Modelo do estudo: Estudo experimental. Introdução: A Doença Renal Crônica (DRC) refere-se a um diagnóstico sindrômico de perda progressiva e irreversível da função renal. O paciente submetido à hemodiálise pode apresentar limitações na capacidade funcional, função pulmonar e força muscular respiratória, com consequentes prejuízos na qualidade de vida. Objetivo: Avaliar os efeitos de um programa de exercício físico sobre a função pulmonar, capacidade funcional, qualidade de vida e dor, em pacientes que realizam hemodiálise. Metodologia: Participaram do estudo 28 pacientes de ambos os sexos, com idade entre 40 e 60 anos, em programa de hemodiálise no Instituto do Rim da Santa Casa de Misericórdia de Presidente Prudente-SP. A força muscular respiratória foi avaliada pela manovacuometria, a capacidade funcional, pelo TC6', a qualidade de vida, pelo questionário KDQOLSF, a função pulmonar, pela espirometria e a dor, pela EVA. Após as avaliações, os pacientes iniciaram o programa de exercícios, que foi desenvolvido três vezes por semana, durante 40 minutos em hemodiálise, por oito semanas. Ao final do programa, os pacientes foram reavaliados. Resultados: Não houve diferença significativa dos valores da CVF e VEF1 pré e pós-programa de exercícios, assim como do Índice de Tiffenau. O valor da PImax pós-programa foi significativamente maior que o obtido na avaliação pré-programa. Para a variável PEmax, não foi encontrada diferença significativa. As avaliações da capacidade funcional inicial e final não apresentaram diferenças significativas (p>0,05). A avaliação da qualidade de vida, quanto aos domínios das áreas específicas da DRC, mostrou que houve significância estatística, ao comparar a lista de sintomas e problemas com a sobrecarga da DRC e papel profissional. Os indicadores relativos à dor foram reduzidos, após o programa (p<0,05). Discussão: O DRC enfrenta situações complexas de dependência física, social e financeira. Apesar de não apresentar resultados estatisticamente significativos em todas as variáveis avaliadas, este estudo, corroborando outros encontrados na literatura, sugere um programa de exercício físico, com aspectos positivos para essa população. Conclusão: Embora a capacidade pulmonar e a capacidade funcional (TC6') não tenham apresentado alterações ao final do experimento, os níveis reduzidos de dor, cansaço e dispneia sugerem melhora do desempenho funcional, após programas de exercício físico para DRC.
RESUMOA incidência global de doenças renais crônicas vem aumentando a cada dia. No Brasil, aproximadamente 54,5 mil pessoas se encontram em tratamento dialítico. A doença renal crônica, diagnóstico sindrômico, progressivo e irreversível da função renal, culmina invariavelmente na terapia dialítica. Apesar dos avanços tecnológicos e dos benefícios no incremento da sobrevida, a hemodiálise acarreta complicações e alterações funcionais que prejudicam a qualidade de vida desses sujeitos. O presente estudo buscou avaliar e comparar a qualidade de vida de pacientes submetidos à fisioterapia com controles. Foram selecionados sessenta doentes renais crônicos de ambos os gêneros. Aplicou-se o questionário de qualidade de vida Kidney Disease Quality of Life Short Form para avaliar dois grupos de pacientes: um controle, sem tratamento fisioterapêutico, e um que realizava o tratamento fisioterapêutico. Os resultados demonstraram comprometimento relacionado à faixa etária, tempo de hemodiálise e gênero em grande parte das dimensões analisadas em ambos os grupos; porém, quando se fez a análise entre os grupos, pacientes que realizaram fisioterapia obtiveram melhores índices de qualidade de vida. Concluiu-se que a fisioterapia contribui para uma tendência de melhora geral da qualidade de vida de pacientes renais crônicos submetidos à hemodiálise.
ResumoProcedimentos cirúrgicos torácicos podem alterar a mecânica respiratória, repercutindo na função pulmonar. A presença de profissionais fisioterapeutas é fundamental no preparo e na reabilitação dos indivíduos que são submetidos à cirurgia cardíaca, visto que dispõem de um grande arsenal de técnicas. O objetivo foi verificar a efetividade de exercícios respiratórios, com e sem a utilização de dispositivos, e o treinamento muscular respiratório pré-cirurgia cardíaca na redução das complicações pulmonares pós-operatórias. Mesmo existindo controvérsias a respeito de qual técnica utilizar, estudos demonstram a eficácia da fisioterapia respiratória pré-cirúrgica na prevenção e na redução de complicações pulmonares pós-operatórias. A natureza progressiva da doença renal crônica (DRC) gera um desequilíbrio e disfunção a longo prazo, que leva a limitações físicas e psicológicas, alterando a qualidade de vida desses doentes. O objetivo foi avaliar a percepção do estado de saúde e da qualidade de vida relacionada com a saúde em doentes com insuficiência renal crónica em diálise. Participaram da pesquisa 27 indivíduos. A qualidade de vida foi avaliada pelo instrumento Kidney Disease and Quality-of-Life Short Form (KDQOL-SF). Mostraram-se muito alterados os domínios relacionados ao papel profissional, funcionamento físico, vitalidade e função mental. A natureza progressiva da doença renal crônica (DRC) gera um desequilíbrio e disfunção a longo prazo. A presença de distúrbios da mecânica respiratória, limitação do fluxo aéreo distal e redução da capacidade de difusão pulmonar é freqüente nessa população. O objetivo foi avaliar os efeitos de um programa de exercício durante oito semanas sobre a Capacidade Vital (CVF), Volume Expiratório Forçado no Primeiro Segundo (VEF1) e Índice de Tiffeneau (VEF1/CVF). Participaram da pesquisa 20 indivíduos. A função pulmonar foi avaliada através da espirometria. Para análise estatística foi utilizado o Teste de Kurskal-Wallis. Para localizar a diferenças entre as variáveis foi aplicado o Teste Post Hoc de Bonferroni. A espirometria mostrou redução significativa dos valores espirométricos (CVF e VEF1) em relação aos valores preditos (p=0,0001). Não houve mudança significante entre esses valores pré e pós-exercício. O índice de Tiffeneau não mostrou diferença estatística em nenhum momento. Concluímos que programas de exercício aeróbico com pacientes em hemodiálise não produzem efeito significativo sobre a função pulmonar.Palavras-chave: doença renal crônica, função pulmonar, exercício.
