Este estudo objetivou analisar o envolvimento de equipes da Atenção Básica à Saúde nas ações de controle da tuberculose, ante a percepção dos coordenadores do Programa de Controle da Tuberculose de nove municípios prioritários do Estado de São Paulo. Trata-se de uma pesquisa qualitativa, cujos dados foram coletados em junho/2005 por meio de entrevista semi-estruturada com nove coordenadores e analisados pela técnica de análise de conteúdo-modalidade temática. Os resultados apontaram dificuldades para incorporação das ações de controle da tuberculose, na atenção básica, relacionadas à debilidade quantitativa e qualitativa de recursos humanos e à visão centralizada e fragmentada da organização dessas ações no sistema de saúde. A integração das atividades de controle da tuberculose na atenção básica será possível mediante organização do sistema de saúde, seguindo os princípios da atenção primária e elaboração/implementação de uma política de recursos humanos que garanta formação e capacitação contínua das equipes de saúde.
OBJECTIVE:To assess the access to tuberculosis treatment in healthcare services with Programa Saúde da Família (PSF -Family Health Program) and at a reference outpatient clinic. METHODS:A descriptive inquiry was carried out in 2007 with 106 patients who received tuberculosis treatment through the PSF or the reference outpatient clinic in Campina Grande, Northeastern Brazil, from July 2006 to August 2007. To assess the health services, the instrument Primary Care Assessment Tool was used, validated and adapted to assess tuberculosis care in Brazil. The main variables analyzed referred to the transportation and distance to the service and patients' supervision. RESULTS:Of the 106 patients, 83.9% performed self-administered treatment and 16.0% received supervised treatment. The indicators from the PSF units and from the reference outpatient clinic that were similar (p>0.05) were: 65.1% "losing half work day to attend the medical visit"; 65.0% "having to use motorized transport"; 50.0% "always having to pay for motorized transport"; and 69.0% "not receiving treatment at healthcare units near home". The indicators "using motorized transport", "paying for transport to attend the medical visit" and "receiving treatment near home" were statistically different (p<0.05) between the services. Standardized and non-standardized Cronbach's alpha coefficients were, respectively, 0.7275 and 0.7075, based on the eight items of the questionnaire. CONCLUSIONS:Although the city has 85 PSF teams, supervised treatment was carried out by few health workers. Although the tuberculosis treatment is offered by the public health service, it still represents a cost to the patients, due to the distance to the healthcare service and losing half work day in order to attend medical visits.
Objective: This study aimed to characterize the epidemiological profile of tuberculosis cases reported in the city of Ribeirão Preto between 1998 and 2003, according to patient HIV status, gender, age bracket and treatment outcome. Methods:This was a descriptive epidemiological study that employed the Brazilian National Tuberculosis NotificationDatabase as an instrument of data collection. The study sample consisted of all cases of human immunodeficiency virus/ tuberculosis co-infection occurring in residents of Ribeirão Preto and reported between 1998 and 2003. Results: During this period, 1273 new cases of tuberculosis were reported, 377 of which were in HIV-positive individuals, for a coinfection rate of 30%. Of the cases of co-infection, 76% were in men, and the majority occurred in individuals in the 20-59 age bracket. In terms of treatment outcome, cure was achieved in 52%, treatment abandonment was reported in 11%, and death occurred in 32%. The predominant clinical form of tuberculosis was the pulmonary form, which accounted for 58% of the cases. Conclusion: A high prevalence of co-infection was observed in the community studied.The treatment outcomes seen among the cases in our study sample underscore the need to adopt special strategies to monitor this clientele. Comparing the cases of tuberculosis in isolation with the cases of co-infection, no gender-related or age-related differences were observed.
RESUMO A busca de sintomáticos respiratórios (BSR) é uma das prioridades do Programa Nacional de Controle da Tuberculose. Espera-se que esta atividade esteja inserida no cotidiano de trabalho dos Agentes Comunitários de Saúde (ACS), visando a detecção precoce de casos. O objetivo do estudo foi analisar a incorporação da BSR no controle da tuberculose sob a percepção do ACS. A coleta de dados foi realizada através do levantamento de resultados de baciloscopias antes e após a implantação da BSR no Programa de Agentes Comunitários de Saúde (PACS) e através de aplicação de questionário aos ACS. Após a implantação da BSR houve aumento de baciloscopias solicitadas nos dois primeiros meses, com decréscimo nos meses seguintes. Os aspectos que poderiam estar dificultando a sustentabilidade da BSR são de natureza operacional, relacionados ao trabalho externo do ACS, e de organização dos serviços de saúde (falta de integração entre os diferentes pontos de atenção do sistema de serviços de saúde: descontinuidade do fluxo de informação, rigidez nas rotinas/horário de envio de escarro). Conclusão: Concluiu-se que a abordagem junto às famílias é uma atividade complexa exigindo que os ACS tenham uma supervisão permanente da enfermeira ou médico.
OBJETIVO: Avaliar o desempenho dos serviços de saúde no controle da tuberculose para o estabelecimento do vínculo entre doente e profissional de saúde no Município de Ribeirão Preto - SP, em 2007. MÉTODOS: Foi aplicado um questionário a 100 doentes, elaborado para atenção básica e adaptado para atenção à tuberculose, que contém dez indicadores de avaliação de vinculo. RESULTADOS: Mais de 60% dos doentes de cada unidade referiam que conversam sobre outros assuntos além da tuberculose com os profissionais da saúde, mais de 90% relataram ter tempo suficiente para esclarecer dúvidas sobre o tratamento. Mais de 50% dos doentes de cada ambulatório relataram procurar o médico quando necessitam de cestas básicas ou vale-transporte. CONCLUSÃO: A organização da assistência à tuberculose realizada por equipes especializadas nos Programas de Controle da Tuberculose apresentou indicadores favoráveis ao vínculo, o que pode contribuir para a identificação das necessidades e busca de soluções.
Estudo epidemiológico descritivo que analisa a coordenação da assistência ao doente de Tuberculose em Serviços de Atenção Primária segundo 23 doentes, 16 profissionais e 17 gestores em Ribeirão Preto-SP, através de instrumento adaptado para avaliar a tuberculose. De acordo com os informantes, a coordenação da assistência ao doente em tratamento pela equipe do programa de controle da tuberculose foi considerada satisfatória. No entanto, quando há necessidade de encaminhar o doente a outros pontos de atenção, a coordenação da assistência apresenta pontos deficientes como descontinuidade do fluxo de comunicação e participação incipiente do doente no processo de atenção, havendo necessidade de aumentar a responsabilização pelo cuidado do doente e estimulá-lo como agente ativo do processo.
Objective: to evaluate the quality of the sputum sample before and after the Nursing guidance to patients. Methods: this is a quasi-experimental research design, single group type, before and after, non-randomized study. The study enrolled patients with suspected pulmonary tuberculosis, respiratory symptomatic patients for over three weeks, aged over 18 years, of both genders and without tuberculosis history in the last two years. The educational intervention consisted of individualized guidance on the collection of sputum sample, which was based on the guidelines of the Ministry of Health of Brazil and on the explanatory folder delivery. Results: in this study participated 138 patients with suspected pulmonary tuberculosis. The results showed significant increase of the samples with purulent particles, volume greater than 5 mL and increased rate of patients diagnosed with tuberculosis, after the educational intervention. Conclusion: it was shown that after the educational intervention, it was observed sputum samples with better quality, with satisfactory aspect and volume for the effectiveness of the bacilloscopic examination.
