Objective of the Review: To systematise the information on the incidence of diabetes mellitus (DM) in patients who had the novel coronavirus infection; on the features of virus mechanism activation and pathological cascades in the body; on potential ways to control blood glucose during COVID-19. Key Points. During the novel coronavirus pandemic (COVID-19) associated with high death toll in all countries, specific groups of patients were identified, where the death rate was higher than the mean figures. These groups include DM patients. DM is associated with a high risk of severe coronavirus infection and is the second common comorbidity during COVID-19. DM, degree of its compensation and complications progression impact prognosis, coronavirus infection clinical course and survival. Therefore, effective teamwork of the healthcare professionals and patient is essential. The primary objective of coordination is creation of a clear algorithm of DM management during COVID-19: strict blood glucose monitoring and prompt intensification both of antihyperglycemic therapy and specific ethiopathogenetic management of COVID-19. Conclusion. DM patients require special attention and timely adequate assistance both in outpatient and inpatient settings. Long-term follow-up of DM patients after COVID-19 to minimise the risk of DM complications is also very important. Keywords: COVID-19, diabetes mellitus, novel coronavirus infection.
Цель исследования-выявление специфических иммунных изменений, отражающих состояние микроструктур нервной ткани при шизоаффективном психозе, и установление их возможной связи с герпетической инфекцией. Дизайн: аналитическое контролируемое исследование. Материалы и методы. Были обследованы 56 пациентов с шизоаффективным психозом (F-25 по Международной классификации болезней 10-го пересмотра) в состоянии обострения и 100 психически здоровых лиц контрольной группы. Для определения сывороточного содержания нейротропных аутоантител (ауто-АТ) класса IgG использовался метод «ЭЛИ-Н-Комплекс-12» на основе твердофазного иммуноферментного анализа (ИФА). Уровни IgM и IgG к герпес-вирусам в сыворотке крови определяли методом твердофазного ИФА. Результаты. Показаны аномалии профилей ауто-АТ (преимущественно к основному белку миелина, белкам S-100 и NF-200, глиальному фибриллярному кислому белку), отражающие деструктивные и воспалительные процессы в тканях нервной системы. Выявленные корреляции большинства изученных параметров с содержанием сывороточных АТ к вирусам группы герпеса (к вирусу простого герпеса 1-го, 2-го и 6-го типов, вирусу Эпштейна-Барр, цитомегаловирусу) предполагают связь отмеченных процессов с герпетической инфекцией. Заключение. Установленные иммунные аномалии являются вторичным явлением при развитии инфекционно-воспалительных процессов, сопряженных с вирусами рассматриваемой группы. В свою очередь, эти процессы выступают в качестве триггера стойких аутодеструктивных реакций в нервной ткани. Objective of the Review: To describe the problem of chronic musculoskeletal pain associated with a previous SARS-CoV-2 infection. Key Points. COVID-19 is a disease caused by the novel coronavirus infection SARS-CoV-2, characterised by multiple organ damage, systemic immune inflammation, coagulopathy, and serious neuroendocrine and metabolic disorders. The consequences of COVID-19, even with a relatively favourable course of this disease, can be degenerative changes in many organs (primarily, diffuse fibrosis), various functional and mental disorders. Therefore, in some patients (10% to 50%, depending on the severity of the course of COVID-19), various symptoms of the disease persist for a long time after the acute manifestations of the infection subside. This condition has been called “post-COVID syndrome” (PCS), or “long-term COVID-19”. One of the main manifestations of PCS is musculoskeletal pain, weakness and fatigue. COVID-19-induced damage to the musculoskeletal and nervous systems can contribute to the exacerbation of diseases characterised by chronic pain. PCS management should be aimed at correcting functional disorders, pain control, and active rehabilitation of patients. Conclusion. Chronic pain is one of PCS manifestations that requires special attention, timely diagnosis, and comprehensive individual therapy. Keywords: COVID-19, post-COVID syndrome, musculoskeletal pain.
Objective of the Review: To review literature data on the need for cardiological rehabilitation of COVID-19 survivors and the features of rehabilitation of patients with cardiovascular pathologies during pandemic. Key Points. SARS-CoV-2 virus uses several cardiotropic mechanisms and can damage the myocardium. The COVID-19 pandemic can result in an increase in the number of patients with acute and chronic cardiological pathologies who require rehabilitation. Traditional cardiological rehabilitation has a limited range of measures during a pandemic; the majority of experts recommend rehabilitation at home and using telemedicine. Conclusion. COVID-19 survivors can need cardiological rehabilitation. Remote management is a key trend in cardiological rehabilitation during a pandemic. Моделирование динамики импорта РФ с помощью модели коррекции ошибок В работе оценивается модель коррекции ошибок для агрегированного импорта РФ. На первом шаге с помощью DOLS оценивается долгосрочная функция спроса на импорт, зависящая от показателя агрегированных расходов и относительных цен, на втором шаге оцениваются параметры краткосрочной динамики с помощью OLS и GMM. В качестве переменной агрегированных расходов рассматривается шесть альтернативных показателей. Наилучшей детерминантой динамики импорта с точки зрения минимизации ошибки предсказания и наукастинга вне выборки оказывается ВВП за вычетом госрасходов.
