The suggested approach of testing different types of phaco settings on equal halves of the same nucleus can be easily applied for quantitative comparisons under identical surgical conditions. The data obtained suggest that IP technology is preferable when compared to a combination US mode for soft lenses while the combined US is more suitable for hard nuclei.
Сравнение плотности эндотелиальных клеток роговицы после факоэмульсификации плотной катаракты с использованием комбинированного ультразвука и торсионного ультразвука с Intelligent Phaco Санкт-Петербургский филиал ФГАУ «МНТК "Микрохирургия глаза" имени академика С.Н. Ярослава Гашека, 21, Санкт-Петербург, 192071, Российская Федерация РЕЗЮМЕ Цель: выбрать из двух вариантов настроек факоэмульсификатора (комбинированный УЗ и торсионный УЗ с Intelligent Phaco (IP)) более безопасный в отношении сохранения эндотелиальных клеток роговицы и уменьшения послеоперационных осложнений. Пациенты и методы. 72 пациентам была выполнена факоэмульсификация с имплантацией ИОЛ. Пациенты случайным образом были разделены на две группы (основная n = 33 и контрольная n = 39). На этапе аспирации фрагментов хрусталика в основной группе использовали комбинированный УЗ, в контрольной группе -торсионный УЗ с IP. Подсчет эндотелиальных клеток и другие исследования были проведены через 1 день, 1 неделю и 6 месяцев после операции. Результаты. Острота зрения с коррекцией вдаль в исследуемых группах через 1 неделю после операции была сходной: в основной группе -0,813 ± 0,228, в контрольной -0,765 ± 0,250. Статистически достоверной разницы в толщине роговицы между группами не отмечено: в основной группе -533,48 ± 12,41, в контрольной -536,44 ± 10,92. В то же время относительно плотности эндотелиальных клеток была выявлена статистически достоверная разница: в основной группе -1871,30 ± 187,41 (через 1 неделю), 1865 ± 178,9 (через 6 месяцев); в контрольной -1809,63 ± 225,43 (через 1 неделю), 1791 ± 230,82 (через 6 месяцев). Процент потери эндотелиальных клеток оказался, соответственно, более низким в основной группе во все сроки наблюдения после операции: 3,90% (через 1 день), 4,54% (через 1 неделю), 4,9% (через 6 месяцев). В контрольной группе: 7,71% (через 1 день), 9,25% (через 1 неделю), 10,4% (через 6 месяцев). Заключение. Данные исследования показали преимущества комбинированного УЗ по сравнению с торсионным УЗ с IP при аспирации плотного хрусталика. Павлова, 4 СПбГБУЗ «Городская многопрофильная больница № 2», Санкт-Петербург Проведена сравнительная оценка характера и степени структурных изменений, возникающих после операции в зоне роговичного разреза, сформированного с помощью фемтосекундного лазера «Victus» (Bausch & Lomb, TPV, США) и кератома «Mani» 2,2 мм (Mani, Япония) по данным оптической когерентной томографии.Результаты проведенного исследования показали преимущества формирования основного многопрофильного туннельного роговичного разреза с использованием фемтолазерной технологии: точность и воспроизводимость за-данных параметров профиля и размеров; возможность выбора параметров ширины, длинны, угла, профиля разреза; минимальные структурные повреждения роговичного разреза в послеоперационном периоде. Based on optical coherence tomography, we compared the character and extent of structural changes occurring postoperatively in the corneal incision created with a femtosecond laser (Victus, Bausch & Lomb, TPV, USA) or a keratome (2.2 mm, Mani, Japan). Our findings showed that a corneal tunnel incision created with a femtosecond laser was more beneficial because it was more accurate and allowed reproducibility of the specified profile and size parameters as well as the choice of the width, length, angle and profile parameters. The corneal tunnel incision created with a femtosecond laser caused minimal structural changes in the corneal incision postoperatively.
Акутальность. В настоящее время увеличилось количество пациентов с возрастной катарактой, которым в 80-90-х была выполнена радиальная кератотомия. Точный расчет ИОЛ у таких пациентов затруднен и требует индивидуального подхода. Цель. Представить клинический случай имплантации торической трифокальной интраокулярной линзы пациенту после радиальной кератотомии. Материал и методы. Пациенту П. 52 лет в 1989году на оба глаза была выполнена передняя радиальная кератотомия. В сентябре 2019 года пациент пришел на предоперационную диагностику по поводу хирургического лечения катаракты. Было выполнено несколько вариантов расчета ИОЛ с использованием различных данных кератометрии и различных формул. В ходе фемтофакоэмульсификации катаракты имплантирована ИОЛ PanOptixToric. Результаты. Через 3 месяца после операции острота зрения вдаль составила: 0,15/0,75.(LogMAR/ таблица Снеллена), и 0.0/1.0 для средней и близкой дистанции, соответственно. Выводы. Имплантация мультифокальной торической ИОЛ у пациентов с роговичным астигматизмом после перенесенной радиальной кератотомии может позволить получить высокие зрительные функции. Радиальная кератотомия не является противопоказанием к имплантации мультифокальной торической ИОЛ.
Purpose. To compare quality of life and patient satisfaction in patients after cataract phacoemulsification with implantation of trifocal IOLs ZEISS AT Liza try 839 MP and AcrySof IQ PanOptix using an online computer simulator (Halo & Glare Simulator) and a questionnaire. Materials and methods. The study included 40 patients with senile cataracts who underwent phacoemulsification of cataracts with implantation of trifocal diffractive IOLs. In the postoperative period, patients were questioned to determine subjective satisfaction with the treatment result, according to the results of which the lowest number of points corresponded to the maximum satisfaction. The perception of halo and glare effects was investigated using an online computer simulator (Halo & Glare Simulator), which uses a scale of halo intensity and size from 0 to 100. The simulator classifies halos into three types: T1, T2 and T3. Results and discussion. The mean halo sizes for PanOptix and AT Liza try 839 MP were 31.15±23.88 and 30.14±21.62 (p-value=0.9683), respectively; in terms of halo intensity 34.77±20.38 and 32.43±27.69 (p-value=0.7511), respectively; glare size was 23.46±18.91 and 23.14±9.58 (p-value=0.5785), respectively; glare intensity was 27.31±21.52 and 35.57±8.34 (p-value=0.6055), respectively. The mean values of the sum of points according to the results of the questionnaire survey for PanOptix and AT Liza try 839 MP were 8.46±6.75 and 9.7 ±7.25 (p-value=0.6055), respectively. Conclusion. Our data demonstrate no significant differences in the size and intensity of halo and glare effects, as well as in the sum of scores according to the results of subjective patient satisfaction after implantation of two compared trifocal diffraction IOLs ZEISS AT Liza try 839 MP and AcrySof IQ PanOptix. Purpose. To compare the results of trifocal IOL calculation using various corneal tomographic data (ring and zone). Methods. This retrospective study involved 46 patients (46 eyes), underwent cataract surgery with trifocal IOL implantation (AcrySof IQ PanOptix). The calculation was performed using Tomey OA-2000 according to 2 formulas (Barrett II Universal, Olsen). Keratometry values included Km (the average of two main meridians of a cornea) provided by Pentacam HR Power Distribution Apex map, which describes total corneal refractive power (TCRP) with diameter of 3.0, 4.0 and 5.0 mm on a ring and zone. Mean (MAE) and median (MedAE) predicted postoperative refraction errors were assessed after surgery. Results. Mean Km value on 3 mm zone and ring was: 42.75±1,46 D and 42,91±1,43 D, respectively (p<0,0001). Mean Km on 4 mm zone and ring was: 42.6±1.5 D and 43.3 ± 1.5 D, respectively (p <0.005). Mean Km value on 5 mm zone and ring was: 43,09±1,5 D and 43,55±1,48 D, respectively (p<0,0001). Calculations using the Barrett II Universal formula revealed significant difference between MAE and MedAE of the predicted postoperative refraction on 5mm zone and ring (p=0.045). When using the Olsen formula in the calculations, significant difference was revealed using the Km data with a diameter of 3 mm and 5 mm (p=0.001 и p=0.009, respectively). The calculation on 3 mm ring was more accurate than for 3 mm zone. With a 5 mm diameter, the calculation is more accurate according to the zone data. Conclusion. Mean Km value on Power Distribution Apex map according to ring is significantly greater then according to zone. 1) The calculation of the trifocal IOL based on the TCRP zone data is reliably more accurate than the ring data according to both formulas (Barrett II Universal and Olsen) with a diameter of 5 mm. 2) According to the Olsen formula with a diameter of 3 mm, the calculation of the optical power of trifocal IOL based on TCRP ring data is more accurate.
