CONTEXT AND OBJECTIVE: High blood pressure (hypertension) is the most frequent cause of morbidity and a major risk factor for cardiovascular complications. The aim here was to describe the prevalence of blood pressure greater than or equal to 140/90 mmHg in the adult Brazilian population and federal states, along with self-reported information about previous medical diagnoses of hypertension, use of medication and medical care for hypertension control. DESIGN AND SETTING: Cross-sectional study analyzing information from the National Health Survey of 2013, relating to Brazil and its federal states. METHODS:The sample size was estimated as 81,254 households and information was collected from 64,348 households. The survey consisted of interviews, physical and laboratory measurements. Systolic blood pressure was considered to be high when it was ≥ 140 mmHg and diastolic blood pressure, ≥ 90 mmHg. RESULTS: It was found that 22.8% of the population has blood pressure measurements ≥ 140/90 mmHg. The proportion was higher among men than among women: 25.8% versus 20.0%. The frequency increased with age, reaching 47.1% in individuals over 75 years and was highest in the southeast and south. 43.2% reported previous medical diagnoses of hypertension and, of these, 81.4% reported using medication for hypertension and 69.6%, going to the doctor within the past year for pressure monitoring, thus showing regular medical follow-up. CONCLUSION: These results are important for supporting measures for preventing and treating hypertension in Brazil, with the aim of achieving the World Health Organization's goal of reducing hypertension by 25% over the next decade.RESUMO CONTEXTO E OBJETIVO: Pressão alta (hipertensão) é a causa mais frequente de morbidade e importante fator de risco para complicações cardiovasculares. TIPO DE ESTUDO E LOCAL: Estudo transversal, analisando informações da Pesquisa Nacional de Saúde em 2013, referentes ao Brasil e Unidades Federadas. MÉTODOS: A amostra foi estimada em 81.254 domicílios e foram coletadas informações em 64.348 unidades domiciliares. A PNS consistiu em entrevistas, medidas físicas e laboratoriais. A pressão arterial foi considerada elevada quando a sistólica aferida ≥ 140 mmHg ou a pressão arterial diastólica ≥ 90 mmHg. RESULTADOS: Foi identificado que 22,8% da população tem pressão arterial medida ≥ 140/90 mmHg, sendo mais elevada em homens 25,8%, e 20,0% em mulheres. CONTEXT AND OBJECTIVE: Noncommunicable diseases (NCDs) are the leading health problem globally and generate high numbers of premature deaths and loss of quality of life. The aim here was to describe the major groups of causes of death due to NCDs and the ranking of the leading causes of premature death between 1990 and 2015, according to the Global Burden of Disease (GBD) 2015 study estimates for Brazil. DESIGN AND SETTING: Cross-sectional study covering Brazil and its 27 federal states. METHODS:This was a descriptive study on rates of mortality due to NCDs, with corrections for garbage codes and underreporting of deaths. RESULTS: This study shows the epidemiological transition in Brazil between 1990 and 2015, with increasing proportional mortality due to NCDs, followed by violence, and decreasing mortality due to communicable, maternal and neonatal causes within the global burden of diseases. NCDs had the highest mortality rates over the whole period, but with reductions in cardiovascular diseases, chronic respiratory diseases and cancer. Diabetes increased over this period. NCDs were the leading causes of premature death (30 to 69 years): ischemic heart diseases and cerebrovascular diseases, followed by interpersonal violence, traffic injuries and HIV/AIDS. CONCLUSION: The decline in mortality due to NCDs confirms that improvements in disease control have been achieved in Brazil. Nonetheless, the high mortality due to violence is a warning sign. Through maintaining the current decline in NCDs, Brazil should meet the target of 25% reduction proposed by the World Health Organization by 2025. RESUMO
Resumo O objetivo deste artigo é avaliar o desempenho da APS sob a perspectiva dos usuários e sua associação com as características sociodemográficas, condições de saúde autorreferidas e fatores de risco comportamentais para Doenças Crônicas Não Transmissíveis. Estudo transversal de base populacional, com dados do Inquérito Telefônico Vigitel 2015. Utilizou-se o “Primary Care Assessment Tool” versão reduzida. A população do estudo são adultos maiores de 18 anos que utilizaram os serviços da APS em Belo Horizonte nos últimos 12 meses (n = 872). Realizado o modelo de regressão logística múltipla para estimar o odds ratio. Observa-se que adultos sem plano de saúde têm 3,21 (IC95% 2,08-4,96) mais chances do que com plano de saúde de avaliarem a APS com alto escore (≥ 6,6), bem como adultos com baixa escolaridade tem 2,81 (IC95% 1,48-5,32) mais chances, os diabéticos têm 1,84 (IC95% 1,05-3,24) mais chances, os obesos tem 1,97 (IC95% 1,20-3,24) mais chances e os idosos tem 1,19 (IC95% 1,00-1,41) chances de reportar alto escore para a qualidade da APS do que os demais. A utilização do PCATool na versão reduzida em inquérito telefônico, mostrou-se nova possibilidade de avaliação do desempenho da APS e pode se tornar útil na gestão dos serviços de saúde.
Objetivo: analisar o perfil de utilização dos serviços de atenção primária à saúde (APS) e a associação com as características sociodemográficas, condições de saúde, os fatores de risco comportamentais para as doenças crônicas não transmissíveis (DCNT). Métodos: análise do modulo sobre a avaliação da APS incluído no inquérito telefônico Vigitel 2015. Foram entrevistados 2.006 adultos. Os critérios de elegibilidade foram adultos ≥18 anos que utilizaram a APS nos 12 meses anteriores à entrevista. Foram calculadas as razões de prevalência bruta e ajustada por escolaridade e raça/cor. Utilizou-se a regressão de Poisson para verificar a associação da utilização da APS com características sociodemográficas, condições de saúde e fatores de risco. Resultados: observou-se que a utilização da APS foi maior entre entrevistados sem plano de saúde (RP=1,76; IC95% 1,551,99); com baixa escolaridade, isto é, ≤8 anos de estudo (RP 1,59; IC95% 1,35-1,87), seguido de nove a 11 anos de estudos (RP 1,37; IC95% 1,16-1,61); e as mulheres (RP 1,34; IC95% 1,19-1,51). Usaram menos APS os entrevistados que referiram uso abusivo de álcool (RP=0,79; IC95% 0,66-0,95) e na faixa etária de 40 a 59 anos (RP 0,93; IC95% 0,88-0,99). Conclusão: o inquérito telefônico consiste em ferramenta útil para conhecer a realidade de saúde da população. O estudo atual pela primeira vez analisou módulo-piloto do Vigitel sobre uso de serviços de saúde e identificou que a utilização da APS foi mais frequente na população de baixa escolaridade, sem planos de saúde, mulheres, 40 a 59 anos e em uso abusivo de álcool.
Objective: to describe the first choice for use of health services and to analyze sociodemographic factors associated with seeking Primary Health Care (PHC) by the adult population of Brazil's Federal District in 2015. Methods: this was a population-based study, with 2,007 individuals, using a sample from the 2015 VIGITEL survey conducted by landline telephone, including questions on use of health services; we used logistic regression to identify factors associated with seeking PHC services. Results: participants' first choice was predominantly for private health services (57.6%), respondents reported seeking public health services less (39.5%), particularly PHC services (primary health centers: 24.6%); multivariate analysis showed that educational level (postgraduate [OR=0.15-95% CI 0.04;0.59] and high school [OR=0.37-95%CI 0.18;0.75]), and not having private health insurance (OR=27.77-95%CI 10.61;72.70) were variables associated with seeking PHC services. Conclusion: individuals with low educational level and without private health insurance are those who mostly seek PHC services as their first choice in Brazil's Federal District.
