Цель исследования-всесторонняя оценка нашего опыта непосредственного имплантационного протезирования с использованием нового способа «Трефойл», применяющегося при атрофии альвеолярной части нижней челюсти в боковых отделах. Материал и методы. Нами проведено клиническое и параклиническое обследование 29 пациентов (20 женщин и 9 мужчин). Возраст пациентов составил от 47 до 88 лет (среднее значение-65,45±9,7). Пациенты анкетировались с использованием анкет о состоянии здоровья и о качестве жизни-GOHAI (до лечения и спустя 6 нед после). Всем пациентам проводилось введение трех имплантатов Nobel Трефойл CC с усилием не менее 35 Н/см. В качестве каркаса конструкции «Трефойл» использовали стандартную балку (NobelBiocare). Для фиксации уточненного с помощью проверочной шины положения фиксирующих винтов в каркасе имплантационного протеза проводили микроплазменную сварку аппаратом Primotec Phazer mx (Германия) или «МОЛНИЯ 4.1» круговым способом. Для того чтобы сравнить результаты опросов GOHAI до и после операции, применили критерий t-Стьюдента для зависимых выборок. Были сформулированы статистические гипотезы: H0-результаты опроса GOHAI до операции не отличаются от результатов опроса GOHAI после операции; H1-результаты опроса GOHAI до операции отличаются от результатов опроса GOHAI после операций. Результаты. Через 18 мес сохранны были 98,85% имплантатов. В послеоперационном периоде спустя 6 мес мы измеряли пространство между телом протеза и слизистой оболочкой. Пространство оказывалось незначительным-от 1,23 до 3,45 мм (1,99±0,67 мм). Результаты опроса GOHAI после операции статистически достоверно отличаются от результатов опроса пациентов до операции. Если провести анализ средних значений по выборке, то можно утверждать, что средний результат по опросу GOHAI до операции (29,86) значительно ниже среднего по опросу GOHAI после операции (54,93). Заключение. Метод имплантационного протезирования «Трефойл» имеет высокие показатели сохраняемости имплантатов и протезов и дает хорошие результаты, обеспечивая высокий уровень качества жизни пациентов. Этот способ имплантационного протезирования является перспективным стандартом в геронтостоматологии, особенно для маломобильных пациентов.

Relevance. The studies which assess the quality of the implant-supported prostheses in elderly patients as well as the impact of the immediate loading on the peri-implant bone are scarce and inconclusive. Various automatic assessment software is known and applied in the clinic. A compact, informative and flexible software is obviously necessary for daily use and continuous control of the quality of the prosthetic treatment. The aim of studies was to improve the automatic quality assessment of implant prostheses in older adults and elderly patients.Materials and methods. 408 responders (151 male and 257 female subjects) aged from 54 to 85 (mean age 73.9 ± 4.9) participated in the clinical testing of the ROZA questionnaire. The conceptual (construct) validity included convergent, divergent and discriminant subtypes. In convergent validity, comparison with a battery of equivalent tests with similar requirements was used: Geriatric Oral Health Assessment Index (GOHAI), Oral Health Impact Profile-14 (OHIP-14) questionnaire, visual analogue scale (VAS) set and two related expert prosthesis quality assessment programs TREVOL and TRESIM. Results. Cronbach’s alpha, that confirms reliability and internal stability of the studied instrument, was 0.82 which is a good level. The correlation of the authors’ technique was low and insignificant (r = 0.242; p = 0.257) compared to the discriminant tests, which demonstrated the target focus of the studied technique.Conclusion. The studied questionnaire ROZA was highly sensitive, reliable for the examination of elderly patients with different implant-supported prostheses. It can be used to assess the quality of the prostheses in elderly, among others to evaluate the quality of the prosthetic treatment.
The aim of this study was the creation of a conceptual model of the standard implant prosthetics for edentulous patients using the data of comprehensive examination and implantation prosthetics in 372 patients aged 38 to 80 years (201 women, 171 men) with 582 implant supported prosthesis of various types, supporting on implants «Nobel Replace Select/Groove» and «Nobel Speedy Groove» (3675 implants). Clinical classification of implant-supported prosthesis for edentulous patients included 5 classes: class 1 (1-2 implants) included 6.2% of implant supported prosthesis, class 2 (3-4 implant) - 19.2% of prostheses, class 3 (5-6 implants) - 30.2% of replacement structures; IV (7-10 implants) - 44%, and V - 0,3%. Restoration class was age-dependent.
Relevance. In our clinical practice we’ve noticed a substantial amount of patients seeking prosthodontic treatment with the situation where all the teeth in dental arches (maxilla or mandible) have to be extracted due to distrophic, degenerative periodontal conditions. We started to define this condition as “decompensated dental arch” (compromised dentition). However, there is no information on treatment tactics in such patients.Purpose. To develop medical tactics in the dental treatment of patients with decompensated dental arches.Materials and methods. We examined 211 patients (74 males, 134 females) aged from 54 to 83 years old (mean age: 72.4 ± 6.3) with 275 decompensated (compromised) upper (73) and lower (202) dental arches. We used clinical and paraclinical (additional) diagnostic methods such as ultrasound, Doppler, electromyography, CT scan, OPG evaluation.Results. Comprehensive description of “decompensated dentition” (compromised)lead us to implementation of radical treatment approach during preparation for prosthodontic rehabilitation. First of all that approach facilitates quick transition from handicapped condition to the high quality of life in case of applying principals of immediate (including implant supported) prosthetics. Clinical experience proves that “Merciful” (tooth saving) tactics with the attempt to relative saving of some teeth with conservative approach will prolong the agony of decompensated dentition (compromised) especially in case of treating elderly patients with compromised medical condition. Radical approach helps to get long term survival of implants and prostheses.Conclusion. That’s the reason that in cases of clinical situation of decompensated dentition (compromised) we have to focus on more radical preparation to prosthodontic -especially implant supported – rehabilitation.
