Este estudo procurou analisar, sob a percepção dos enfermeiros supervisores do Programa de Agentes Comunitários de Saúde (PACS), a incorporação da busca ativa de sintomáticos respiratórios (SR) para o controle da tuberculose em um distrito de saúde do município de Ribeirão Preto. A população estudada constituiu-se de oito enfermeiros/supervisores. Utilizou-se entrevista semi-estruturada como instrumento de coleta de dados. O tratamento dos dados fundamentou-se no método de análise de conteúdo, elegendo-se duas Unidades Temáticas: 1) a inserção do PACS em Unidades de Saúde e suas debilidades, e 2) a relação entre o PACS e a Unidade de Saúde nas ações de controle da Tuberculose. As debilidades apontadas referem-se ao processo de conformação e organização do PACS: ausência de discussão sobre a inserção do agente na equipe de saúde; ausência de liderança municipal do PACS; acúmulo de funções do enfermeiro supervisor; capacitação insuficiente dos agentes. Quanto à incorporação das ações de controle da tuberculose apontam que a descentralização poderia resultar em benefícios para o usuário. Entretanto, julga-se necessário repensar a forma de organização dos serviços de saúde de modo que seja assumido um conjunto de ações que avancem para uma nova lógica de trabalho.
Objective: This study aimed to characterize the epidemiological profile of tuberculosis cases reported in the city of Ribeirão Preto between 1998 and 2003, according to patient HIV status, gender, age bracket and treatment outcome. Methods:This was a descriptive epidemiological study that employed the Brazilian National Tuberculosis NotificationDatabase as an instrument of data collection. The study sample consisted of all cases of human immunodeficiency virus/ tuberculosis co-infection occurring in residents of Ribeirão Preto and reported between 1998 and 2003. Results: During this period, 1273 new cases of tuberculosis were reported, 377 of which were in HIV-positive individuals, for a coinfection rate of 30%. Of the cases of co-infection, 76% were in men, and the majority occurred in individuals in the 20-59 age bracket. In terms of treatment outcome, cure was achieved in 52%, treatment abandonment was reported in 11%, and death occurred in 32%. The predominant clinical form of tuberculosis was the pulmonary form, which accounted for 58% of the cases. Conclusion: A high prevalence of co-infection was observed in the community studied.The treatment outcomes seen among the cases in our study sample underscore the need to adopt special strategies to monitor this clientele. Comparing the cases of tuberculosis in isolation with the cases of co-infection, no gender-related or age-related differences were observed.
Resumo Este trabalho apresenta informações geográficas provenientes do primeiro levantamento epidemiológico da tuberculose realizado junto à Vigilância Epidemiológica da Secretaria Municipal de Saúde do município de Ribeirão Preto, para o período de 1990 a 2000, objetivando contribuir tanto para o entendimento do processo endêmico da tuberculose quanto para a complementação do sistema de informação em saúde do município. Os dados foram coletados por meio de consulta direta às fichas de notificação compulsória, para o período entre 1990 e 1997 e ao programa computacional Epi-Tb para os demais anos. O banco de dados foi elaborado utilizando a técnica da dupla verificação. As informações foram compiladas e geocodificadas com o auxílio do software MapInfo Professional, versão 6.5. Os casos de tuberculose estiveram concentrados em uma faixa da região noroeste do município, consistindo de bairros de classe-média ou média-baixa da população. Os resultados contribuem para o conhecimento da distribuição espacial da tuberculose no município de Ribeirão Preto, ressaltando a importância da categoria espaço como alternativa metodológica para auxiliar no planejamento, monitoramento e avaliação das ações em saúde, direcionando as intervenções para diminuir as iniqüidades. Summary This study presents results from the first epidemiological Tuberculosis survey held in the Epidemiological Surveillance Department of the Municipal Health Secretariat in Ribeirão Preto-São Paulo-Brazil for the period from 1990 to 2000, obtained by directly consults to the compulsory notification cards and electronic consultations were also realized by means of the Epi-Tb computer program from 1998 onwards, with a view to contributing to the understanding of the endemic process state of Tuberculosis and to the complementation of the municipal health information system. The information was compiled and geo-codified with the help of the MapInfo Professional software, version 6.5. Many cases were concentrated in the northeast region in the city, which consists the quarters of medium and low economical income. The results contribute to show the Tuberculosis spatial distribution in Ribeirão Preto, they also reinforce the space category as a methodological alternative to manage, to monitor and to evaluate the health actions by directing interventions to decrease the iniquities.
Esta investigação teve como objetivo analisar através da visão da equipe executora, o tratamento supervisionado no controle da tuberculose, implantado nas Unidades gerenciadas pela Secretaria Municipal de Saúde de Ribeirão Preto, a fim de caracterizar, a finalidade desta intervenção e suas formas de abordagem. A população do estudo constituiu-se de 13 profissionais envolvidos diretamente com o Programa de Controle da Tuberculose. Utilizou-se a entrevista semi-estruturada como instrumento de coleta de dados. O tratamento dos dados fundamentou-se no método de análise de conteúdo, permitindo eleger como Unidade temática "o tratamento supervisionado como perspectiva de mudança na prática: uma intervenção de que tipo?". Concluiu-se que, o tratamento supervisionado se coloca na prática como uma intervenção permeada por diversas formas de abordagem: como uma ação de enfoque terapêutico; como um meio de reduzir a transmissão seja através do controle dos comunicantes, seja privilegiando ações educativas no âmbito familiar; e finalmente, como uma possibilidade de ampliar a capacidade de interação e atuação dos agentes, através de estabelecimento de vínculo, acolhimento, responsabilidade, na perspectiva de garantir a adesão do paciente ao tratamento e maior qualidade da atenção.
