Relevance. The success of implant-supported prostheses depends on the quality of the jawbone. Traditionally, it is assessed radiographically, but this method is not only invasive but also unreliable and inaccurate for predicting the outcome of treatment.Material and methods. The study included 80 patients (49 women and 31 men) with a mean age of 71 ± 7 years, which formed four groups. Group A (control group, n = 20) consisted of patients with healthy periodontium; comparison group B, n = 20, comprised patients with terminal dentition; the main group C (n = 20) included patients with extended rehabilitation, fixed 7-10 days before; group G (n = 20) was composed of patients with “Trefoil” implant-supported prostheses, fixed three years earlier. The blood flow of peri-implant tissues was assessed using ultrasound Doppler flowmetry (UDF). All patients (n = 20) underwent dual-energy X-ray absorptiometry (DXA) before the prosthetic treatment.Results. The analysis of pre-prosthetic-treatment ultrasound Doppler flowmetry results showed low values of microcirculation in the alveolar ridge mucous membrane in patients with terminal dentition compared with the control group. On the 7th day after implant-supported prosthetic treatment, group C demonstrated an increase in microcirculation by 11.42% compared to the control group and by 147.36% compared to group B. Three years after implant-supported prosthetic treatment, the ultrasound data revealed a statistically significant increase in blood flow velocity 0.342 ± 0.04 (cm/s) (p < 0.01). The Pearson coefficient determined a high correlation between T-scores of DXA and ultrasound Doppler flowmetry data (r = 0.829, p = 0.0001).Conclusion. Ultrasound Doppler flowmetry (UDF) can be the main method for studying the peri-implant tissue condition at various stages of implant-supported prosthetic treatment.

Currently, there are two dental approaches to the oral rehabilitation of elderly patients before implant prosthetics. One of them is conservative, based on a purely sparing approach, inspired by the classical dental prosthetics canons with commonplace replacement structures, the other is radical, empirically confirmed by unfavourable long–term prosthetic results, taking into account chronic diffuse periodontitis being a relative contraindication for implantation due to the abundance of periodontogenic and anaerobic flora in the oral cavity and in the periapical foci, as well as – polymorbidity of the elderly. This increases the risk of mucositis, peri-implantitis, leading to failure of implants, replacement of prostheses within warranty periods, and repeated prosthetics. The purpose of this work was to create a measuring instrument for a target preliminary choice of an alternative from two available approaches to oral health improvement in elderly patients before implant prosthetics. The object of the study was 10 expert software programs for assessing the quality of various dental and jaw prosthesis designs, including extended implant restorations with a few artificial supports. The methods and criteria used in these systems were studied. A portable computer program has been created that allows making a target choice between conservative, radical and moderately radical approaches to preparing the oral cavity of elderly patients for immediate implant prosthetics.The program is used to assess the morphofunctional state of those teeth whose prognosis is doubtful, and the indications for extraction are not absolute. The program implements a scoring system for evaluating the answers received according to a questionnaire of 20 criteria with the possibility of automatic summation and the issuance of a final conclusion.

Background. The special status and position of older and oldest old in the therapeutic, diagnostic, and preventive activities of a dentist lead to a firm conviction of an urgent necessity and expediency of creating a new medical specialization — geriatric dentistry.Aim. Based on many years of clinical experience and scientific research, including extensive use of high-tech oral prosthetics for older adults, to make the first attempt at creating an optimal therapeutic resource for a new medical specialization — geriatric dentistry.Materials and methods. Examination and prosthetics were performed on 645 patients aged 60 to 85 years old, with various types of implantable prostheses in the period o 3 to 7 years after their implantation. In addition, 903 implantable prostheses and 3287 of their artificial supports (implants) were also examined.Results. The mean age of patients who visited the orthopedic dentistry department for dental prosthetics was 64.72 ± 6.8 years old. As the main method of rehabilitation of older patients’ masticatory and speech apparatus, we have used implant-supported prosthesis with its digital transformation. Most of the patients underwent reasonably radical oral cavity preparation for prosthetics. The original state retention of the tissues of the implant bed ranged from 78.04 to 100%. At the same time, fixed structures of implantable prostheses were characterized by higher indicators. The preservation of the implants was from 96.8 to 100%, depending on the observation period.Conclusion. The high values of implant and prosthesis retention rates, good condition of the peri-implant mucous membrane and bone in remote periods after prosthetics, as well as a high level of patient satisfaction and significantly higher achieved level of their quality of life, along with high medical and economic efficiency of the method, allow us to consider it a promising standard of geriatric dentistry. Thus, a modern therapeutic, or rather, rehabilitation, resource for effective pre-prosthetic preparation and implant prosthesis has been developed and justified, which is the optimal approach in geriatric dentistry.
