksia tutkimalla on mahdollista saada arvokasta tietoa asiakastietojärjestelmien kehittämisen ja suunnittelun tueksi. Jatkotutkimuksen aiheena on yksityiskohtaisempia ja fokusoidumpia analyysejä laajasta kyselytutkimusaineistosta muun muassa eri vastaajajoukkojen osalta.
Artikkelissa tarkastellaan sosiaalialan korkeakoulutettujen ammattilaisten arvioita käyttämistään asiakastietojärjestelmistä. Artikkelissa arvioidaan Sote-tieto hyötykäyttöön strategian tavoitteiden toteutumista sosiaalialan ammattilaisten näkökulmasta ja tuotetaan tuotemerkkikohtaista tietoa käyttäjäkokemuksista kehittämisen tueksi. Artikkelin tutkimuskysymyksenä oli: Minkälaisia arvioita sosiaalialan korkeakoulutetut ammattilaiset antavat käyttämilleen asiakastietojärjestelmille vuonna 2020 käyttäjäkokemuksiinsa perustuen? Tutkimusaineistona hyödynnettiin STePS 3.0 -hankkeessa syys-lokakuussa 2020 ammattiliittojen kanssa yhteistyössä kerättyä tutkimusaineistoa (n=990), josta tähän artikkeliin valittiin ne vastaajat, joilla oli käytössä sähköinen asiakastietojärjestelmä (n=974). Aineistoa analysoitiin kuvailevin menetelmin hyödyntämällä suoria jakaumia ja ristiintaulukointia. Kouluarvosanoista esitettiin keskiarvot, keskihajonnat, keskiarvojen 95% luottamusvälit sekä heikkojen ja kiitettävien arvosanojen osuudet. Tulokset osoittavat, että sosiaalihuollossa käytössä olevien tuotemerkkien välillä on vaihtelua toimivuudessa, käytettävyydessä, työn ja yhteistyön tuessa sekä koetuissa hyödyissä. Tuotemerkkien arvosanojen keskiarvot vaihtelivat kohtalaisesta (6) hyvään (8). Parhaimmat arviot saivat Nappula ja Domacare, joita käytetään yleensä rajatummissa sosiaalihuollon toimintaympäristöissä. Laajasti sosiaalihuollossa käytössä olevista asiakastietojärjestelmistä parhaimman arvion sai Pro Consona (tyydyttävä). Asiakastietojärjestelmien suunnittelu ja mukauttaminen pienemmän ja rajatumman käyttäjäryhmän tarpeisiin on luonnollisesti yksinkertaisempaa ja järjestelmän käytön oppiminen nopeampaa verrattuna suurten käyttäjäryhmien ja monipuolisten toiminnallisuuksien asiakastietojärjestelmiin. Kaikkien asiakastietojärjestelmien kehittämiskohteiksi tunnistettiin yhteistyön ja tiedonvaihdon tukeminen, asiakkaan palvelukokonaisuuden hahmottaminen sekä asiakkaan sähköisen asioinnin kehittäminen. Paikannetut kehittämiskohteet ovat merkittäviä tulevia hyvinvointialueita ajatellen. Asiakastietojärjestelmillä olisi potentiaalia tukea sote-uudistukselle asetettuja tavoitteita, mikäli niitä kehitetään tulevaisuudessa vastaamaan paremmin ammattilaisten tarpeisiin.
This qualitative study examines the facilitators and challenges of integrating Experts by Experience (EbyE) activity in mental health services in the public sector from the perspective of mental health professionals and trained EbyE. The research data consist of four focus group interviews conducted in one hospital district in Finland. In the hospital district, EbyE activity is an established working model in mental health services. The professional focus groups had 9, and the EbyE focus groups had 13 participants. The data were analysed using abductive content analysis. The experiences of professionals and EbyE emphasised the successful integration of EbyE activity. The identified facilitators of integration included an organisational culture that values EbyE activity, facilitating operating structures, and extensive implementation and utilization of EbyE activities. Despite successful integration, certain challenges were described with regard to actors and practices. The findings indicate that the hospital district has overcome many of the obstacles to the integration identified in previous studies. The findings highlight the role of effectively implemented, organised EbyE activity.
Sosiaalialan korkeakoulutettujen ammattilaisten kokemuksia kartoitettiin nyt ensimmäistä kertaa osana valtakunnallista Sosiaali- ja terveydenhuollon tietojärjestelmäpalveluiden seuranta ja arviointi (STePS 3.0) -hanketta. Tutkimuksen tavoitteena on tuottaa tietoa Sote-tieto hyötykäyttöön -strategiassa määriteltyjen tavoitteiden toteutumisesta sosiaalialan ammattilaisten osalta. Tutkimuksessa kysyttyiin, miten kokeneita ja osaavia sosiaalialan ammattilaiset ovat tietojärjestelmien käyttäjinä, miten tietojärjestelmät tukevat sosiaalialan ammattilaisten tiedonhakua ja -vaihtoa ja miten hyvin asiakastietojärjestelmät tukevat sosiaalialan ammattilaisten työtä. Tutkimusaineisto muodostui sähköiseen kyselylomakkeeseen syys-lokakuussa 2020 vastanneiden 990 sosiaalialan ammattilaisen vastauksista, jotka työskentelivät julkisella, yksityisellä tai kolmannella sektorilla moninaisissa sosiaalialan työtehtävissä. Tulosten mukaan sosiaalialan korkeakoulutetut ammattilaiset ovat kokeneita tietojärjestelmien käyttäjiä, mutta järjestelmien tarjoama tuki työlle on paikoitellen heikkoa. Julkisella sektorilla työskentelevien sosiaalialan ammattilaisten arviot tietojärjestelmien tarjoamasta tuesta työlle olivat kriittisempiä muihin sektoreihin verrattuna. Tietojärjestelmiä hyödynnetään tiedonhaussa pääasiallisena asiakastiedon lähteenä, mutta edelleen turvaudutaan myös vanhanaikaisiin ja tietoturvaltaan heikkoihin tiedon haun menetelmiin. Sosiaalihuollon asiakastiedon arkistoa ei vielä juurikaan hyödynnetä sosiaalialalla tiedonhaussa. Tutkimus nostaa esiin tarpeen tunnistaa tietojärjestelmien hyödyntämistä vaikeuttavat esteet sekä huomioida sosiaalialan vuorovaikutukseen ja sosiaalisiin verkostoihin kiinnittyvä käytäntö sekä erilaisiin ammatillisiin työtehtäviin liittyvät tarpeet paremmin tietojärjestelmien kehityksessä, jotta tietojärjestelmät voisivat jatkossa paremmin tukea sosiaalialan ammattilaisia heidän päivittäisessä työssään.
