Background: Depression is a leading cause of morbidity in modern world and introduction of selective serotonin reuptake inhibitors (SSRIs) was a revolution for treating depression. However, sexual dysfunction and weight gain always remained an issue for patients leading to discontinuation of treatment. Vilazodone is a novel SSRI and literature show better efficacy and decrease weight gain and sexual dysfunction. Aim of study: This study aims to compare the efficacy, sexual dysfunction, and weight gain caused due to vilazodone and escitalopram. Methodology: This is an open-label randomized, controlled study; 60 patients diagnosed as depression were divided into two groups of 30 each. One group was started on vilazodone and one on escitalopram randomly. The groups were compared on the basis of efficacy, weight gain, and sexual dysfunction by applying Hamilton Depression Rating Scale (HDRS) and Arizona Sexual Experience Scale (ASEX) at baseline, 4, and 12 weeks interval. Statistical analysis was done by applying Chi-square, t -test, and fisher exact test and descriptive analysis. Results: Vilazodone group shows fall in HDRS with 18.77 ± 4.3, 14.83 ± 3.68, and 9.63 ± 3.06 while it was 18.8 ± 4.09, 14.3 ± 2.96, and 8.43 ± 2.1 at baseline, 4, and 12 weeks, respectively. ASEX score in vilazodone was found to have 16.87 ± 2.97, 15.37 ± 3.1, and 11.73 ± 3.55 while on escitalopram, 16.4 ± 3.35, 17.13 ± 3.48, and 18.37 ± 4.09 at first visit, 4, and 12 weeks, respectively. Patients on vilazodone had mean weight of 69.63 ± 7.7, 69.83 ± 7.83, and 69.9 ± 7.69 while on escitalopram, 72.47 ± 7.95, 72.87 ± 7.9, and 75.6 ± 8.46 at baseline, 4, and 12 weeks, respectively. Conclusions: Our study shows that vilazodone has better efficacy, lesser weight gain, and lesser sexual dysfunction.

Background: Specific learning disability (SLD) is an important cause of academic underachievement among children, which often goes unrecognized, due to lack of awareness and resources in the community. Not much identifiable data is available such children, more so in Indian context. The objectives of the study were to study the demographic profile, risk factors, co-morbidities and referral patterns in children with specific learning disability.Methods: The study has a descriptive design. Children diagnosed with SLD over a 5 years’ period were included, total being 2015. The data was collected using a semi-structured proforma, (based on the aspects covered during child’s comprehensive assessment at the time of visit), which included socio-demographic aspects, perinatal and childhood details, scholastic and referral details, and comorbid psychiatric disorders.Results: Majority of the children were from English medium schools, in 8-12 years’ age group, with a considerable delay in seeking medical help, were referred mostly by the teachers for academic issues. Most of them had all the three disabilities-dyslexia, dysgraphia and dyscalculia. 38.56% of children had ADHD. Psychological maternal stress, developmental issues and various co-morbidities were accompanying in many cases, of which speech delay and fine motor issues were more in children having comorbid ADHD.Conclusions: Awareness, early identification and referral to appropriate services is crucial to deal with the challenge of learning disability. Health professionals should look for early signs in routine visits of children and co-morbidities, particularly ADHD, should be adequately taken care of. Role of media and education system is crucial for its destigmatization in community.

Objective: To assess if the patients with pre hCG high perifollicular PSV values, would benefit with a double IUI, instead of single IUI. Method: A randomized prospective study was done of 350 IUI cycles who were stimulated by either CC or rFSH (recombinant FSH) or Letrozole with rFSH and showed a pre hCG perifolliclular PSV of 15cms/sec. Single IUI was done at 36 -38 hours after hCG injection and in double IUI cases the IUI were done at 12-14 hours and 36-38 hours. Results: With CC stimulated cycles when perifollicular PSV was 15-20, single IUI gave nearly similar conception rates with both single and double IUI. With PSV 20 -25 cms/sec, double IUI showed higher conception rates than single IUI. But when was PSV > 25, conception rates were very low with single IUI than with double IUI. With rFSH cycles with PSV 15-20, conception rates were comparable with both single IUI and double IUI but with PSV > 20, the conception rates were significantly better and with PSV > 25, the pregnancy rates with single IUI were significantly low. Same was the case with letrozole + rFSH cycles. Conclusion: In all cases with PSV > 25 cms/sec on the day of hCG, a double IUI gives better pregnancy rates, but this limit lowers to PSV 20, when it is a rFSH cycle. P20.13Estimation of uterine cavity codition by three-dimensional hysterosonosalpingograpy Background: Congenital and acquired uterine anomalies are relatively often and correlate with infertility and habitual abortions. Objective: To estimate sensitivity and specificity of three-dimensional hysterosonosalpingography (3D HSSG) compared to hysteroscopy. Methods: 60 patients, divided into two groups, were included in this prospective study. In the first group, which consisted of 30 patients, estimates were done by 3D HSSG with negative contrast, and compared to findings of hysteroscopy done on the same patients. The second group, which also consisted of 30 patients, had uterine cavity estimation done by 3D HSSG with hyperechogenic contrast, and compared to finding of hysteroscopy done on these 30 women. Results: Sensitivity and specificity of 3D HSSG with negative contrast was 100% compared to hysteroscopy. Sensitivity of 3D HSSG with hyperechogenic contrast compared to hysteroscopy was 66.6% for synechiae, 94.7% for septum and 100% for other uterine anomalies, while specificity was 100% for all uterine malformations. Conclusion: Results of this study show that 3D HSSG with negative contrast is the best method for uterine cavity vizualisation, and estimation, and it presents precise and minimally invasive alternative, that provides this method to be used in routine ambulatory practice.
