Background Elderly people in nursing homes are particularly vulnerable to COVID-19 due to their age, the presence of comorbidities, and community living. On March 14, 2020, at the beginning of the first epidemic wave of COVID-19 in France, a cluster was reported in a nursing home in the Nouvelle-Aquitaine region. We monitored the outbreak as well as the infection prevention and control (IPC) measures implemented. Methods A confirmed case was defined as laboratory-confirmed COVID-19 in a resident or staff member present in the nursing home between March 7 and May 1, 2020; and a probable case as a person presenting an acute respiratory illness after contact with a confirmed case. Symptomatic inpatient residents and symptomatic staff members were systematically tested for SARS-CoV-2. In addition, two screening sessions were held on site. Results We identified 109 cases (98 confirmed, 11 probable). The attack rate was 66% among residents and 45% among staff. Half of all cases were identified during the screening sessions. One-quarter of cases had minor symptoms or were asymptomatic. The case fatality rate among residents was 29%. IPC measures were rapidly implemented such as the quarantine of residents, the reinforcement of staff personal protective equipment, and home quarantine of staff testing positive, which were supplemented in April by systematic controls at the entrance of the nursing home and the creation of additional staff break rooms. Conclusions This outbreak confirmed the considerable health impact of SARS-CoV-2 transmission in a nursing home. In addition to the implementation of IPC measures, the early detection of cases through the screening of residents and staff is essential to identify asymptomatic and pre-symptomatic cases and limit the spread of the virus.
Cet article fait un survol des tendances internationales des réformes en cours en santé mentale. Il décrit les modèles et les stratégies déployés afin d’améliorer les dispositifs de soins. Pour illustrer les réformes en santé mentale par les exemples du Québec et de la France, l’article se base sur une recension des écrits et sur des recherches en cours sur l’organisation de ces services. Dans la dernière décennie, la consolidation des soins primaires, intégrant le déploiement de bonnes pratiques et une meilleure intégration des services, a été au coeur des transformations du système de santé mentale. Des défis importants d’implantation marquent néanmoins les réformes. L’article contribue à la réflexion sur la planification des services afin d’améliorer leur efficience et permettre le transfert des connaissances acquises au cours de ces transformations.This article is an overview of international tendencies on current mental healthcare transformation. It describes best practices based models and strategies aimed at improving efficiency of mental healthcare systems. To illustrate reforms in Quebec and France, the article reviews current literature and ongoing research on mental health care and service organization. During the last decade, primary care reinforcement, including best-practices and service integration, has been at the core of the mental healthcare system transformation. However, challenges regarding implementation appear to undermine the transformation’s success. This article discusses mental healthcare services planning in order to improve their efficiency and allow the transfer of knowledge acquired through these reforms
Le case management est un processus qui évalue l’interaction des individus avec l’environnement. Il permet d’établir un plan global d’intervention suffisamment souple pour être réactif et intégré. Le case management vise à aider les sujets vulnérables à maintenir et améliorer leurs compétences dans leur environnement habituel. Ce processus de soins et de services est multidimensionnel, dynamique et longitudinal. Le Québec, les États-Unis et la Grande Bretagne développent des modèles de case management. Dans le contexte français, il existe des formes proches du case management dans les pratiques de secteurs qu’ils s’agissent des pratiques infirmières ou des travailleurs sociaux. On parle alors de la référence soignante ou éducative, des fonctions de coordonnateur des soins ou de parcours/projet. Cet article a pour objectif de présenter les formes et présupposés théoriques du case management , d’en discuter les conditions de mise en œuvre, les limites et l’adaptation possible de cette approche en France. Enfin, un lien sera fait entre le case management et l’intégration des soins et des services en réseau.
Les xix e et xx e siècles ont répondu massivement à la question sociale, en organisant les questions posées, et les populations qui en étaient victimes, en « disciplines chargées de les résoudre ». Or, dans les deux dernières décennies du xx e siècle, cette construction disciplinaire se heurte à des situations de vulnérabilité qui n’entrent pas ou plus dans ce paradigme dominant. D’où l’apparition du mot « incasable » pour caractériser ces personnes qui se situent en dehors de ces champs disciplinaires particuliers. Les lois des années 2000 sont une première réponse législative à ce problème : en instituant le plan personnalisé – de compensation, d’aides, de l’enfant, de soins –, qui considère la personne prise en compte comme une ressource, on va s’efforcer de créer les parcours (de vie, de soins) les plus judicieux. Mais cette manière de concevoir l’intervention nécessite, pour être efficace, l’intégration des soins et services à tous les échelons du territoire et la mise en réseau des acteurs. Un exemple de cette stratégie est donné par les dispositifs d’intégration dits MAIA (Maisons pour l’autonomie et l’intégration des malades d’Alzheimer), qui se structurent autour de six leviers : la concertation, le guichet intégré, la gestion de cas, l’évaluation multidimensionnelle, le plan de services individualisé, le système d’information partagé. Malgré un certain nombre de résistances, cette expérience contient de réels facteurs de succès, la gestion de cas s’instituant comme levier de « coordination de parcours ». Un autre exemple pourrait intéresser le Plan autisme dans une démarche méthodique que l’article analyse avec précision. C’est une révolution culturelle nécessaire, une prise en compte de l’éthique de responsabilité, visant une plus grande rationalité de l’action collective organisée.
