Objective: To calculate the cost and assess the results on implementing technological resources that can prevent medication errors. Methods: A retrospective, descriptive-exploratory, quantitative study (2007-2015), in the model of case study at a hospital in the Brazilian Southeastern Region. The direct cost of each technology was calculated in the drug chain. Technological efficacy was observed from the reported series of the indicator incidence of medication errors. Results: Thirteen technologies were identified to prevent medication errors. The average cost of these technologies per year in the prescription stage was R$ 3.251.757,00; in dispensing, R$ 2.979.397,10; and in administration, R$ 4.028.351,00. The indicator of medication error incidence decreased by 97.5%, gradually between 2007 to 2015, ranging from 2.4% to 0.06%. Conclusion: The average cost per year of the organization to implement preventive technologies in the drug chain totaled up R$ 10.259.505,10. There was an average investment/year of R$ 55,72 per patient and its association with smaller indicator of incidence of medication errors confirms a satisfactory result in this reported series regarding such investment.
Custo do erro de medicação e eventos adversos à medicação na cadeia medicamentosa: uma revisão integrativa Cost of the medication error and adverse drug events in the medication therapy chain: review literature as a topic
Objetivos: Mapear e descrever as atividades e riscos de um processo cirúrgico em um hospital especializado, e propor estratégias para a segurança do paciente por meio de uma proposta de gestão de risco. Metodologia: Pesquisa qualitativa, na modalidade de pesquisa-ação, realizado com 12 profissionais atuantes em um centro cirúrgico de um hospital especializado do noroeste paulista, no período de maio a junho de 2017. Foram utilizadas as ferramentas brainstorming para conhecimento do fluxo interno, Business Process Model and Notation e o Supplier, Input, Process, Output e Cliente para mapeamento do processo, Matriz de Risco da Agência Nacional de Saúde Suplementar para a estratificação dos riscos e para as propostas de melhorias, utilizou-se a metodologia proposta pelo Institute of Healtcare Improvement. Resultados: No mapeamento, o centro cirúrgico foi dividido em admissão; sala cirúrgica e recuperação anestésica, que resultaram em 13 processos e 42 atividades descritas. Foi possível observar interações relevantes com processos administrativos e de apoio. Foram identificadas 42 situações de risco; sendo que 31,8% foram classificadas como risco médio, 43,1% alto e 25,0% extremo, que geraram 87 propostas de intervenções para a melhoria da segurança do paciente. Conclusão: Mapear as atividades e os riscos dos processos cirúrgicos instrumentaliza o gestor quanto à sua tomada de decisão; sobretudo, a adoção de estratégias corretivas para cada um dos pontos de risco no fluxo do atendimento ao paciente.
Introdução: O ato cirúrgico é um procedimento relevante para a recuperação e manutenção da saúde, uma vez que, após tal acontecimento, é esperado que o cliente evolua com uma melhor qualidade de vida. Objetivo: Investigar a taxa, causas e especialidades responsáveis pelos cancelamentos cirúrgicos. Métodos: Estudo quantitativo, retrospectivo de janeiro a dezembro de 2016, realizado em hospital de ensino. Foi adotada a fórmula proposta pelo Compromisso com a Qualidade Hospitalar (CQH). Resultados: O indicador de qualidade variou de 5,9% a 11,5%. As principais causas de cancelamentos relacionados à instituição foram divididas entre o Sistema Único de Saúde (SUS) que apresentou a falta de tempo hábil (38,7%) e a troca pela equipe entre pacientes devido à gravidade (16,1%) e o Sistema de Saúde Suplementar (SSS), com a não autorização pela operadora de saúde (55,1%). Em relação aos motivos dos pacientes encontrou-se no SSS e SUS a falta de internação (SUS - 26,4% e SSS - 29,3%) e falta de condições clínicas do paciente (SUS - 25% e SSS - 18,4%). Em relação a especialidade médica, 83,3% não foi identificada, seguida da equipe da ortopedia com 4,1%. Conclusão: O indicador de cancelamento cirúrgico mostrou-se elevado nesse período do estudo, quando comparado a CQH, e a especialidade foi ortopedia.Palavras-chave: centro cirúrgico, suspensão de tratamento, enfermagem de centro cirúrgico.
Introdução: Todo cidadão brasileiro tem direito a saúde, no entanto, algumas medicações não estão disponíveis pelo Sistema Único de Saúde e, para acessá-las, o cidadão pode precisar judicializar o sistema. Objetivo: Descrever o perfil demográfico e clínico de demandantes e os custos dos medicamentos judicializados em um município brasileiro. Métodos: Estudo descritivo, transversal com abordagem quantitativa, realizado a partir de dados de 2017 disponibilizados pelo Departamento de Saúde de um município de pequeno porte do Sudeste brasileiro. Resultados: Foram requisitadas 140 medicações por 63 indivíduos a partir de 67 mandados de segurança. Dentre eles, a maioria era mulheres (52,4%), com idade média de 51 anos; a principal patologia foi diabetes mellitus (26,9%) e os medicamentos mais solicitados foram antidiabéticos (21,9%). Os mandados mais custosos foram referentes à purpura trombocitopênica (R$ 78.000,00; US$ 24,451.41) e o menos custoso foi referente à especialidade vascular (R$168,00; US$52.66). O custo total dos medicamentos foi R$362.519,70 (US$113,642.53). Conclusão: Considerou-se alto o gasto de R$21,32 (US$ 6.68) /habitante, um valor que sobrecarrega e onera o Sistema Público de Saúde. Assim, as informações geradas por esta pesquisa suscitam medidas de promoção e prevenção por meio da atenção básica que podem reduzir esses gastos.
