A good health care does not only depend on good medical practice, but also needs great management of its resources, which are generally short. In this sense, PROAHSA has been training new health managers since 1972. With the arrival of the COVID-19 pandemic, it was clear that medicine will go through a new phase, where telehealth will be present in this ''Improved Normal''. This report is about how a pilot teleconsultation study was carried out for HCFMUSP patients through the Scrum-like framework. It is to deploy a pilot of remote assistance involving a doctor and a patient in the Ambulatory of Hepatology and Liver Transplantation of HCFMUSP. METHODS: We applied the Scrum-like framework to carry out this work with an interdisciplinary multifunctionality team. RESULTS: A full telemedicine service flow was implemented within eight weeks using existing infrastructure and resources implementing the Scrum methodology. Twenty-three teleconsultations were scheduled and eight guides built. CONCLUSION: Scrum framework has a great potential to improve the training of students and to conclude pilot projects.
The fast spread of the COVID-19 epidemic has imposed unprecedented challenges to contemporary society, impacting several aspects of human life globally. Strict social distancing protocols had to be implemented in affected countries to contribute to reduce viral dissemination, and consequently, limit acquisition of new infections. Health systems were compelled to put emergency plans in place to cope with the overburden of potentially severe cases needing hospitalization, with the ultimate aim to preserve as many lives as possible. A auditoria clínica envolve um processo de verificação das práticas preconizadas e o cuidado prestado nas instituições de saúde implicando na qualidade do atendimento, neste contexto o trabalho tem como objetivo elaborar um protótipo, avaliar aplicabilidade e oportunidades de melhoria de um instrumento norteador para análise da eficácia da implantação de protocolos. Realizado em hospital pediátrico de ensino de grande porte entre abril e setembro de 2020, no município de São Paulo, através de levantamento bibliográfico e de protocolos institucionais, interações com profissionais da Instituição e um teste-piloto. O instrumento em sua versão validada foi baseado na estrutura comum de seis protocolos, constando de seis critérios correspondentes ao objetivo, abrangência, fluxo de atendimento, condutas adotadas, registros em prontuário e indicadores sendo as informações classificadas em conforme, parcialmente e não conforme. No processo de validação verificou a necessidade de instrumento específico para cada protocolo auditado. O instrumento elaborado atendeu ao objetivo proposto de nortear a prática de auditoria dentro do hospital e demonstrou conseguir em conjunto com específico agregar valor ao atendimento apontando possíveis melhorias.Palavras-chave: Auditoria Clínica; Qualidade da Assistência à Saúde; Indicadores de Qualidade em Assistência à Saúde; Protocolos Clínicos ABSTRACTGuiding instrument for clinical audit of protocolsThe clinical audit involves a process of verifying the recommended practices and the are provided in health institutions, implying in the quality of care, in this context the work aims to develop a prototype, evaluate applicability and opportunities for improvement of a guiding instrument for analyzing the effectiveness the implementation of protocols. Held in a large teaching pediatric hospital between April and September 2020, in the city of São Paulo, through a bibliographic survey and institutional protocols, interactions with professionals of the Institution and a pilot test. The instrument in its validated version was based on the common structure of six protocols, consisting of six criteria corresponding to the objective, scope, service flow, conduct adopted, medical records and indicators being classified as conforming, partially and non-conforming. In the validation process, he verified the need for a specific instrument for each audited protocol. Design Thinking é uma abordagem inovadora centrada em descobrir soluções mais aderentes às reais necessidades das pessoas. Possui a capacidade de utilizar a empatia, de forma a colocar as pessoas no centro do desenvolvimento do projeto. Para isso, é preciso se aprofundar sobre os problemas enfrentados pela instituição e através do processo de empatia, identificar suas dificuldades e ganhos. Com a intenção de aplicar a empatia como uma abordagem para resolução de problemas gerenciais do hospital, a pesquisa foi proposta com foco na comunicação institucional.
Objetivo: Desvelar a vivência de doadores no processo de doação para a realização do transplante de fígado intervivos (TxIV) em um hospital público. Método: Para compreender o vivencial dos doadores optou-se por realizar uma pesquisa com abordagem qualitativa, utilizando a vertente fenomenológica, segundo a modalidade estrutura do fenômeno situado. Para a realização da pesquisa foram entrevistados sete doadores, que já haviam concluído o processo de preparação e estavam aguardando a realização do procedimento. Resultado: A análise dos discursos evidenciou os seguintes temas: “a busca de uma solução para a doença do filho”, “o conhecimento da possibilidade de realização do TxIV”, “a decisão de doar”, “o preparo para a realização do TxIV”, “a espera para a realização do TxIV” e “a importância da família no processo”. Conclusão: A essência do fenômeno “a vivência de doadores no processo de doação para a realização do transplante de fígado intervivos” foi desvelada como sendo uma situação de buscas e dificuldades para encontrar a solução para a doença do filho, de certeza na decisão de doar parte do fígado para salvar o filho, de ansiedade no período de espera, de medo de perder o filho antes da realização do procedimento e de esperança no sucesso do transplante.
Background: The new coronavirus led the world into the pandemic known as COVID-19. Still, without any effective treatment, oxygen therapy is the most used treatment for patients. Without proper management of this medicinal gas, lives can be lost for lack of it. Aim: Find an indicator of a strong correlation with oxygen consumption. Methods: A single-center retrospective study, that evaluated the oxygen consumption billing data and patient-day indicator of Central Institute of HCFMUSP between 2019 and 2020. Results: A total of 380,245 patient days were analyzed. A strong correlation was identified between oxygen consumption and patient day with invasive mechanical ventilation (0.92). An average of 41.6 (± 7.8) cubic meters per patient day with invasive mechanical ventilation was found. Conclusion: There is a strong correlation between oxygen consumption in cubic meters and patient days with invasive mechanical ventilation at the Central Institute of HCFMUSP. The average values of consumption per patient day can help in planning oxygen management in other hospitals.
