BackgroundPreterm birth (PTB) is a major determinant of neonatal morbimortality with adverse consequences for health. The causes are multifactorial, with intrauterine infection probably explaining most of these outcomes. It is believed that infection with Chlamydia trachomatis (CT) is also involved in PTB and premature rupture of membranes.ObjetivesTo evaluate the prevalence of and associated factors for CT among cases of PTB attended at a University Hospital in Vitoria, Brazil.MethodsA cross-sectional study performed among parturient who had preterm birth from June 2012 to August 2013 in Vitoria, Brazil. Participants answered a questionnaire including demographic, behavioral, and clinical data. A sample of urine was collected and screened for CT using polymerase chain reaction. Chi-square tests were used for proportion differences and Student’s-t tests and variance analysis were used for testing differences between mean values. Odds ratio was used as a measure of association with a 95% confidence interval.ResultsThe prevalence of PTB during the period of the study was 26% and the prevalence of CT among them was 13.9%. A total of 31.6% pregnant women were younger than 25 years old and women infected by CT were even younger than women not infected by CT (p = 0.022). Most of them (76.2%) were married or had a living partner, and CT infection was more frequent among the single ones (p = 0.018); 16.7% of women reported their first sexual intercourse under 14 years old. The causes of prematurity were maternal-fetal in 40.9%; rupture of the membranes in 29.7% and premature labor in 29.4%. In multivariate analysis, being married was a protective factor for infection [OR = 0.48 (95%CI:0.24–0.97)]. None of the other characteristics were associated with CT infection.ConclusionsThis study shows a high prevalence of CT infection among parturient who have preterm birth. This high prevalence highlight the need for defining screening strategies focused on young pregnant women in Brazil.

Obsessive-compulsive disorder (OCD) is a prevalent psychiatric disorder of unknown etiology. However, there is some evidence that the immune system may play an important role in its pathogenesis. In the present study, two polymorphisms (rs1800795 and rs361525) in the promoter region of the cytokine tumor necrosis factor-alpha (TNFA) gene were genotyped in 183 OCD patients and in 249 healthy controls. The statistical tests were performed using the PLINK® software. We found that the A allele of the TNFA rs361525 polymorphism was significantly associated with OCD subjects, according to the allelic χ2 association test (p=0.007). The presence of genetic markers, such as inflammatory cytokines genes linked to OCD, may represent additional evidence supporting the role of the immune system in its pathogenesis.

Background and Aim: Progression of endoscopic diagnosis contributes improvement of detecting early gastric cancer (EGC). Endoscopic resection like endoscopic submucosal dissection (ESD) achieves surefire treatment of EGC. Now metachronous gastric cancer (MGC) after ESD becomes a problem. We analyzed the patients who had MGC after ESD to clarify the feature of MGC. Methods: We conducted retrospective analysis of EGC cases resected by ESD in our institution. From April 2014 through August 2019, 393 patients were performed ESD and 38 cases (9.7%) were MGC including 10 synchronous (within 1 year after ESD). We analyzed age, sex, interval of recurrence, location, macroscopic type, histological type, size of tumor, depth of invasion, history of eradication of Helicobacter Pylori (HP), detecting method and curative ratio by ESD. The study was approved by IRB. Result: The outcomes were as follows: MGC including synchronous was 9.7% (38/393) during this surveillance. Sex: male/female ratio was 35/3. Average age: 74.0AE4.7 years old (61-86, median 74). Average interval of recurrence: 45.7AE41.6 months (2-168, median 33.5). 68% (26/38) of cases experienced a recurrence within 5 years after previous ESD. The maximum recurrence was 3 times. Location: Upper 19, Middle 4, Lower 15. 76% (29/38) of cases arose from different location from the previous one. Macroscopic type: 0-IIa 12, 0-IIb 4, 0-IIc 22. 42% of cases were same type as previous one. Histological type: 37 tubular adenocarcinoma and 1 poorly differentiated adenocarcinoma. Average size: 11.4AE7.8 mm (1.5-37, median 10). Depth of invasion: M 36, SM 2. History of eradication of HP: 53% (20/38). In those cases, the longest interval of recurrence was 12 years and the shortest one was 2 years. Detecting method: white-light imaging 74%, indigo carmine dye 26%. Narrow-band imaging (NBI) or magnifying endoscopy with NBI was not used at the first detection of EGC. Curative ratio by ESD: 97% (37/38). Almost cases were less than 30mm tubular adenocarcinoma existed in mucosal layer, therefore, they were curative. Just one case (por, SM1) was not curative by ESD. Conclusion: In our analysis, MGC after ESD was 9.7%. The majority of cases were curative by ESD. 53% of cases had history of eradication of HP. They had severe atrophic gastritis and metaplasia. Even after eradication of HP, the patients performed ESD for EGC also still need continuous periodic endoscopic examination. To make sure effectivity of eradication of HP after ESD for EGC, additional prospective analysis is necessary.

Introduction and importance Esophagectomy for esophageal cancer is one of the most challenging surgical procedures, with high rates of morbidity, especially from respiratory complications. SARS-COVID19 represents a health threat nowadays. Peri-operative SARS-COVID19 infection after esophagectomy might negatively affect the postoperative outcomes. The use of tocilizumab as an alternative to reduce the inflammatory response in SARS-COVID19 is an option that has not been described in the literature after esophagectomy. Case presentation A SARS-COVID19-vaccinated (CORONAVAC) 73-year-old man with pulmonary emphysema, coronary artery disease, previous asymptomatic pulmonary embolism, and adenocarcinoma of the esophagogastric junction tumor was submitted to laparoscopic transhiatal esophagectomy (ypT2N0M0) after perioperative neoadjuvant chemotherapy. He was also infected with SARS-COVID19, confirmed by PCR test at the 14th postoperative day. During follow-up, mild hypoxemia persisted without evidence of infection except for SARS-COVID19, and a high-flow cannula was required to maintain oxygenation. Tocilizumab was administered following high parameters of a high-flow cannula, and invasive mechanical ventilation was avoided. Discussion Besides of the risk of secondary infection, after administration of tocilizumab, the parameters of oxygen supplementation were systematically reduced, and he stayed in the ICU for seven days. He was discharged from the ward six days later. He developed late cervical anastomotic leakage, which was treated with conservative therapy. Conclusion Although the patient had high-risk comorbidities, esophagectomy, and SARS-COVID19 infection, the use of tocilizumab was safe and improved the pulmonary recovery.
